Clinical Characteristics Associated with Fetomaternal Hemorrhage: A Retrospective Multicenter Descriptive Study (2009-2026).

Fetomaternal hemorrhage refers to the transfer of fetal erythrocytes into the maternal circulation. Although clinically significant fetomate … cal cardiotocography was observed in 79% of cases, including 42% with a sinusoidal pattern. Reduced fetal movements were reported in 51% of cases. Elevated middle cerebral artery peak systolic velocity, suggestive of fetal anemia, was documented in 50% of cases with an overall documentation rate of 31%; abdominal trauma was present in 15% of cases. Caesarean sec …

Usefulness of maternal fetal red blood cell count in rhesus-positive pregnant women.

Background Fetal red blood cells (FRBC) in maternal blood are counted in rhesus-negative women to determine the amount of anti-D immunoglobulin to be administered in the case of a rhesus-positive fetus. In rhesus-positive pregnant women this is done in not always very well-defined indications including trauma, miscarriage, fetal death and diminished fetal movements. The aim of this study is to determine if the detection of FRBC is useful in rhesus-positive pregnant woman. This was done by assessing maternal and fetal characteristics that are more likely to give a positive test. Materials and m…

Maturation of body and breathing movements in 24-33 week-old fetuses threatening to deliver prematurely.

Maturation of spontaneous fetal body and breathing movements of 24- to 33-week-old fetuses in 168 pregnancies threatening to deliver prematurely were examined on the basis of newborn outcome (premature compromised, premature healthy, term healthy). Maturation of fetuses in 60 low-risk pregnancies delivering as healthy full-term infants served as a normative comparison group. Each fetus was observed for 30 min; the amount of body and breathing movements were noted and an estimation of amniotic fluid volume was made. The pattern of behavioural maturation was similar for all outcome groups; with

Acute maternal rehydration increases the urine production rate in the near-term human fetus.

OBJECTIVE: We sought to investigate the effect of a decrease of maternal plasma osmolality produced by hypotonic rehydration on the fetal urine production rate in normal near-term human fetuses. STUDY DESIGN: Twenty-one healthy pregnant women attending the clinic for antenatal care were studied between 37 and 40 weeks' gestation. The fetal urine production rate was assessed by serial measurements of 3 diameters of the fetal bladder. The hourly fetal urine production rate was determined by linear regression analysis of the calculated bladder volumes versus time and was initially determined aft

Ultrasound prediction of fetal pulmonary hypoplasia in pregnancies complicated by oligohydramnios and in cases of congenital diaphragmatic hernia: a review.

The objective of this review is to discuss the current knowledge of fetal pulmonary hypoplasia and to summarize the clinical significance of the many ultrasound methods for predicting pulmonary hypoplasia in pregnancies complicated by oligohydramnios due to spontaneous rupture of the fetal membranes and in cases complicated by fetal congenital diaphragmatic hernia. We concluded that the presence or absence of polyhydramnios, fetal breathing movements, mediastinal shift, thoracic position of the stomach, fetal breathing-related nasal and oropharyngeal fluid flow, ductal flow velocity modulation

[Steinert disease and pregnancy].

Steinert's syndrome and pregnancy with delivery are rarely associated. It is a dominant autosome disease. Certain signs in pregnancy may suggest that the fetus is affected. One case made it possible to demonstrate these facts. Hydramnios with infrequent fetal movements and a slow fetal heart rate may lead to a suspicion of fetal involvement. This is always serious. The presence of the disease in the mother should be sought if it is not already known. If there is already a known family history the patient should see a geneticist so long as early amniocentesis has not yet been performed.

Allantoic fluid compositional changes during acute urine drainage in fetal sheep.

OBJECTIVE: This study examines the effects of ovine fetal urine drainage, which prevents urine inflow into the allantoic and amniotic cavities, on allantoic and amniotic fluid osmolalities and electrolyte concentrations in an attempt to study solute movement out of the allantoic cavity. METHODS: Nine chronically catheterized fetal sheep at 132 +/- 2 (mean +/- standard error) [SE] days' gestation, with allantoic and amniotic cavity catheters, were studied. Seven animals were studied for 9 hours, with maternal and fetal blood drawn hourly, and amniotic and allantoic fluid sampled for osmolality

Differences in the sleep states of IUGR and low-risk fetuses: An MCG study.

