Association of mixed exposure to microplastics with sperm dysfunction: a multi-site study in China.

BACKGROUND: Microplastics are environmental pollutants detected in various human organs and tissues. These particles originate from multiple sources including the degradation of larger plastic items and the intentional inclusion in consumer goods. Potential risks for human health resulting from microplastics exposure have also been reported. However, the distribution in the male reproductive system and its effect remains largely unknown. This study aims to investigate the presence of multiple microplastics in human semen and urine and their association with sperm quality in a multi-site study

Decompressive craniectomy versus craniotomy for patients undergoing surgical evacuation of an acute subdural hematoma: RESCUE-ASDH RCT and cost effectiveness.

BACKGROUND: Traumatic acute subdural haematomas often require surgical evacuation via craniotomy or decompressive craniectomy. Decompressive craniectomy may prevent intracranial hypertension; however, it is unclear whether it is associated with better outcomes. OBJECTIVE: Multicentre, pragmatic, parallel-group randomised trial to compare the clinical and cost-effectiveness of decompressive craniectomy versus craniotomy for evacuation of acute subdural haematomas. DESIGN: International, multicentre, pragmatic, parallel-group randomised trial with additional observational arm. SETTING: Hospit

Stopping anticoagulation for isolated or incidental subsegmental pulmonary embolism: the challenges and lessons from the STOPAPE RCT.

BACKGROUND: The increasing use of computed tomography pulmonary angiography to investigate patients with suspected pulmonary embolism has led to an increase in diagnosis of small subsegmental pulmonary embolism, which is rarely detectable with nuclear medicine-based imaging, the standard imaging modality prior to the development of computed tomography pulmonary angiography. The case fatality of pulmonary embolism has fallen in line with the increase in subsegmental pulmonary embolism diagnoses from computed tomography pulmonary angiography suggesting that we may be over-diagnosing pulmonary em

Global landscape of locally produced alcohol-based handrub in health care settings: a scoping review.

BACKGROUND: Reliable access to alcohol-based handrub (ABHR) is essential for hand hygiene and infection prevention, yet many low- and middle-income countries (LMICs) continue to face supply constraints. A 2011 WHO global assessment demonstrated that WHO-recommended ABHR formulations produced locally at low cost, were well accepted by healthcare workers, but also highlighted persistent barriers, including challenges in procuring ingredients and dispensers and in ensuring adequate quality control. This review aimed to provide an updated global synthesis of evidence on local ABHR production in he

An innovative approach to improve ear, nose and throat surgical access for remote living Cape York Indigenous children.

INTRODUCTION: On a background of high rates of severe otitis media (OM) with associated hearing loss, children from the Torres Strait and Cape York region requiring ear, nose and throat (ENT) surgery, faced waiting times exceeding three years. After numerous clinical safety incidents were raised, indicating a failure of the current system to deliver appropriate care, the governing Hospital and Health service opted to deliver surgical care through an alternate process. ENT surgeries were performed on 16 consented children from two remote locations via the private health care system, funded by a

Stretching Scarce Authorizing Legislation as Far as Possible: A Legislative History of the 340B Drug Pricing Program.

UNLABELLED: Policy Points The original purpose of the 340B program was to exempt Public Health Service Act funded clinics and state and local public hospitals from the inflationary best-price component of the recently enacted Medicaid drug rebate program. The secondary purpose was to reduce drug prices for these clinics and hospitals in order to preserve and expand access to free and discounted care. The cutoff for the disproportionate share hospital-based eligibility criteria was selected as part of political compromises to qualify specific nonprofit hospitals. CONTEXT: The 1992 340B Drug Pr

Associations of proton pump inhibitors with susceptibility to influenza, pneumonia, and COVID-19: Evidence from a large population-based cohort study.

