[Lumbosacral plexopathy caused by aneurysms of the abdominal aorta].

We report three patients who presented pain in the sciatic-crural region as presenting feature of an aneurysm of the abdominal aorta. The first patient developed a fullblown plexopathy with bad condition due to a large aneurysm of common iliac artery. The remaining two patients had abdominal aneurysms each that manifested as femoral neuropathy in one case and as L5-S1 radiculopathy in the other patient. The mechanism of neural involvement is discussed and those clinical and radiologic findings which may prompt us to think of such etiology in front of painful clinical pictures in this region ar…

Common peroneal and posterior tibial ischemic nerve damage, a rare cause.

A 56-year old gentleman presented to our orthopaedic foot and ankle clinic, with unusual symptoms in his left foot. He described a tight sensation over his toes, "like sandpaper under his skin". The pain had started post operatively following a bilateral aorto-femoral bypass. He was subsequently investigated and found to have an ischemic lesion Identified in his common peroneal and posterior tibial nerve with associated muscle atrophy on EMG. This represents a previously unreported complication of aorto-femoral bypass surgery.

An Alternate Approach to Using Candy Cane Stirrups in Vaginal Surgery.

BACKGROUND: Candy cane stirrups are widely used for vaginal surgery because they provide sufficient operating space. When placed in the standard perpendicular alignment, however, these stirrups cause excessive flexion, abduction, and external rotation of the thigh, which may injure the femoral, lateral femoral cutaneous, sciatic, and common peroneal nerves. TECHNIQUE: We corrected this deficiency by positioning the stirrups parallel to the operating table, just above and tilting slightly toward its lower break. After placing the patient in stirrups, we inserted a double-layer foam pad between

Effect of lithotomy positions on strain of the obturator and lateral femoral cutaneous nerves.

The purpose of this study was to determine if various lithotomy positions increase strain on the obturator and lateral femoral cutaneous nerves in fresh adult cadavers. A static load cell was used to record strain changes of the obturator and lateral femoral cutaneous nerves in the pelvis and anterior thigh when the lower limbs were placed in three sets of positions of varying hip abduction and flexion. The means of the strain measurements, which were measured in grams in all positioning angles were compared to the baseline 0 degrees measurements. Analysis of variance was calculated for the di

Selective MR neurography-guided lumbosacral plexus perineural injections: techniques, targets, and territories.

The T12 to S4 spinal nerves form the lumbosacral plexus in the retroperitoneum, providing sensory and motor innervation to the pelvis and lower extremities. The lumbosacral plexus has a wide range of anatomic variations and interchange of fibers between nerve anastomoses. Neuropathies of the lumbosacral plexus cause a broad spectrum of complex pelvic and lower extremity pain syndromes, which can be challenging to diagnose and treat successfully. In their workup, selective nerve blocks are employed to test the hypothesis that a lumbosacral plexus nerve contributes to a suspected pelvic and extr

Meralgia Paresthetica: A Case Report With an Update on Anatomy, Pathology, and Therapy.

Meralgia paresthetica, a condition characterized by tingling, numbness, and burning pain in the lateral aspect of the thigh, is caused by compression of the lateral femoral cutaneous nerve. The incidence of meralgia paresthetica increases with obesity and diabetes. The unique anatomy of the nerve that tunnels through the inguinal ligament predisposes it to inflammation, trauma, and entrapment. The pathology of meralgia paresthetica parallels that of entrapment neuropathies but with additional inflammatory overlay in certain instances. The clinical diagnosis is relatively simple due to its uniq

Incidence and Clinical Course of Limb Dysfunction Post Cardiac Catheterization - A Systematic Review.

BACKGROUND: We systematically reviewed the available literature on limb dysfunction after transradial access (TRA) or transfemoral access (TFA) cardiac catheterization. METHODS AND RESULTS: MEDLINE and EMBASE were searched for studies evaluating any transradial or transfemoral procedures and limb function outcomes. Data were extracted and results were narratively synthesized with similar treatment arms. The TRA group included 15 studies with 3,616 participants and of these 3 reported nerve damage with a combined incidence of 0.16% and 4 reported sensory loss, tingling and numbness with a pool

Segmental zoster paresis of the lower extremity: Case report.

Herpes zoster (HZ) is a segmental vesicular eruption, pain, and sensorial symptoms. Segmental motor weakness can rarely be seen as a complication of HZ. Here, we present two cases of motor paresis associated with HZ, case 1 was L2 and L3 segmental motor paresis with femoral neuropathy and case 2 was L5 and S1 segmental motor paresis with sensorial ganglion involvement. In both cases after electrotherapy, exercise program, and medication for pain, there were no motor weakness and pain. Zoster motor paresis is a rare complication that responds to treatment and physicians should be careful about …

[Iliac haematoma as a differential diagnosis to lumbar disc herniation].

