Looking further upstream to prevent fetal alcohol spectrum disorder in Canada.

Half of all pregnancies in Canada are unintended. Whether a pregnancy is intended or unintended has a bearing on the risk of prenatal alcohol exposure. Research indicates that women who experience an unintended pregnancy are significantly more likely to consume alcohol while pregnant. Most fetal alcohol spectrum disorder (FASD) prevention frameworks in Canada have adopted a mid-stream approach focused on preventing alcohol consumption among women who are already pregnant. Yet there is a second approach, further upstream, that is rarely discussed as an FASD prevention tool in this country - pre

Evaluating the psycholegal abilities of young offenders with fetal alcohol spectrum disorder.

Individuals with a diagnosis of fetal alcohol spectrum disorder (FASD) experience a range of physical, cognitive, and behavioral deficits thought to interfere with their ability to competently navigate the arrest, interrogation, and trial process. This study examined the psycholegal abilities of young offenders with FASD, including their understanding and appreciation of Miranda rights, and adjudication capacities (factual knowledge of criminal procedure, appreciation of the nature and object of the proceedings, ability to participate in a defense and communicate with counsel). Two groups of y

Identifying the neurobehavioral phenotype of fetal alcohol spectrum disorder in young children.

BACKGROUND: Most children with Fetal Alcohol Spectrum Disorder (FASD) do not display the typical facial changes, making the diagnosis much more challenging due to poor specificity of the brain dysfunction exhibited by these children. We have recently described and validated a behavioral phenotype of FASD using items from the Child Behavior Checklist (The Neurobehavioral Screening Test, NST). This tool has high sensitivity and specificity in separating children aged 6-13 yrs with FASD from those with ADHD and from healthy controls. OBJECTIVES: To test the validity of the NST for children aged

An exploration of the experiences and perspectives of New Zealanders with fetal alcohol spectrum disorder.

BACKGROUND: The experiences and perspectives of New Zealanders with fetal alcohol spectrum disorder (FASD) need to be heard since no research to date has been performed. FASD, a neuro-developmental disability with life-lasting effects, is irreversible. The condition is caused by prenatal exposure to alcohol. OBJECTIVES: This study aimed to explore and understand the daily challenges of New Zealand individuals with FASD. METHODS: Our sequential mixed methods design used two discrete but compatible qualitative methodologies--transcendental phenomenology in Phase One and classic grounded theory

Fetal alcohol spectrum disorder in Israel: increased prevalence in an at-risk population.

BACKGROUND: Maternal exposure to alcohol during pregnancy can lead to a wide range of clinical manifestations in their offspring, termed fetal alcohol spectrum disorder (FASD). In Israel, relatively few cases of FASD have been diagnosed and the prevalence has not been systematically evaluated. OBJECTIVES: To determine the number of children with FASD or at risk for FASD in a select population of high risk patients seen at a clinic evaluating foster and adopted children. METHODS: Israeli children under 2 years old who were candidates for domestic adoption or in foster care were prospectively

Evaluation of a successful fetal alcohol spectrum disorder coalition in Ontario, Canada.

Leading a successful coalition that benefits both the members and the community is a difficult task. Coalitions are complex and require a great deal of skill to initiate, lead, and evaluate. This article examines a successful coalition, developed to build community capacity to address fetal alcohol spectrum disorder (FASD). FASD is a complex, multidimensional health issue common in many communities. Coalitions can be effective in tackling these types of issues and fit with community capacity-building approaches to health promotion. The Internal Coalition Outcome Hierarchy (ICOH) model (Cramer,

Can prenatal screening for fetal alcohol spectrum disorder be justified? A commentary.

Fetal alcohol spectrum disorder (FASD) is the leading cause of non-genetic mental retardation in the USA, possibly exceeding even Down syndrome, which is currently approaching 1 in 500 live births. Alcohol consumption during pregnancy results in brain, craniofacial and heart defects, neurotoxicity, and immune dysfunction. The preferred action taken to prevent alcohol consumption during pregnancy is abstinence. However, the detection, diagnosis, and treatment of FASD remain a major public health need in this country and throughout the world. The biochemical molecules involved in the development

Diagnosing fetal alcohol spectrum disorder: History, challenges and future directions.

Fetal alcohol spectrum disorder (FASD) is one of the most common preventable causes of developmental disability, and is currently one of the most pressing public health concerns in Canada. FASD refers to the range of physical, mental, behavioural and learning disabilities that an individual may acquire as a result of maternal alcohol consumption. In the present paper, the history of the diagnostic approach to alcohol-related disorders over the past 35 years is reviewed. Research supporting the importance of early diagnosis for the long-term outcomes and management of individuals with FASD is p

Fetal alcohol spectrum disorder: New Zealand birth mothers' experiences.

