Angulated greenstick fractures of the distal forearm in children: closed reduction by pronation or supination.

The purpose of this study was to evaluate a reduction method that is based on the theory of Evans to reduce angulated greenstick fractures of the distal forearm with a rotation manoeuvre, to evaluate an immobilisation technique and to evaluate a brief survey on surgeon practice for treatment of these fractures. A retrospective study was performed on 21 patients. Fractures were reduced with a pronation or supination manoeuvre depending on the angulation of the fracture and were immobilised in pronation or supination. A good reduction was achieved in all patients. Six weeks after manipulation a

Recovery of mandibular motion after closed and open treatment of unilateral mandibular condylar process fractures.

The purpose of this study was to determine the rate of recovery of mandibular motion in patients treated for fractures of the mandibular condylar process. One hundred and thirty-six patients (111 men, 25 women), 74 treated by closed and 62 by open methods, were included. They underwent testing of mandibular and condyle mobility at 6 weeks, 6 months, and 1, 2, and 3 years post surgery. Their ranges of motion were compared to those of 52 controls (26 men and 26 women). A jaw-tracking device was used to assess mandibular motion. Multilevel statistical models were used to assess differences betwee

Functional limitation immediately after cast immobilization and closed reduction of distal radius fractures: preliminary report.

The majority of research on distal radius fractures consists of retrospective, descriptive studies of patients with unstable fractures requiring fixation. The purpose of this investigation was to report on impairments in flexibility, grip strength, and motor control and on the presence of swelling and atrophy immediately after cast immobilization of closed reductions of simple distal radius fractures. Sixteen adult subjects from Kaiser Permanente Medical Center, San Francisco, entered the study, and 13 completed it. At the initial evaluation, upper extremity ranges of motion, grip strength, fo

Comparison of open versus closed reduction of intraarticular calcaneal fractures: a matched cohort in workmen.

Seventeen displaced intraarticular calcaneal fractures treated operatively were matched with 17 nonoperatively treated displaced intraarticular calcaneal fractures among Workers' Compensation Board patients. Matching criteria included type of fracture, age, year of injury, and occupation. After follow-up averaging 6.3 years and 5.4 years, respectively, no significant differences existed between the two groups in heel pain, subtalar motion, and return to work. However, in those fractures treated operatively, the overall clinical result was better when an anatomic reduction of the subtalar joint

Blade-plating of closed displaced supracondylar fractures of the distal femur with the AO system.

We reviewed a series of 42 closed and displaced supracondylar and intercondylar fractures of the distal end of the femur that were treated over a 7-year period by open reduction and internal fixation using the AO system (blade-plate). A strict rating scale incorporating subjective data and objective motion of the involved knee was used for the results. At a mean follow-up of 5 years, the results in eight patients were rated excellent, the results in 14 were rated as good, the results in 11 as fair, and the results in nine as poor. Complications were found in 15 patients (37.5%). Complications

Coronal split fracture of the proximal tibia epiphysis through a partially closed physis: a new fracture pattern.

A comminuted coronal split fracture of the proximal tibial epiphysis is an uncommon injury in children. The authors evaluated and treated two patients who sustained an epiphyseal fracture through a partially closed proximal tibial epiphysis. Plain radiography and computed tomography with three-dimensional reconstruction showed a comminuted coronal split fracture of the proximal tibial epiphysis. Each patient underwent definitive operative fixation of the fracture and was followed at least 1 year after clinical union. A mechanism consisting of three-point bending on the tibial plateau is propos

Failure after operative repair is higher for ballistic femoral neck fractures than for closed, blunt-injury fractures: a multicenter retrospective cohort study.

INTRODUCTION: The purpose of this study was to describe the outcomes after operative repair of ballistic femoral neck fractures. To better highlight the devastating nature of these injuries, we compared a cohort of ballistic femoral neck fractures to a cohort of young, closed, blunt-injury femoral neck fractures treated with open reduction and internal fixation (ORIF). METHODS: Retrospective chart review identified all patients presenting with ballistic femoral neck fractures treated at three academic trauma centers between January 2016 and December 2021, as well as patients aged ≤50 with clo

New Surgical Devices for Closed Reduction of Frontal Sinus Bone Fracture.

Traditional surgical interventions for frontal sinus fractures necessitate a cut on the forehead skin, and extant closed reduction techniques aimed at enhancing accessibility continue to grapple with secure tool fixation, stable bone elevation, and screw breakage risk. To address these challenges and augment surgical efficiency, this study introduces novel surgical devices. Design parameters for models with spiral or L-shaped tips are established, considering practical medical requirements and constraints, and subsequently validated through finite element method numerical simulations using com

Closed Intramedullary Pinning of Displaced Radial Neck Fracture (Metaizeau Technique).

BACKGROUND: Radial neck fractures account for 1% of all pediatric fractures and 5% to 10% of pediatric elbow fractures. The mechanism of injury is typically a fall with the elbow in hyperextension and the forearm in supination. A valgus force compresses the radial head against the capitellum, causing a radial neck fracture. Displaced radial neck fractures are difficult to treat and account for a disproportionate number of bad outcomes, including malunion, nonunion, and osteonecrosis. The preferred treatment is closed reduction and fixation, as open reduction is associated with an inordinately

Entrapment of the Extensor Digitorum Communis Tendon Following a Closed Metacarpal Fracture.

We report a unique patient with extensor digitorum communis (EDC) tendon entrapment following a closed metacarpal fracture. A 19-year-old male presented after punching a metal pole with his right hand. The diagnosis of a closed metacarpal fracture of the right middle finger was made, and the patient managed nonoperatively. Following subsequent worsening range of motion, further investigation was performed, and a portable ultrasound scan revealed entrapment of the right middle finger EDC tendon into the fracture site. This was confirmed intraoperatively, and the patient made a satisfactory reco

Peroneal Tendons Rupture in a Closed Talar Body Fracture: A Rare Injury.

The combination of tendon and ligament ruptures with fracture of the talus is very rare. We demonstrate our experience in the acceptable management of a 34-year-old male referred with a closed comminuted fracture of the talar body after falling 7 meters. During the surgery, complete rupture of the peroneus brevis tendon, partial rupture of the peroneus longus tendon, and an avulsed superficial deltoid ligament from medial malleolus were found. Twelve months after open reduction and internal fixation of the talar body fracture and repair of the peroneal tendons and superficial deltoid ligament,

Compartment Syndrome After Isolated Closed Transverse Fibular Shaft Fracture.

In the lower extremity, compartment syndrome has been associated with fractures of the tibial plateau, shaft, and plafond. The patient was an 89-year-old male driver involved in a T-bone type motor vehicle accident. He sustained a closed transverse fibular shaft fracture and initially had fullness in his anterior/lateral compartments but no pain with passive and active range of motion of his ankle. Because of previous cardiac stenting, he was on dual anticoagulation therapy. Serial examination demonstrated tense compartments with notable pain during ankle range of motion. He was taken emergent

Closed technique for naso-orbito-ethmoid fracture management: Technical note.

Trauma to the naso-orbito-ethmoid region invariably results in detachment of medial canthal tendon. The shape of medial canthus is of important esthetic and functional concern. Accurate reposition of the medial canthus is important to achieve normal form and function. The various available techniques advocate open approach leaving an obvious scar in the esthetically prominent region. This technique intends to address these fractures through a closed approach with the possibility to make finer adjustments as and when required. We report the experience of treating 4 naso-orbito-ethmoid fractures
