The effects of foot core exercises and minimalist footwear on foot muscle sizes, foot strength, and biomechanics: A systematic review and meta-analysis.

BACKGROUND: Specific foot exercises and the use of minimalist shoes during running or daily life were suggested to strengthen the intrinsic foot muscles and to modify locomotion biomechanics. We aimed to review the effectiveness of these interventions to modify foot muscle sizes, foot strength, and biomechanical outcomes. METHOD: PubMed, Embase, Cochrane Library and SportDiscus databases were searched (last update: 12 March 2024). Randomized controlled trials with intervention duration of four weeks or more were included. A random-effect meta-analysis was performed when at least two studies w…

Quality of Foot Care Among Patients With Diabetes: A Study Using a Polish Version of the Diabetes Foot Disease and Foot Care Questionnaire.

Diabetic foot syndrome is a common, yet serious complication of diabetes, affecting 4% to 10% of patients with diabetes. Polish Diabetes Association guidelines emphasize the need for prevention among patients with diabetes; a key role is played by consistent education on foot care and ulceration prevention. The Diabetes Foot Disease and Foot Care Questionnaire (DFDFC-Q) is the only research instrument available for comprehensive assessment of foot self-care. The study purposes were to explore the quality of foot care among patients with diabetes and to evaluate the psychometric properties of t…

The clinical assessment study of the foot (CASF): study protocol for a prospective observational study of foot pain and foot osteoarthritis in the general population.

BACKGROUND: Symptomatic osteoarthritis (OA) affects approximately 10% of adults aged over 60 years. The foot joint complex is commonly affected by OA, yet there is relatively little research into OA of the foot, compared with other frequently affected sites such as the knee and hand. Existing epidemiological studies of foot OA have focussed predominantly on the first metatarsophalangeal joint at the expense of other joints. This three-year prospective population-based observational cohort study will describe the prevalence of symptomatic radiographic foot OA, relate its occurrence to symptoms,…

Biomarkers in Diabetic Foot.

Diabetes mellitus affects nearly 600 million adults worldwide and frequently leads to complications such as diabetic foot ulcers (DFU), driven by neuropathy, ischemia, infection, and impaired wound healing. Biomarkers have emerged as essential tools to improve early diagnosis, risk stratification, and management. Inflammatory markers like CRP, IL-6, and TNF-α, along with oxidative stress indicators such as MDA and 8-OHdG, highlight chronic inflammation and cellular damage in nonhealing wounds. Wound surface area trends and metabolic markers-including HbA1c, fasting glucose, and CGM metrics-fur…

Beaming the Charcot Foot.

The Charcot diabetic foot presents unique challenges to the podiatric surgeon in the quest to salvage the limb. This disorder is an intersection of prototypical metabolic diseases and neurodegenerative disorder. Furthermore, it can be considered a disease of bone and ligaments that is often complicated by peripheral vascular disease and serious deep infection. Presently, simplistic ablative surgical procedures and the brace-makers art, still have a valid place in treating this disorder. Newer methods of surgical reconstruction are rapidly evolving to address distorted and nonfunctional limb. T…

Imaging the Equine Foot.

Over the past 5 years, advancements in diagnostic imaging technology have led to improvement of radiographic technique and development of standing computed tomography (CT) and PET-CT scanners. Although these modalities are in their initial stages of development and clinical applications, they are meant to revolutionize the diagnosis and management of diseases of the foot in the standing patient, in particular detecting subclinical lesions, and the establishment of computer-assisted surgical suits. This article also reviews the improved radiographic projections of the equine foot and benefits o…

The diabetic foot ulcer.

BACKGROUND: Diabetic foot ulcers are associated with significant morbidity and mortality and can subsequently lead to hospitalisation and lower limb amputation if not recognised and treated in a timely manner. OBJECTIVE: The aim of this article is to review the current evidence for preventing and managing diabetic foot ulcers, with the aim to increase clinicians' confidence in assessing and treating these complex medical presentations. DISCUSSION: All patients with diabetes should have an annual foot review by a general practitioner or podiatrist. A three-monthly foot review is recommended f…

Diagnosing diabetic foot osteomyelitis.

Bone involvement during an infection of the diabetic foot represents a serious complication associated with a high risk of amputation, prolonged antibiotic treatment and hospitalization. Diabetic foot osteomyelitis (DFOs) require a multidisciplinary approach given the usual complexity of these situations. DFO should be suspected in most cases especially in the most severe forms of soft tissue diabetic foot infections (DFIs) where the prevalence of bone infection may be up to 60%. Suspicion is based on clinical signs in particular a positive probe-to-bone (PTB) test, elevated inflammatory bioma…

Dystonic Pseudo Foot Drop.

The most common presentation of foot dystonia in patients with Parkinson's disease (PD) or dystonia is inversion of the foot accompanied by flexion of the toes, with or without extension of the hallux. Less commonly, foot dystonia may mimic foot drop, as occurs with weakness of the dorsiflexors muscles, resulting in a pseudo foot drop. This has rarely been reported in the literature and has been poorly recognized, often leading to misdiagnosis and unnecessary investigations and treatment. We report 5 patients with dystonic pseudo foot drop, one of them diagnosed with early-onset PD, 2 with spo…

Mouth in Foot Disease.

Toothpicks are commonly used household items that rarely cause serious injury or infection. Toothpick-related injuries often occur due to ingestion with subsequent trauma/infection at distal sites within the gastrointestinal tract; however, cardiovascular, pleural, and soft tissue infections have been reported. Eikenella corrodens is a gram-negative, facultative anaerobic bacillus found in oral flora associated with bite wound infections. A few case reports describe E. corrodens osteomyelitis from toothpick puncture wounds. We report a case of foot cellulitis and abscess in an elderly diabetic…

The adult paralytic foot.

The adult paralytic foot is a common clinical entity. It has numerous neurologic, systemic, and traumatic causes that result in muscle imbalance and foot deformity. A thorough physical examination and diagnostic work-up, as well as an understanding of the relevant functional anatomy, are essential to proper management. Treatment goals include the establishment of a plantigrade foot, elimination of deforming forces, and, when possible, restoration of active motor control.

A tropical diabetic foot.

Foot infections are a common problem and an important cause of morbidity in patients with diabetes. We report a patient with type 2 diabetes, presenting with a chronic foot wound resistant to standard care, in whom the diagnosis of eumycetoma was made through histopathological examination of a bone biopsy specimen and confirmed by polymerase chain reaction (PCR). Diagnosis and treatment of eumycetoma are reviewed. Eumycetoma caused by Madurella mycetomatis is an uncommon cause of osteomyelitis in patients with diabetes in Europe, but should be considered in patients from endemic regions when (…

[Foot and ankle surgery].

Rheumatoid arthritis is a systemic disease that often affects the foot and ankle (85%-100% of patients). There are characteristic deformities in relation to the stage of disease. Clinical assessment of both lower limbs is important, since factors such as valgus deformity of the knee can cause malposition of the foot and ankle. In the early stages, patients present with joint effusion which causes destruction of the cartilage ("stiff type") and distends ligaments and capsules ("loose type"). Medial and lateral ankle tendons are destroyed and become insufficient. A reduction in walking distance,…
