Modified Carlevale Intraocular Lens Fixation Technique: Two Vitrectomy Ports As Lens Plug Fixation Sites.

PURPOSE: To describe and evaluate the effectiveness of the modified Carlevale intraocular lens (IOL) fixation technique, using two vitrectomy ports as lens plug fixation sites. MATERIALS AND METHODS: This prospective, consecutive, interventional study examined 60 eyes in 60 patients, who underwent 25- or 23-gauge vitrectomy for an IOL subluxation/luxation, lens dislocation, or aphakia, with Carlevale IOL implantation. RESULTS: Postoperatively, transient ocular hypotension was observed in four eyes. The mean refractive prediction error was -0.27 ± 0.78 diopters. No postoperative complications

[The influence of divergent strabismus on horizontal neuronal connections in the cat visual cortex].

We have investigated the neuronal connections of ocular dominance columns in the visual cortex of 5- 6-month-old divergent strabismic cats. Strabismus was induced by tenotomy of medial rectus muscle at the age of 13-16 postnatal days. The angle between visual axes of the eyes was assessed using photographic technique. It was found that severed ocular muscle partially reattaches to the eye globe. In some animals the deviated eye has returned to normal position, in the others the residual eye deviation was 5-15 degrees. The loss of cortical binocularity for such animals is well known [Sireteanu

Systematic arcminute-scale fixational offsets in patients with early visual cortex damage.

Cortically induced blindness (CB) resulting from stroke damage to the early visual cortex leads to extensive, typically extrafoveal visual deficits and is known to alter large-scale oculomotor behavior. Here, we show that even with preserved foveal acuity, fixational oculomotor behavior is subtly altered in CB patients. Using high-precision eye tracking, we observed a small but consistent gaze offset toward the blind field during passive fixation, which disappeared during a high-acuity central task. Despite this offset, fixation precision in both tasks was comparable, and it was similar betwee

Modified Intraocular Lens Intrascleral Fixation Technique Using Two Vitrectomy Ports as Lens Haptic Fixation Sites.

INTRODUCTION: We developed a new technique that uses two of the vitrectomy ports as intraocular lens (IOL) haptic fixation sites and compared that with a conventional T-fixation method. METHODS: A total of 33 eyes were retrospectively divided into the port-fixation (n=21) and conventional (n=12) groups. For the port-fixation group, supranasal and inferotemporal trocars (25-gauge) were placed in the center of a T-shaped lamellar scleral incision 2 mm from the corneal limbus and a supratemporal trocar at 3.5 mm. Following a vitrectomy, along with lens or IOL extraction as needed, the infusion c

Monocular fixation with the optic nerve head: a case report.

PURPOSE: To document and discuss the case of a patient with left esotropia (ET) who uses the left optic nerve head (ONH) for monocular 'fixation'. CASE REPORT: The patient was an 80-year-old male with left ET from early childhood. Retinal tracking monocular fixation measurements with a Nidek MP-1 revealed stable fixation within the left ONH area. In an attempt to challenge the initial observation, further assessments of fixation were performed with a smaller target size and requiring various gaze positions. MP-1 fixation data showed remarkably stable monocular fixation (+/-1 degrees over 30 s

Paroxysmal tonic upgaze: A heterogeneous clinical condition responsive to carbonic anhydrase inhibition.

BACKGROUND: Paroxysmal tonic upgaze (PTU), defined as an involuntary upward movement of the eyes, has been considered as a benign phenomenon but may also be associated with ataxia and developmental delay. METHODS: We report eight children with PTU; six of them also exhibiting symptoms of ataxia and/or developmental delay. Treatment with carbonic anhydrase inhibition was offered to children with persisting and/or severe forms. RESULTS: Whole-exome sequencing and genome-wide array analysis (n = 7) did not reveal mutations in the three known genes associated with PTU (CACNA1A, GRID2, SEPSECS),

Slow reading in children with anisometropic amblyopia is associated with fixation instability and increased saccades.

BACKGROUND: Previous studies show slow reading in strabismic amblyopia. We recently identified amblyopia, not strabismus, as the key factor in slow reading in children. No studies have focused on reading in amblyopic children without strabismus. We examined reading in anisometropic children and evaluated whether slow reading was associated with ocular motor dysfunction in children with amblyopia. METHODS: Anisometropic children (7-12 years) with or without amblyopia were compared to age-similar normal controls. Children silently read a grade-appropriate paragraph during binocular viewing. Screen tests used to map out ocular deviations.