BACKGROUND: Adverse effects of proton pump inhibitors (PPIs) have raised wide concerns. The association of PPIs with influenza is unexplored, while that with pneumonia or COVID-19 remains controversial. Our study aims to evaluate whether PPI use increases the risks of these respiratory infections. METHODS: The current study included 160,923 eligible participants at baseline who completed questionnaires on medication use, which included PPI or histamine-2 receptor antagonist (H2RA), from the UK Biobank. Cox proportional hazards regression and propensity score-matching analyses were used to est

Developmental profiles of children at risk for autism spectrum disorder at school entry.

Functional abilities in children with autism spectrum disorder (ASD) are highly heterogenous, and impairments can overlap with non-ASD neurodevelopmental disorders. We compared the profiles of children assessed for ASD with and without an ASD diagnosis using a retrospective cohort study of 101,739 children born in British Columbia (2000-2008). The children were grouped into the following five comparison groups: (1) ASD- (n = 1131), (2) ASD+ (n = 1583), (3) Ministry of Education designated ASD+ (n = 654), (4) special need other than ASD (n = 11,663), and (5) typically developing (n = 86,708). Part 3: A community divided.

The task of those who seek to encourage and offer social support has become more difficult as the majority of social institutions, and the state have established, over time, stronger and more pervasive modes of communication. The intricacies of gay identity have been articulated largely by forces outside of the gay movement, with the inevitable result that GSOs and ASOs have occupied less space in the consciousness of gay men. Additionally, I hypothesize that men who are HIV-positive are engaging in fewer sexual contacts than men who are HIV-negative, and consequently lessening their attendanc

Polonium-210 poisoning: a first-hand account.

BACKGROUND: Polonium-210 ((210)Po) gained widespread notoriety after the poisoning and subsequent death of Mr Alexander Litvinenko in London, UK, in 2006. Exposure to (210)Po resulted initially in a clinical course that was indistinguishable from infection or exposure to chemical toxins, such as thallium. METHODS: A 43-year-old man presented to his local hospital with acute abdominal pain, diarrhoea, and vomiting, and was admitted to the hospital because of dehydration and persistent gastrointestinal symptoms. He was initially diagnosed with gastroenteritis and treated with antibiotics. Clost

Changes in breastfeeding support in maternity hospitals due to the COVID-19 pandemic from the perspective of healthcare professionals in Germany.

PURPOSE: The COVID-19 pandemic had an impact on obstetrics and breastfeeding, but data from hospital experiences are rare. This study assessed routines of breastfeeding management in maternity hospitals before and during the pandemic. METHODS: In the SINA study (Breastfeeding in North Rhine-Westphalia, Germany) between October 2021 and March 2022 hospitals had experienced the pandemic for more than one year. In the statewide cross-sectional survey, chief physicians and responsible ward staff were interviewed by phone about breastfeeding support structures and practices before the pandemic, as

Understanding the Mental Health Impacts of Climate Change in South Africa: Qualitative Insights From Researchers, Practitioners, Ethicists, and Funders on the Need for a Transdisciplinary Approach.

BACKGROUND: Against a backdrop of entrenched structural inequalities, poverty, and limited healthcare resources, climate change represents a major public health challenge in South Africa, with significant yet under-recognized consequences for mental health. Expert perspectives on these impacts remain limited, highlighting a critical gap in informing transdisciplinary responses. OBJECTIVES: To explore stakeholder perspectives (researchers/academics, research ethics committee members, employees of organization that funds climate or health research) on transdisciplinary responses to the mental h

A strategic blueprint for strengthening respiratory syncytial virus prevention among under-five children in low- and middle-income countries: Bangladesh as a model for new immunisation approaches.

Respiratory syncytial virus (RSV) is the leading cause of respiratory morbidity and mortality in children under five years of age, especially in low- and middle-income countries (LMICs). In 2023, two RSV immunisation options, including a maternal RSV vaccine and a monoclonal antibody (mAb), became approved in several high-income countries. The World Health Organization has recently prequalified the maternal vaccine and the Gavi has committed funding for its introduction, raising the prospect of RSV vaccine availability in LMICs in the near future. This analysis thus underscores the need for co

Why MASLD Trials Underselect the MENA Region Sites and What Sponsors and Regulators Must Change.

