The paper presents the results of the 10-year follow-up of 1,237 patients at high risk for stomach cancer. Among them, 669 patients suffered from chronic gastric ulcer, 78--polyps, 382--chronic gastritis and 108 underwent resection of the stomach more than 10 years ago. Repeated gastroscopic examinations, including biopsy, carried out within the last two years, revealed malignancies in 405 cases (32,7%): chronic ulcers--10.8, polyps--11.1 and chronic gastritis--0,6%. Gastroscopy was performed in 543 patients with an average of 52.8 years. Of these patients 248 had atrophic gastritis. In over one third of the 248 patients the gastritis inclosed the entire mucosa. PURPOSE: The safety and efficacy of esophageal dilation is enhanced when direct visualization is possible, tactile sensation and feedback from resistance is maintained, and multiple dilations with a single passage of the dilating device is an option. The InScope Optical Dilator (InScope, a Division of Ethicon Endo-Surgery, Inc, Cincinnati, Ohio) is a flexible, transparent bougie fitted over a standard endoscope, with 3 dilating segments allowing sequential dilation under direct visualization. The endoscopic diagnoses of 51 patients with carcinoma of the oesophagus and cardia were studied. Tissue for histology and cytology was obtained through either the flexible (28) or rigid (23) endoscope and the results compared. Patients with arthritis who take nonsteroidal anti-inflammatory drugs (NSAIDs) often develop peptic ulceration.
