Reducing inequalities in cardiovascular disease: focus on marginalized populations considering ethnicity and race.

Cardiovascular disease (CVD) and its risk factors are more prevalent among traditionally marginalized racial, ethnic, and Indigenous groups. These populations also often face greater barriers to accessing cardiovascular health care, further contributing to the health equity gap. To address the challenge of inequalities and disparities in cardiovascular health outcomes, the Lancet Regional Health-Europe convened experts to evaluate the current state of knowledge on inequalities and disparities in cardiovascular health among marginalized populations and propose recommendations to address these d

Incorporating recipient choice in kidney transplantation.

Despite the acute shortage of cadaveric organs for kidney transplantation, more than 10% of cadaveric kidneys are discarded each year because of marginal quality. Transplant recipients' access to these kidneys and to information about their quality is limited. A Monte Carlo model was developed to simulate the operations of an organ procurement organization over a 10-yr period. Donor and recipient characteristics were generated from the United States Renal Data System. Kidneys were assigned one of five possible grades, which were determined by calculating the relative risk of graft failure asso

How early adversity shapes adult physical health: Structural disadvantage and gendered pathways to cardiovascular risk.

OBJECTIVES: Adverse childhood experiences (ACEs) are well-established predictors of mental health problems, yet their long-term impact on physical health, especially cardiovascular disease (CVD), remains less well understood. Guided by a life course perspective, this study investigates the association between ACEs and adult CVD risk, with a focus on the mediating role of structural disadvantage and gender-specific pathways. STUDY DESIGN: Using data from Waves I, III, and IV of the National Longitudinal Study of Adolescent to Adult Health (Add Health), we analyzed a sample of 12,224 individual

"There is a target on you": Female neurosurgeons' and cardiothoracic surgeons' experiences of overt and covert discrimination in the workplace.

BACKGROUND: The purpose of this study was to explore how gender-based discrimination manifests in neurosurgery and cardiothoracic surgery by interviewing women surgeons in these fields about their lived experiences. METHODS: Interviews were conducted via videoconference with 22 female surgeons (18 neurosurgeons and 4 cardiothoracic surgeons) practicing in academic and community settings in North America. A thematic analysis based on grounded theory principles was used to code the data for major themes. Results encompass inductively derived themes describing experiences of gender discriminatio

Gender Diversity Among Surgeons on US ACPA-Approved Cleft and Craniofacial Teams.

BACKGROUND: Gender diversity among surgeons, particularly within subspecialties, is limited compared with other medical professions. Although diverse patient care teams have been shown to improve patient trust, communication, and overall care, female representation among multidisciplinary teams and in leadership roles remains low. This study aims to evaluate gender diversity among surgeons on American Cleft Palate Craniofacial Association (ACPA)-approved cleft and craniofacial teams in the United States, with emphasis on subspecialty representation and leadership roles. METHODS: Cleft teams a

Through the scope, not the ceiling: Gender distribution among endoscopy sessions at the German Visceral Medicine Congress.

BACKGROUND AND STUDY AIMS: Despite awareness of the importance of diversity, equity, and inclusion (DEI) in endoscopy, structural impediments persist. Disparities include underrepresentation of women as speakers and chairs at endoscopy congresses. METHODS: Data from endoscopy sessions of the annual German Visceral Medicine congresses from 2013 to 2024 were analyzed. Gender ratios of speakers, chairs, leadership, and award recipients were analyzed over 336 sessions using linear regression and the Cochran-Armitage test for trend. Epistemic injustice in healthcare: How doctor gender and speciality shape patient voice in women's thyroid care.

Women's health concerns are frequently dismissed in clinical settings, a pattern described as medical gaslighting. Drawing on Fricker's (2007) framework of epistemic injustice and Carel and Kidd's (2014) extension to healthcare, this study conceptualises patient voice, encompassing feelings of involvement, empowerment, and being heard, as a measurable correlate of epistemic injustice in the clinical encounter. We distinguish two forms: testimonial injustice, where a patient's symptom account is assigned insufficient credibility, and hermeneutical injustice, where gaps in interpretive framework

A discourse analysis of social inequities, gender, and stigma in tuberculosis policies of seven countries from Africa, Asia, Europe and South America.

BACKGROUND: Interventions tackling the social aspects of tuberculosis (TB) are widely suggested, yet we miss insights into how policies incorporate these. The language and framing of policies to address TB can lend important insights into how these social drivers are perceived, problematized, and responded to. OBJECTIVE: To understand how discourses in current TB policies frame social dimensions of TB, especially concepts of social inequity, gender, and stigma. METHODS: We conducted a comparative critical discourse analysis of twenty-one publicly available TB-related policies from Belarus, B

A Layered Psychodynamic Model of Suicidal Identity Development: Identification and the Senses of Self.

Suicide in adolescents and emerging adults is often understood through symptoms and risk factors rather than as a disturbance of the self. This article presents a conceptual analysis and model building study that elaborates the Existential Network of Suicidal Identity (ENSI) with a specific focus on existential anchors and the senses of self they support. Drawing on object relations and defense theory, mentalization and attachment research, existential psychology on meaning and psychache, and narrative identity work on coherence and foreclosure, we develop a layered psychodynamic account of su

Stress Processes and Resilience Pathways of Sexually and Gender Diverse Communities: A Narrative Review of 15 Years of Evolving Research Priorities and Practices.

Sexually and gender diverse (SGD) people in North America have navigated significant sociopolitical shifts that continue to shape their lived experiences. Given the persistent exposure to stigma, SGD communities are disproportionately vulnerable to stress-related health disparities. While sexual and gender minority stress models highlight the detrimental impact of stigma, research also emphasizes the role of protective factors and resilience pathways in mitigating these effects. This narrative review of my research program over the last 15 years explores stress physiology as a potential mechan

X ≠ Y: The Need for Gender-Specific Pain Education-A Perspective Shift.

The perception of pain differs between males and females owing to inherent biological distinctions. Social, cultural, cognitive, economic, and occupational factors play significant roles in shaping pain perception in women. Pain education aims to help individuals better understand their pain. Academicians, researchers, and clinicians worldwide have embraced person-centred treatment strategies. Despite our growing understanding of pain, gender-specific pain education methods are yet to be developed. This narrative review aims to explore pain experiences in women from social, cultural, economic,

Global and regional burden and quality of care of non-rheumatic valvular heart diseases: a systematic analysis of Global Burden of Disease 1990-2017.

BACKGROUND: With an increase in the incidence and prevalence of non-rheumatic valvular heart diseases (NRVHDs), having a proper understanding of the disease current status in terms of quality of care and healthcare access can considerably affect further planning for the healthcare system. OBJECTIVE: In this study, we aimed to evaluate and compare the quality and equity of care concerning NRVHDs in terms of gender and sociodemographic index (SDI) using a newly proposed index. METHODS: We obtained the primary measures (e.g. incidence) from the Global Burden of Disease (GBD) data about NRVHD fr
