More than a fever: Encephalitis due to typhoid fever.

Worldwide, 10.9 million cases of typhoid fever occur every year, with 116,000 deaths. The largest burden of disease occurs in Africa and Asia but imported cases in travelers, including those visiting friends and relatives, are common. Although the majority of cases are uncomplicated and respond well to antibiotics alone, severe disease may occur, particularly, in incompletely or untreated cases. Mortality is particularly common where complications, including shock, intestinal perforation, or neurologic involvement, occur. A case of typhoid fever with severe neurologic involvement resulting in …

Dengue fever and dengue haemorrhagic fever--a diagnostic challenge.

The number of cases of dengue fever in returning travellers is increasing worldwide. In Australia, two mosquito vectors exist and the Aedes aegypti mosquito has already been responsible for local transmission within Queensland. For these reasons, general practitioners need to be able to recognise dengue fever and its complications: dengue haemorrhagic fever (DHF) and dengue shock syndrome. Infections can vary from severe to asymptomatic. The incubation period, duration of fevers, presence of rash and relative bradycardia can assist in the diagnosis of dengue. Dengue haemorrhagic fever is a sev…

Fluoroquinolones for treating typhoid and paratyphoid fever (enteric fever).

BACKGROUND: Fluoroquinolones are recommended as first-line therapy for typhoid and paratyphoid fever, but how they compare with other cheaper antibiotics and different fluoroquinolones is unclear. OBJECTIVES: To evaluate fluoroquinolone antibiotics for treating enteric fever in children and adults compared with other antibiotics, different fluoroquinolones, and different treatment durations of the same fluoroquinolone. SEARCH STRATEGY: We searched the Cochrane Infectious Diseases Group Specialized Register (August 2004), CENTRAL (The Cochrane Library Issue 3, 2004), MEDLINE (1966 to August 2…

Fever of unknown origin: subacute thyroiditis versus typhoid fever.

Fever of unknown origin (FUO) is not infrequently a diagnostic dilemma for clinicians. Common infectious causes include endocarditis and abscesses in adults, and noninfectious causes include neoplasms and certain collagen vascular diseases, for example, polymyalgia rheumatica, various vasculitides, and juvenile rheumatoid arthritis (adult Still's disease). Subacute thyroiditis is a rare cause of FUO. Among the infectious causes of FUO, typhoid fever is relatively uncommon. We present a case of FUO in a traveler returning from India whose initial complaints were that of left-sided neck pain and…

[Flavivirus infections: yellow fever, dengue fever and Japanese encephalitis].

Flavivirus infections, such as dengue fever, yellow fever and Japanese encephalitis, are untreatable. As a result of the high prevalence of dengue fever in endemic areas, it poses a substantial risk for travellers to those areas. When it comes to Japanese encephalitis and yellow fever, the risk for travellers is limited but the diseases are extremely serious, creating a dilemma for the physician when it comes to deciding whether to vaccinate against them. The vaccines against both potentially have side effects. The indications and contraindications for vaccination are described in detail.

Rat bite fever misdiagnosed as Rocky Mountain spotted fever.

A patient who had been exposed to ticks and who had also been bitten by a laboratory rat developed fever, headache, and a rash. He was treated with chloramphenicol for Rocky Mountain spotted fever, and recovered. Blood cultures, however, grew Streptobacillus moniliformis, a causative agent of rat bite fever. The case report illustrates the clinical similarities between rat bite fever and Rocky Mountain spotted fever.

Factors Influencing Fever Defervescence in Children with Enteric Fever.

The duration of fever defervescence in enteric fever has been observed to be increasing in recent times. An evaluation of the factors influencing fever defervescence in 150 children aged 1 month-18 years with confirmed (blood culture yielding Salmonella typhi or S. paratyphi) enteric fever was undertaken, and the minimum inhibitory concentrations (MICs) of ceftriaxone and azithromycin were determined. The median (q1, q3) time to fever defervescence was found to be 88 (58.75, 117.25) hours. Prior antibiotic therapy, splenomegaly and systemic complications were significantly associated with late…

Typhoid conjugate vaccines for preventing typhoid fever (enteric fever).

RATIONALE: Typhoid fever is a major cause of enteric disease-related morbidity and mortality. Vaccination reduces disease burden and prevents outbreaks, but policies and programmes should be informed by the most recent evidence as newer vaccines become available. OBJECTIVES: To assess the benefits and harms of typhoid conjugate vaccines (TCVs) compared to no vaccine, placebo, typhoid-inactive agents (vaccines for another disease) or other typhoid vaccines for preventing morbidity and mortality associated with typhoid fever in adults and children. SEARCH METHODS: In April 2024, we searched th…

Fluoroquinolones for treating typhoid and paratyphoid fever (enteric fever).

BACKGROUND: Typhoid and paratyphoid are febrile illnesses, due to a bacterial infection, which remain common in many low- and middle-income countries. The World Health Organization (WHO) currently recommends the fluoroquinolone antibiotics in areas with known resistance to the older first-line antibiotics. OBJECTIVES: To evaluate fluoroquinolone antibiotics for treating children and adults with enteric fever. SEARCH STRATEGY: We searched The Cochrane Infectious Disease Group Specialized Register (February 2011); Cochrane Central Register of Controlled Trials (CENTRAL), published in The Cochr…

A Q fever case mimicking crimean-congo haemorrhagic fever.

Coxiella burnetii is the bacterium that causes Q fever. Human infection is mainly transmitted from cattle, goats and sheep. The disease is usually self-limited. Pneumonia and hepatitis are the most common clinical manifestations. In this study, we present a case of Q fever from the western part of Turkey mimicking Crimean-Congo haemorrhagic fever (CCHF) in terms of clinical and laboratory findings.