After a fall without fracture an 83-year-old man who was treated with warfarin was admitted with severe groin pain on the left hip. A few days later he had reduced strength in hip flexion and knee extension, absent patellar tendon reflex, and decreased sensibility of the anterior thigh and the medial lower leg. A magnetic resonance imaging revealed a large haematoma in the left iliac muscle. Iliac haematoma-induced femoral nerve compression neuropathy is a rare condition but should be considered as a differential diagnosis for L4 root compression in patients, who are receiving anticoagulant th

Correlation between diabetic lower-extremity arterial disease and diabetic neuropathy in patients with type II diabetes: an exploratory study.

The lower-extremity vascular injuries and neuropathy are the most salient complications of diabetes which could lead to the poor prognosis, especially for the type II diabetes. The lower extremity vascular injuries and neuropathy usually coexist, yet their correlation in the pathogenesis of lower extremity lesions has received little attention in previous studies. To investigate the correlation between the degree of lower-extremity arterial injuries and lower-extremity neurological functional status in patients with type II diabetes, 32 patients with type II diabetes were examined for the mean

Treatment options for persistent lateral femoral cutaneous nerve lesions after total hip arthroplasty via the direct anterior approach: retrospective analysis with clinical assessment.

PURPOSE: The direct anterior approach (DAA) in total hip arthroplasty (THA) has a risk of lateral femoral cutaneous nerve (LFCN) injury. Long-term outcomes and therapeutic options for such injuries are poorly investigated. This study evaluates the impact of iatrogenic LFCN lesions on long-term outcomes and investigates treatments like ultrasound-guided nerve infiltration or neurolysis. METHODS: Our institutional database of primary THAs (2014-2022) was searched for patients with iatrogenic LFCN lesions after DAA, confirmed via ultrasound or electroneurography. First, retrospective analysis of

Surgical decompression of the lateral femoral cutaneous nerve (LFCN) for Meralgia paresthetica treatment: Experimental or state of the art? A single-center outcome analysis.

Meralgia paresthetica (MP) is a rare lateral femoral cutaneous nerve-(LFCN)-mononeuropathy. Treatment for this disorder includes conservative and operative approaches; the latter is considered if conservative therapy fails. The most commonly used surgical approaches are decompression/neurolysis and avulsion/neurectomy. However, there are no definitive guidelines on the optimal surgical approach to be used. The purpose of this study was to evaluate the outcome of surgical decompression of the LFCN for the treatment of persistent MP with preservation of sensation along the distribution of the LF

Anatomical variation of the lateral femoral cutaneous nerve: a case report and review of the literature.

We describe a rare anatomical variation of the lateral femoral cutaneous nerve (LFCN) in a 29-year old female patient discovered during a surgical decompression of the LFCN for meralgia paresthetica. Anatomical variations of the LFCN must always be considered at the time of surgery to maximize success rate and avoid nerve damage during surgical dissection.

Workup and Management of Persistent Neuralgia following Nerve Block.

Neurological injuries following peripheral nerve blocks are a relatively rare yet potentially devastating complication depending on the type of lesion, affected extremity, and duration of symptoms. Medical management continues to be the treatment modality of choice with multimodal nonopioid analgesics as the cornerstone of this therapy. We report the case of a 28-year-old man who developed a clinical common peroneal and lateral sural cutaneous neuropathy following an uncomplicated popliteal sciatic nerve block. Workup with electrodiagnostic studies and magnetic resonance neurography revealed i

The free vascularised iliac crest tissue transfer: donor site complications associated with eighty-two cases.

Seventy-eight patients who had undergone a total of 82 free vascularised iliac crest tissue transfers were reviewed to determine the incidence of donor site complications. The most frequent problems encountered were early postoperative pain and long term sensory changes. Major complications such as femoral neuropathy and incisional hernia formation were encountered infrequently. More serious potential complications are discussed. In general, the functional loss associated with the free vascularised iliac crest tissue transfer was found to be acceptable, but the inclusion of a skin paddle was n…

Inadvertent Prolonged Femoral Nerve Palsy After Field Block with Liposomal Bupivacaine for Inguinal Herniorrhaphy.

Inguinal herniorrhaphy is a common outpatient procedure where analgesia can be augmented with local infiltration. We report a case of prolonged femoral nerve palsy secondary to liposomal bupivacaine use during wound infiltration after inguinal herniorrhaphy. Inadvertent transient femoral nerve palsy is a rare but known complication after ilioinguinal field block. This case both highlights the value of ultrasound imaging in evaluating the complications and demonstrates how the prolonged nature of liposomal bupivacaine can prolong adverse sequela.
