This article deals with soft-tissue injuries of the upper extremity incurred by competitive athletes. The authors describe the pathophysiology, conservative and operative management of musculotendinous elbow injuries, entrapment neuropathy, tendinous disorders, vascular injuries, and dermatologic conditions in athletes. Protective gear for many of the aforementioned injuries are also described.

A 36-year-old white male schoolteacher presented with a painless, semifluctuant mass on the volar aspect of his forearm and symptoms of ring finger tenosynovitis and median nerve compression. At operation there was a widespread infestation of the flexor tendon compartment, the carpal tunnel, and the tendon sheaths of the ringer finger with Onchocerca volvulus, a filarial nematode, not previously reported in the these tissues.

The simultaneous occurrence of ipsilateral humeral supracondylar and forearm fractures is unusual, and the incidence of this combination is not known. For the management of six cases of this fracture combination, initial reduction and transcutaneous pin fixation of the supracondylar fracture were recommended and the followed by reduction and stabilization of the forearm fracture. Serious neurovascular complications associated with these severe extremity injuries are manageable only by prompt diagnosis and treatment.

The etiology of the refracture after forearm fractures in childhood has been studied in a follow-up examination of 199 patients with dislocated forearm fractures. The incidence of the refracture was 13%. In all cases the primary fracture was a greenstick fracture of at least one of the two forearm bones. The incompletely broken through greenstick fracture was found to be significantly responsible for the refracture. Traumatic bowing of the forearm or lower leg is reported in 31 children. It is a relatively rare condition. Bowing occurs most frequently in combination with fracture of the other bone in the same extremity. In a minority of cases a bowing deformity is a single lesion. Age distribution, degree of deformity, mechanism of origin and therapy are presented and discussed.

Compartment syndrome is often trauma-related, but rare cases may arise from soft tissue sarcomas (STS) increasing pressure within muscle compartments. This case emphasizes the need to consider atypical causes of compartment syndrome in older adults. Pyomyositis, often caused by Staphylococcus aureus, is a rare primary infection of skeletal muscle and is usually associated with abscess formation. Pyomyositis caused by Mycobacterium tuberculosis is extremely rare. In this paper, by presenting a case of tuberculous pyomyositis, we tried to provide a simple answer to the question of when we should consider M. tuberculosis in the etiology of pyomyositis.

This case report describes a rare instance of muscle cysticercosis in a 16-year-old vegetarian female from an endemic region, highlighting the challenges in diagnosing atypical presentations of the disease.