BACKGROUND: Intrauterine growth restriction (IUGR) is a fetal condition characterized by growth-rate reduction. Afflicted fetuses tend to display abnormalities in heart rate. OBJECTIVE: To study the differences in the heart-rate variability of low-risk fetuses and IUGR fetuses during different behavioral states. METHODS: A total of 40 fetal magnetocardiograms were analyzed from 20 low-risk and 20 IUGR fetuses recorded using a 151-sensor SQUID-array system. The maternal cardiac signals were attenuated using signal-space projection. Fetal R waves were identified using an adaptive Hilbert trans

The assessment of general movements is a valuable technique for the detection of brain dysfunction in young infants. A review.

Recently, a new assessment technique by which to evaluate brain function in the fetus and newborn infant has been developed. The method is based on the assessment of the quality of General Movements (GMs). GMs are complex movements involving all parts of the body. They are present throughout fetal life and postnatally until about 3-4 months post-term. The persistent presence of abnormal GMs during the neonatal period and the absence of age-specific characteristics ("fidgety" movements) around 3 months-post term are both powerful predictors of the development of handicap.

Reversed end-diastolic flow in the middle cerebral artery preceding death in a normally grown fetus.

BACKGROUND: Reversed diastolic flow in the middle cerebral artery has been described as a terminal sign in the growth-restricted fetus. The implication of this in a normally grown fetus is unknown. CASE: Middle cerebral artery Doppler velocimetry was performed at 28 weeks of gestation in the setting of decreased fetal movement and a category 2 fetal heart tracing. The middle cerebral artery pulsatility index was elevated, and follow-up studies revealed reversed flow. The fetus had appropriate growth, with mild polyhydramnios and no signs of cardiomegaly, effusion, or hydrops. Fetal death was …

Dual lives at risk: managing haemoperitoneum and splenic injury in pregnancy.

A pregnant woman in her 20s, at 31 weeks gestation, presented with severe abdominal pain, reduced fetal movements and mild vaginal bleeding following a fall. Examination revealed pallor, a tense abdomen and absent fetal movements. Point-of-care ultrasound indicated haemoperitoneum and placental abruption. Emergency laparotomy and lower segment caesarean section were performed, delivering a 1 kg neonate with Apgar scores of 5 and 7. Intraoperative findings included 3 L haemoperitoneum, placental abruption and splenic hilar vascular injury, necessitating splenectomy. The mother received blood tr…

Pregnancy outcomes among women born in Somalia and Sweden giving birth in the Stockholm area - a population-based study.

BACKGROUND: Studies report that women born in some African countries, after migrating to the Nordic countries, have worse pregnancy outcomes than women born in the receiving countries. With the aim of identifying unmet needs among Somali-born women, we here study this subgroup. OBJECTIVE: We compared pregnancy outcomes among women born in Somalia to women born in Sweden. Further, we investigated whether the proactive maternal observation of fetal movements has effects on birth outcomes among women born in Somalia. METHODS: In Stockholm, half of the maternity clinics were randomized to interv…

Umbilical Cord Abnormalities.

The umbilical cord is the connection between mother and fetus through which gases and nutrients are exchanged. It's remarkable structure allows for freedom of movement while providing a cushioned, protected conduit from mother to fetus. Fetal development and survival are dependent upon the umbilical cord. This article reviews abnormalities of the umbilical cord that can be seen with structural and chromosomal abnormalities and altered umbilical cord flow associated with fetal growth restriction and poor pregnancy outcomes.

Non-immune hydrops fetalis: a short review of etiology and pathophysiology.

Hydrops fetalis is an excessive accumulation of fetal fluid. Hydrops is traditionally classified into either immune or non-immune hydrops (NIHF), but in practice, nowadays in the Western world >90% of hydrops is of non-immune origin. The basis of the disorder is an imbalance in the regulation of fetal fluid movement between the vascular and interstitial space. We previously suggested a diagnostic flow-chart for NIHF. In this short review we describe the main mechanisms leading to NIHF.

An unruptured tubal pregnancy at term.

We report a case of advanced, unruptured tubal pregnancy proceeding beyond term. A 25-year-old woman was admitted at 42 weeks' gestation because of lower abdominal pain and inability to feel fetal movements. An ultrasound examination suggested fetal death. At laparotomy three weeks after admission, a right-sided, unruptured tubal mass containing a macerated fetus was found.