Metabolic dysfunction associated steatotic liver disease (MASLD) and metabolic dysfunction associated steatohepatitis (MASH) are accelerating global priorities, yet pivotal clinical trial activity remains concentrated in North America and Europe despite substantial burden in the Middle East and North Africa (MENA). Building on prior work quantifying MENA underrepresentation, we provide a policy and operational analysis informed by contemporary epidemiological, regulatory, and clinical trial literature and practical experience in trial delivery. We argue that underselection is driven less by sc

One world, one goal: Advocacy and policy for a unified Parkinson's response.

Parkinson's disease is one of the fastest growing neurological disorders with regards to disability and death. Though this burden is felt globally, inequities in research, advocacy, prioritization and funding mean that the needs of people with Parkinson's disease in low- and middle-income countries, and from marginalized communities in high-income countries, remain poorly recognized. Parkinson's disease is increasingly being acknowledged as a global public health issue requiring a public health response. Global advocacy efforts, awareness-raising initiatives, research collaborations, partnersh

Harnessing the World Health Organization's '3 by 35' Initiative to Finance Oral Health Care in India.

BACKGROUND: Oral diseases such as dental caries, periodontal disease, tooth loss and oral cancer affect billions of people worldwide. India, with the world's largest population, carries one of the heaviest burdens of oral and other Non-Communicable Diseases (NCDs). These diseases share common risk factors such as consumption of tobacco, alcohol and sugary food and drinks. One effective approach to reducing the consumption of these products in the population is to increase taxes on them. WHO recently proposed a '3 by 35' initiative, which calls for raising the prices of these harmful products b

"We're all in this together": patient and public involvement and engagement in developing a new psychosocial intervention for adults with an intellectual disability who display aggressive challenging behaviour.

BACKGROUND: Although there is consensus regarding the added value of adults with an intellectual disability and family carers as Patient and Public Involvement and Engagement (PPIE) members in research studies, there is limited reporting on the practice and impact of their involvement. METHODS: PPIE input was integral to the application process and subsequent research activities in the NIHR-funded PETAL (PErsonalised Treatment packages for Adults With Learning disabilities) programme. We also conducted semi-structured interviews with five researchers/PPIE facilitators and four family carers,

Off-label prescribing of tamsulosin: A nationwide retrospective study combining prescription and regulatory data.

BACKGROUND: Off-label use of medicines raises important safety and regulatory concerns. Tamsulosin, an alpha-1 adrenergic receptor blocker originally approved for benign prostatic hyperplasia (BPH) in men, is also prescribed beyond licensed indications. AIM: This study aimed to describe national on-label and off-label prescribing patterns of tamsulosin in Hungary using insurance and regulatory data. METHODS: We conducted a retrospective, cross-sectional descriptive analysis of two national datasets: the National Health Insurance Fund (NEAK) database of reimbursed prescriptions (2019-2023) an

National Cancer Grid initiative for electronic medical records, India.

PROBLEM: Inefficient workflows, incomplete data and lack of interoperability can hinder the uptake of electronic records systems, challenges particularly relevant in cancer treatment with its complex longitudinal and multidisciplinary nature. Further, products developed in high-income countries are not designed for compatibility with the workflows of low- and middle-income countries, which face additional issues of cost. APPROACH: We evaluated centres with different resources and geographical locations to develop the requirements of our product. We published an invitation to potential vendors

A Fresh Fruit and Vegetable Prescription Program for Prenatal Patients in Flint, Michigan: Baseline Food Security and Dietary Intake.

Although adequate nutritional status during pregnancy is necessary to support optimal fetal development, many low-income women have poor access to fresh, high-nutrient foods. To address these challenges, a pediatric fruit and vegetable (FV) prescription program was expanded to include pregnant women, providing one prescription for fresh FVs worth 15 US dollars during each prenatal office visit for redemption at farmers'/mobile markets. This analysis describes baseline sociodemographic characteristics, food security, and dietary intake among 253 pregnant women in Flint, Michigan in 2022-23. Die