UNLABELLED: The aim of this study was to describe the 'lived' experiences of New Zealand birth mothers, from pregnancy onwards, of a child/ren diagnosed with Fetal Alcohol Spectrum Disorder (FASD). METHOD: A qualitative paradigm was utilized so that the participants could tell their stories through words and text rather than collecting statistical data which is the domain of quantitative research. The adoption of a feminist standpoint theory bridged a communication gap, adjusted the balance of power within society and gave visibility and voice to the women. Eight New Zealand-resident multipar

The fundamentals of fetal alcohol spectrum disorder: Evaluation of an awareness-raising webinar.

BACKGROUND: Health professionals in the UK and internationally often lack knowledge of prenatal alcohol exposure (PAE) and fetal alcohol spectrum disorder (FASD). Raising awareness of PAE and FASD across health and social care sectors is vital to support Scotland's implementation of neurodevelopmental pathways. This study evaluated whether engaging in the Fundamentals of FASD contributed to change in (i) attitudes toward the health advice given to pregnant women, (ii) attitudes toward PAE, (iii) attitudes toward FASD, and (iv) knowledge of FASD. Furthermore, this study examined whether knowled

Building Toward Better Housing for Canadians With Fetal Alcohol Spectrum Disorder.

BACKGROUND: Individuals with disabilities have a right to live independently, be included within their communities, and have adequate standards of living and social protections. However, individuals with fetal alcohol spectrum disorder (FASD) experience elevated obstacles to obtaining and maintaining housing. In addition to brain-based differences that can create barriers to successful navigation of daily demands, many individuals with FASD experience mental health concerns, substance use, or trauma, and the experience of being houseless is, in itself, traumatic. These factors often compound,

The Neurobiology of Learning and Memory in Rodent Models of Fetal Alcohol Spectrum Disorders.

Exposure to ethanol during gestation can lead to the onset of Fetal Alcohol Spectrum Disorders, which describes a range of neurodevelopmental and behavioral dysfunctions that include impairments in learning and memory and can have serious repercussions for scholastic performance during adolescence. The neurobiological basis of learning and memory dysfunction in Fetal Alcohol Spectrum Disorders has been frequently linked to the hippocampal formation, which is due in part to the fact that some hippocampal neurons, called place cells, fire action potentials correlated with an animal's spatial loc…

Parenting by individuals with fetal alcohol spectrum disorders and neurobehavioral outcomes in their offspring.

BACKGROUND: The neurobehavioral health impairments associated with prenatal alcohol exposure are now known to persist through adulthood. However, little is known about how these impairments affect individuals' parenting abilities and the neurobehavioral health of their offspring. This study compares parents with fetal alcohol spectrum disorder (FASD) with socioeconomically matched, nonexposed parents on measures of parenting and family support and assesses the neurobehavioral health of the children in both groups. METHODS: Forty-nine parent-child dyads were recruited from a longitudinal cohor

Fetal alcohol spectrum disorder resources for health professionals: a scoping review.

OBJECTIVES: This scoping review aimed to identify and critically appraise resources for health professionals to identify, diagnose, refer, and support individuals with fetal alcohol spectrum disorder (FASD)-including the extent to which the resources are appropriate for use in communities with First Nations Peoples. METHOD: Seven peer-reviewed databases (April 2022) and 14 grey literature websites (August 2022) were searched. The reference lists of all sources that underwent full-text review were handsearched, and FASD experts were consulted for additional sources. Resources were assessed usi

A multi-organ analysis of the role of mTOR in fetal alcohol spectrum disorders.

Alcohol exposure during gestation can lead to fetal alcohol spectrum disorders (FASD), an array of cognitive and physical developmental impairments. Over the past two and a half decades, Mammalian Target of Rapamycin (mTOR) has emerged at the nexus of many fields of study, and has recently been implicated in FASD etiology. mTOR plays an integral role in modulating anabolic and catabolic activities, including protein synthesis and autophagy. These processes are vital for proper development and can have long lasting effects following alcohol exposure, such as impaired hippocampal and synapse for…

Fetal alcohol spectrum disorders: an overview of current evidence and activities in the UK.

Estimates for the UK suggest that alcohol consumption during pregnancy and prevalence of fetal alcohol spectrum disorder (FASD)-the most common neurodevelopmental condition-are high. Considering the significant health and social impacts of FASD, there is a public health imperative to prioritise prevention, interventions and support. In this article, we outline the current state of play regarding FASD knowledge and research in the UK, which is characterised by a lack of evidence, a lack of dedicated funding and services, and consequently little policy formulation and strategic direction. We hig

Experiences of Children with Fetal Alcohol Spectrum Disorder and Their Families: A Critical Review.

Evidence suggests that children with fetal alcohol spectrum disorder (FASD) experience challenges across many areas of their daily lives and often require interprofessional supports. Recent studies have emphasized the need for an integrated system of care for children with FASD, incorporating medical, allied health, and education services, to facilitate open communication and support for the complex needs that many children experience. To develop such a system of care, it is important to first understand the impact of FASD on children's functioning during daily activities in different environm
