A new treatment of mallet fractures of the distal phalanx is presented. Open reduction and internal fixation was performed using a "hook" plate fabricated from a 1.3 mm AO hand modular system straight plate. This technique avoids the need to place implants or wires through the small avulsion fragment while still being able to achieve a stable "tension-plate" type of fixation construct strong enough to allow protected early active motion of the distal interphalangeal joint. In minimising the need for prolonged splinting, patient comfort is also improved. In a consecutive series of nine fracture

BACKGROUND: Capitellar and trochlear fractures are uncommon fractures of the distal aspect of the humerus. There is limited information about the functional outcome of patients managed with open reduction and internal fixation.
METHODS: The functional outcome of twenty-eight patients, with a mean age (and standard deviation) of 43 +/- 13 years, who were treated with open reduction and internal fixation for capitellar and trochlear fractures was evaluated at a mean duration of follow-up of 56 +/- 33 months. Patient outcomes were assessed with physical and radiographic examination, range-of-mot

OBJECTIVES: There is no consensus as to whether internal fixation or hemiarthroplasty is more appropriate for the treatment of intertrochanteric femur fractures in elderly patients. While the latter offers early mobilization, internal fixation preserves the hip joint and avoids long-term complications associated with the prosthesis. This retrospective study aimed to compare the early results of these treatment modalities.
METHODS: The study included 81 patients who were available for follow-up after surgery for intertrochanteric femur fractures. Of 38 patients (mean age 77.7 years; range 65 t

Rigid internal fixation (RIF) is commonly used for the anatomical reduction and fixation of facial fractures. This technique has many advantages over more traditional methods, such as wire osteosynthesis and maxillomandibular fixation. Properly placed RIF ensures a stable anatomical reduction and allows for immediate or early restoration of function. RIF has a number of complications associated with its use in facial trauma surgery including metal sensitivity, infection, neurologic injury, dental trauma, stress shielding, and malocclusion. Complication rates appear to be inversely proportional

INTRODUCTION: Treatment of fractures of the mandibular condyle fractures varies among centres as there still is no general consensus. The aim of this paper was to determine the safety and efficiency of surgical treatment using a transparotid approach for direct plating.
PATIENTS AND METHODS: A prospective clinical study was conducted on 34 patients with 36 fractures of the condyle. All 36 fractures were displaced, and 14 (39%) of them were fracture dislocations. The fractures were treated surgically with a transparotid facelift or retromandibular approach using miniplates and screws for fixat

BACKGROUND: Thirty-two displaced intra-articular fractures of the calcaneus in 30 patients were treated with open reduction and internal fixation. Fracture classification was based on Sanders computed tomographic classification. There were 18 type II fractures, 10 type III fractures, and 4 type IV fractures.
METHODS: The operations were performed using a standard extended lateral approach, and the fractures were fixed with small-fragment AO T-plates without bone grafting. Average follow-up was 35.4 months (range, 24-53 months). The Creighton-Nebraska Health Foundation Assessment score for fra

Proximal tibial articular fractures are caused by a variety of mechanisms and are characterized by numerous distinct fracture patterns. Surgical treatment for other than minimally displaced or nondisplaced fractures is recommended to restore joint congruity and limb alignment, and to allow early, stable, knee motion.

Arthroscopic reduction and internal fixation of tibial plateau fractures has been well documented over the last 15 years. Better visualization, less traumatic surgery, reconstruction of accompanying injuries and early mobilization have encouraged arthroscopic surgeons to use this technique more widely. With experience, surgeons became more comfortable in using arthroscopy-assisted surgery not only in split fractures of the lateral tibial plateau (41-B1) but also in more complex tibial and femoral fractures. In this study, 31 patients with intra-articular fractures of the lateral tibial plateau

PURPOSE: This computer-based study was performed to determine the suitability of small biodegradable plate systems for mandibular angle fractures.
MATERIALS AND METHODS: In a 3-dimensional computer model of the mandible, fracture mobility and plate strain were calculated for bite forces applied on 13 bite points on the dental arch. The angle fracture was fixed with 2 polylactide (PLA) midiplates or with 2 PLA maxiplates. The first plate was positioned buccally on the external oblique ridge. Two positions of the second plate were studied: halfway up the height of the mandible or on the lower b

Displaced fractures of the glenoid fossa are an uncommon and anatomically diverse group of injuries. Failure to restore anatomy in these fractures results in poor outcome in most cases. The success of a treatment protocol that encompasses appropriate preoperative imaging, injury pattern assessment, prudent approach choice, and a comprehensive reduction and fixation tactic was evaluated. Twenty-seven patients were assessed clinically and radiographically at a mean followup interval of 43 months from surgery. Anatomic reconstruction was achieved in 24 (89%) patients. Three patients had residual

Ipsilateral fractures of the scapular neck and clavicle disrupt the suspensory mechanism of the shoulder, resulting in functional loss and deformity. Fifteen patients who had such fractures were managed with open reduction and internal fixation and were followed for an average of twenty-five months (range, fourteen to forty-seven months). All but one patient had a good or excellent functional result according to the scoring system of Rowe. All fractures healed at an average of eight weeks postoperatively. Complications from concomitant fractures of the ribs were treated uneventfully. We conclu

Complex distal radius fractures present in a variety of patterns that differ by the area and degree of involvement of the articular and metaphyseal fracture components. No single method of treatment is appropriate for all fractures. This article discusses the types of fractures that either require or are best handled by formal open reduction and internal or external fixation.

Twenty-two patients with open ankle fractures or fracture-dislocations were treated with irrigation and debridement, reduction, and immediate stable internal fixation at an average of six hours from initial evaluation. There were 13 women (59%) and nine men (41%), having an average follow-up period of 32 months (range, five to 111 months). There were six Grade I (27%), 15 Grade II (68%), and one Grade III (5%) injuries. Fractures also were classified according to the Danis-Weber scheme (Type A [three cases]; Type B [eight]; Type C [11]). Excellent results were achieved in 14 patients (64%); go

To evaluate the factors resulting in mechanical failures of internal fixation of distal T-type and Y-type humeral fractures, we reviewed the clinical course and final results in 61 consecutive cases treated with AO-ASIF internal fixation devices. The patients were followed for at least 2 years (range, 2-11 years). Mechanical failure occurred in 18 cases (29.5%). In 12 patients this resulted from an unstable fixation. Severe osteoporosis was present in four patients. In two patients no specific causative factor could be detected. A second osteosynthesis was undertaken in five patients, and four

The effect of early mobilization and unrestricted weight bearing on final ankle motion in 51 operatively stabilized ankle fractures was prospectively investigated. Patients were treated with an ankle-foot orthosis (AFO) or a cast. Full weight bearing was unrestricted in both groups. Thirty-two fractures received an AFO and 19 received a cast. The follow-up period ranged from 2.3 to 66 months, with a median of eight months. At the final follow-up examination, the motion in the AFO-treated group was not functionally different from that of the cast-treated group. However, 72% of the patients trea

We reviewed the records and radiographs from 10 hospitals to identify 50 patients with rheumatoid arthritis (RA) who had sustained 52 femoral neck fractures. Most patients were female (88%), elderly (mean age 66 years) and had had severe polyarticular disease for a mean duration of 16 years. Over half had taken systemic corticosteroids, nearly all were severely osteopenic but few had rheumatoid changes in the hip. Of the 20 fractures treated by internal fixation 12 had complications including nonunion (5), osteonecrosis (5), infection (1), and intertrochanteric fracture (1). Only one of the ni