A century ago Hess described how to map out ocular deviations on two pieces of paper in cases of diplopia. This paper discusses how to obtain useful information regarding ocular deviations from a chart that has been obtained from screen tests (Hess, Lancaster, Lees, Weiss).

[Results of surgical treatment of Stilling-Türk-Duane retraction syndrome].

BACKGROUND: The typical signs of the retraction syndrome (Stilling-Türk-Duane) are abnormal head posture, marked limitation of abduction, slight limitation of adduction and convergence, which are always accompanied by retraction of the globe, narrowing of the palpebral fissure, vertical deviation and increased intraocular pressure upon adduction. All signs are explicable by the co-contraction of the horizontal muscles or the failing relaxation of the lateral rectus muscle. Abnormal synergistic innervation occurs when in aplasia of the abducens nerve the lateral rectus muscle receives abnormal

Fixational Eye Movement Waveforms in Amblyopia: Characteristics of Fast and Slow Eye Movements.

Fixational eye movements comprise of fast microsaccades alternating with slow intersaccadic drifts. These physiologic eye movements play an important role in visual perception. Amblyopic patients are known to have fixation instability, particularly of the amblyopic eye. We examined eye movement abnormalities that contribute to this instability. We found that fixation stability is affected by the presence of fusion maldevelopment nystagmus (FMN). However, some amblyopes can have nystagmus without nasally directed slow phases and reversal in direction of the quick phase on ocular occlusion, feat

Driving impairment and altered ocular activity under the effects of alprazolam and alcohol: A randomized, double-blind, placebo-controlled study.

BACKGROUND: Alprazolam, also known by trade-name Xanax, is regularly detected along with alcohol in blood samples of drivers injured or killed in traffic collisions. While their co-consumption is principally legal, policy guidelines concerning fitness-to-drive are lacking and methods to index impairment are underdeveloped. METHODS: In this randomized, double-blind, placebo-controlled, crossover trial, we examined whether legally permissible levels of alcohol [target 0.04% blood alcohol concentration (BAC)], alprazolam (1mg), and their combination impacts driving performance, and whether drivi

Intermittent Divergent Squint in Prematurity and Its Neurophysiological Aspects.

Intermittent distance exotropia is a deviation characterized by an exophoria at near fixation and manifest exotropia at distance fixation. There is normal binocular fusional vergence and stereoacuity at near fixation, but the eyes tend to diverge in bright sunlight, tiredness, day dreaming and the patient may close one eye in such circumstances. Prematurity is associated with numerous eye pathology, besides retinopathy of prematurity, amblyopia, refractive errors, it is also associated with a higher risk esotropia and exotropia. We report a case of a 5-year-old girl (preterm and very low birth

Fibrin Glue (Tisseel) in Pterygium Surgery: A Narrative Review.

Pterygium is a common ocular surface disorder with a risk of recurrence following surgical excision. Conjunctival autografting is widely used to reduce recurrence, traditionally secured with sutures. Fibrin glue, including Tisseel (Baxter Healthcare Corporation, Deerfield, IL, USA), has been introduced as an alternative method of graft fixation. The objective of the study is to review the current evidence on the use of fibrin glue in pterygium surgery, with emphasis on surgical outcomes, recurrence rates, postoperative comfort, and complications. A narrative review of the literature was conduc

Stepping into the virtual unknown: feasibility study of a virtual reality-based test of ocular misalignment.

IntroductionDissociated tests of strabismus provide valuable information for diagnosis and monitoring of ocular misalignment in patients with normal retinal correspondence. However, they are vulnerable to operator error and rely on a fixed head position. Virtual reality headsets obviate the need for head fixation, while providing other clear theoretical advantages, including complete control over the illumination and targets presented for the patient's interaction.PurposeWe compared the performance of a virtual reality-based test of ocular misalignment to that of the traditional Lees screen, t

Multifactorial determinants of visual outcome in very and extremely preterm infants: A systematic literature review of risk factors for visual and oculomotor dysfunctions.

Preterm birth is often associated with neurological, neurodevelopmental and visual impairment arising from the complex interplay of ocular, visual pathway, and cerebral vulnerability in the immature infant. Visual dysfunction in premature infants may reflect ocular, anterior visual pathways, retrogeniculate pathways and visual associative areas abnormalities, as seen in Cerebral Visual Impairment. This systematic review synthesized evidence on the association between pre-, peri-, i and postnatal factors on basic visual (visual acuity, contrast sensitivity, visual field) and oculomotor (fixatio
