Iron metabolism was studied in 10 patients with malignant histiocytosis (MH), in 16 patients with histiocytosis-X (Langerhans cell histiocytosis) and in 34 patients with reactive proliferation of the mononuclear phagocytes (MPS). Eight MH patients had a considerably increased level of serum ferritin (SF). The average level of SF was 6070 +/- 957 mg/l for MH patients, which is significantly greater than the SF level for HX and RH patients. Liver and spleen iron concentrations, serum ferritin level and binding of S-ferritin to concanavalin A (Con A) were measured in 12 patients with thalassaemia major or intermedia at the time of splenectomy. All these subjects had increased liver iron concentration, most of them had hepatic fibrosis but none of them had histological evidence of chronic hepatitis. No patient had ascorbic acid deficiency. Serum ferritin concentration was increased in all cases, ranging from 266 to 5504 micrograms/l. In all but 2 subjects most of the protein did not bind to Con A, thus behaving as tissue ferritin.

Serum ferritin estimation was compared with bone marrow hemosiderin staining in the primary diagnosis of iron deficiency and overload in 19 patients on chronic hemodialysis. Serum ferritin has been shown to be an excellent determinant of iron stores. In a consecutive group of women registering at the regular prenatal clinic, we measured serum ferritin, iron, iron-binding capacity, and hemoglobin to determine their hematologic status as to anemia. It was found that serum ferritin is the most sensitive determinant of depleted iron stores, with serum iron being next in sensitivity. This assay is a rapid, economic, sensitive measure of iron stores, and results are not altered significantly by other types of anemia or oral iron therapy.

Serum ferritins from a patient with haemochromatosis and from a patient with transfusional siderosis were compared with tissue isoferritins on the basis of their iron content, isoferritin spectrum and immunological properties. Both serum ferritins had a low iron content and corresponded to only the most basic isoferritins in liver. The serum ferritins were very similar to the natural apoferritin from liver in all respects.

Homozygous beta-thalassaemia is a disease in which there is a progressive iron overload from infancy to death in early adulthood. This study investigated the protective effect of nuclear factor erythroid 2-related factor 2 (Nrf2) against diquat-induced acute kidney injury (AKI) by regulating mitochondrial ferritin (FtMt). A model of kidney injury was established in male Wistar rats by oral gavage of diquat. The rats were randomly assigned to four experimental groups: control, diquat, diquat + Liproxstatin-1 (Lip-1, a ferroptosis inhibitor), and diquat + Sulforaphane (SFN, an Nrf2 activator).
