Spinal fractures and dislocations in children and adolescents.

During the years 1962 through 1978 a total of 42 children under the age of 15 years with fractures or dislocations of the spine were admitted to the Royal Brisbane Hospital Complex. Sixty-three percent of these children were males. The predominate causes of injury were fall/jump (40%), road traffic accidents (29%), and water-related injuries (19%). Half the injuries occurred to the cervical spine, while injuries at more than one level occurred in 35% of patients. Cord injuries occurred in 14% of patients, while skeletal and head injuries occurred in over half of the spinal injuries associated

Instrumentation of the spine for fracture dislocations in children.

13 children, ages 11-16 years, sustained severe compression fractures or fracture dislocations of the thoracolumbar spine. All cases were treated with Harrington instrumentation with fusion and six also had decompressive laminectomy. All injuries had significant instability and neurologic deficits (nine complete). Follow-up of 2-10 years is provided for analysis of maintenance of reduction and neurologic improvement. This technique appears to offer stability, reduction of orthopedic defects, and assumption of a more aggressive rehabilitation program without introducing significant operative mo…

[Fracture-dislocations of the upper end of the humerus].

30 cases have been collected and treated by the authors. They classify the anatomical types according to the direction and nature of the fracture line. The treatment is more difficult when the fracture is intra-capsular than when it is extra capsular, that is metaphysial or below the tuberosities. A particular type was described, called "vertical fracture" by the authors. The major complication was a persistent joint stiffness, commonly bound to an avascular necrosis of the humeral head, especially in intra-capsular fractures. An orthopaedic reduction should always be attempted when the fractu

Treatment of thoracolumbar dislocation and fractures with paraplegia.

Ninety per cent of fractures of the dorsal lumbar spine with paraparesis or paraplegia can be best treated and reduced by closed methods when satisfactory alignment can be obtained. The pathology of the corc can thus be allowed to regress and recover if it will do so. Strict posturing techniques are the most important part of medical and nursing care. Approximately 10 per cent of the cases can be treated by gentle, active surgical techniques such as manipulation under general anesthesia or open reduction. The use of spinal rods and clamps is rarely indicated. Surgical techniques may be advisab

Combined Irreducible Femoral Head Fracture-Dislocation and Ipsilateral Segmental Femur Fracture: A Case Report.

CASE: A 48-year-old man presented to a Level 1 trauma center after a motor vehicle crash with a right irreducible posterior hip dislocation and ipsilateral fractures of the femoral head and segmental femur shaft. This injury was managed with open reduction and internal fixation and antegrade intramedullary nailing. CONCLUSION: This is the first report of a combined irreducible femoral head fracture-dislocation and a segmental femur fracture, a rare injury that requires a stepwise approach to operative management of each injury. Surgeons must recognize the clinical and radiographic findings as

An uncommon case of irreducible ankle fracture-dislocation: the "Bosworth-like" tibio-fibular fracture.

An irreducible ankle-fracture dislocation characterized by the dislocation of the proximal fragment of the fibula posteriorly to the tibial tubercle is defined as "Bosworth injury". In the present report it is described, for the first time in literature, a complex case where the Bosworth injury was associated to a tibial plafond fracture: the proximal fibular fragment was entrapped between the tibial pilon and the fractured posterior tibial tubercle, which acted as a clamp, thus avoiding ankle reduction. Due to the presence of the tibial fracture, it was not possible to observe the typical rad…

[Fracture of the glenoid fossa without mandibular condylar dislocation or fracture: two case reports].

The mandibular condyle region which protects the middle cranial fossa from facial and jaw traumas has an excellent osteomuscular structure. Condylar structures reduce or limit the force of trauma. Most importantly, the condylar neck is the weakest part of the mandible and is easily fractured without dislocation. Generally, this mechanism prevents condylar penetration into the middle cranial fossa; however, there are condylar penetration into the middle cranial fossa can be rarely. Glenoid fossa fractures without mandibular condylar fracture and dislocation can be made. In this article, we pres

Traumatic Cervical Facet Fractures and Dislocations.

Cervical facet injuries, though less common than other spinal injuries, represent a subset of subaxial cervical spine injuries and can present significant challenges in terms of diagnosis, management, and outcomes. A major concern with cervical facet fracture is identifying instability patterns that may necessitate surgical stabilization. Particularly in cases of cervical facet fracture dislocations, there remains controversy regarding requirements for closed reduction, timing, surgical approach, need for preoperative MRI, and method of fixation. This review article aims to provide a thorough …

A Case of Bilateral Galeazzi Fracture-Dislocations.

Bilateral Galeazzi fracture-dislocations are extremely rare injuries and only a few case reports have been described so far in the literature. Herein, we report the case of a 31-year-old bicyclist who sustained bilateral Galeazzi fracture-dislocations after a collision with a car. Both radial shaft fractures were simple (AO type 22-A2.3), at the same level (Type I: <7.5 cm from the joint line), and without severe comminution having their apex located dorsally. Internal fixation was accomplished in both fractures with an 8-hole, 3.5-mm locking plate; the stability of the distal radioulnar joint

Fracture dislocations of the carpometacarpal joints of the fingers.

OBJECTIVE: Fracture dislocations of the multiple carpometacarpal joints [CMCJ] of the fingers are uncommon injuries that can significantly compromise hand function and durability if managed sub-optimally. These injuries are at risk of being missed as they are commonly a part of major high energy trauma with associated more obvious and immediately threatening injuries getting all the attention. The clinical and radiological parameters which could help a surgeon to detect and analyse these injuries well are discussed. The management of these injuries with emphasis on the pattern of K-wire fixati…

Fractures and dislocation of the base of the thumb metacarpal.

Acute traumatic lesions of the base of the first metacarpal are frequent and their consequences can affect the opposition of the thumb. They usually occur after trauma in compression along the axis of the thumb in flexion. Restoring the anatomy and biomechanics of the trapeziometacarpal joint is essential when treating these injuries, hence why surgical treatment is usually indicated. We distinguish trapeziometacarpal dislocations, small-fragment and large-fragment Bennett's fractures, articular three-fragment Rolando and comminutive fractures and extra-articular fractures of the base of the f

Monteggia fracture dislocations: a historical review.

The eponym Monteggia fracture dislocation originally referred to a fracture of the shaft of the ulna accompanied by anterior dislocation of the radial head that was described by Giovanni Battista Monteggia of Italy in 1814. Subsequently, a further classification system based on the direction of the radial head dislocation and associated fractures of the radius and ulna was proposed by Jose Luis Bado of Uruguay in 1958. This article investigates the evolution of treatment, classification, and outcomes of the Monteggia injury and sheds light on the lives and contributions of Monteggia and Bado.

[Talocalcaneonavicular dislocation without associated fractures].

Talocalcaneonavicular or subtalar dislocation is the simultaneous dislocation of the talar joints at the talocalcaneal and talonavicular levels. It may occur in any direction and results in foot deformity. The most common type is medial dislocation. Lateral, anterior and posterior dislocations are less common. These dislocations are associated with osteochondral fractures. Closed reduction and immobilization continue to be the cornerstones of treatment. X-rays and computerized axial tomography scan of the involved ankle and foot confirm the congruence of the subtalar joint after reduction and
