INTRODUCTION: In Brazil, shoulder surgeries are traditionally performed under general anesthesia combined with regional blocks. This study aimed to describe the feasibility and safety profile of regional anesthesia with sedation, without general anesthesia, for shoulder surgery.
MATERIALS AND METHODS: Following approval by the institutional ethics committee, 10 consecutive patients undergoing elective shoulder surgery (arthroscopy, n = 6; arthroplasty, n = 4) received an ultrasound-guided superior trunk block of the brachial plexus combined with an intermediate superficial cervical plexus blo

INTRODUCTION: Gastrointestinal complications requiring emergency general surgery (EGS) after cardiac surgery are associated with high morbidity and mortality. Identifying predictors of 30-d mortality and intuitive preoperative laboratory-based futility thresholds may enhance risk stratification and clinical decision-making.
MATERIALS AND METHODS: We conducted a single-center retrospective cohort study of adults aged 18-90 y who underwent cardiac surgery between 2013 and 2023. Patients requiring EGS intervention for gastrointestinal complications during their index cardiac surgery hospitalizat…

BACKGROUND: High-quality patient care necessitates knowledge of health disparities and strategies for cultural humility, yet no widely accepted health equity curriculum is available for general surgery residents. We developed such a curriculum based on adult learning theory and local public health priorities. The format included quarterly cultural complications Morbidity and Mortality conferences, interprofessional panels, and case-based discussions. We aimed to assess the impact of the integration of this curriculum.
METHODS: Postsession surveys and focus groups with general surgery resident…

Artificial intelligence (AI) and machine learning (ML) models rapidly transform health care with applications ranging from diagnostic image interpretation, predictive modeling, personalized treatment planning, real-time intraoperative guidance, and outcome prediction. However, their implementation in general pediatric surgery remains limited due to the rarity and complexity of pediatric surgical conditions, small and heterogeneous datasets, and a lack of formal AI training and competencies among pediatric surgeons. This narrative review explores the current landscape of AI and ML applications i

BACKGROUND: Surgical residents experience higher levels of negative stress and helplessness compared to the general population. Studies have linked stress to negative eating habits. Despite the high stress and burnout among surgical residents, studies on their disordered eating behaviors remain limited. Understanding the factors contributing to these findings will help optimize mental health during residency training.
METHODS: This study is a mixed-methods cross-sectional survey of all general surgery residents in Canada. The survey assessed disordered eating, quality of life, and self-compas…

INTRODUCTION: Vaginal approaches have become routine in the field of gynecologic surgery, whereas in general surgery vaginal wall transection is an infrequent practice typically reserved for extensive tumor resections. Approximately two decades ago, natural orifice transluminal endoscopic surgery (NOTES) revolutionized conventional boundaries by accessing the peritoneal cavity transorally, transrectally, or transvaginally, enabling general surgery without visible scars. Although transvaginal approaches have been successfully used for various abdominal procedures by general surgeons, a gap rema…

BACKGROUND: Emergent surgical conditions are common in geriatric patients, often necessitating major operative procedures on frail patients. Understanding risk profiles is crucial for decision-making and establishing goals of care.
METHODS: We queried NSQIP 2015-2019 for patients ≥65 years undergoing open abdominal surgery for emergency general surgery conditions. Logistic regression was used to identify 30-day mortality predictors.
RESULTS: Of 41,029 patients, 5589 (13.6 %) died within 30 days of admission. The highest predictors of mortality were ASA status 5 (aOR 9.7, 95 % CI,3.5-26.8, …

OBJECTIVE: Policy concern and debate surround the concept of overlapping spine surgery. Overlapping surgery specifically refers to nonessential portions of the case or noncutting time overlap. This differs from concurrent surgery, in which critical portions of the procedure overlap. Here the authors explore the barriers for safe and efficient overlapping surgery in academic spinal deformity practice.
METHODS: Over a 24-month period, cases of spinal deformity, degenerative cases, anterior cervical discectomy and fusions (ACDFs), and laminectomy were reviewed for duration in operating room (OR)

Intra-abdominal infections (IAI) are among the most common global healthcare challenges and they are usually precipitated by disruption to the gastrointestinal (GI) tract. Their successful management typically requires intensive resource utilization, and despite the best therapies, morbidity and mortality remain high. One of the main issues required to appropriately treat IAI that differs from the other etiologies of sepsis is the frequent requirement to provide physical source control. Fortunately, dramatic advances have been made in this aspect of treatment. Historically, source control was

BACKGROUND: Emergency general surgery (EGS) conditions are increasingly common among nursing home residents. While such patients have a high risk of in-hospital mortality, long-term outcomes in this group are not well described, which may have implications for goals of care discussions. In this study, we evaluate long-term survival among nursing home residents admitted for EGS conditions.
METHODS: We performed a population-based, retrospective cohort study of nursing home residents (65 years or older) admitted for one of eight EGS diagnoses (appendicitis, cholecystitis, strangulated hernia, b…

Introduction Surgery can be dangerous not only for patients, but it also carries a risk for the surgeon in case of medical error, which can affect their future medical career. The purpose of our research is to assess the current situation regarding medical liability in General Surgery in Greece, the reasons for the allegations of medical malpractice as well as the relationship between these issues and the court results. Methods Published court decisions of criminal, civil, administrative and disciplinary content were searched in legal information banks, between year 1973 and 2020. Court decisi…

Smarter Medicine: From the Diagnosis to the Intervention in General and Visceral Surgery Abstract. More is not always a plus. With this slogan, in smarter medicine it is a principle to make a decision together. In general surgery diagnostic or therapeutic interventions have the potential to harm the patient. The admission to the emergency room of a young patient with abdominal pain does not necessarily require a computed tomography. A potential acute appendicitis without typical clinical and laboratory findings could potentially be unmasked by watchful waiting, or even a nonessential operation…

BACKGROUND: Emergency general surgery (EGS) is a high-volume and high-risk surgical service. Interhospital variation in EGS outcomes exists, but there is disagreement in the literature as to whether hospital admission volume affects in-hospital mortality. Scotland collects high-quality data on all admitted patients, whether managed operatively or nonoperatively. Our aim was to determine the relationship between hospital admission volume and in-hospital mortality of EGS patients in Scotland. Second, to investigate whether surgeon admission volume affects mortality.

BACKGROUND: Gastrostomy placement is the preferred means of long-term enteral feeding for patients who cannot eat by mouth. During laparoscopic gastrostomy, it is standard to perform gastropexy, apposing visceral and parietal peritoneum. In some settings, due to altered anatomy from prior surgery, direct apposition of the stomach to the abdominal wall is not possible. This study reports a series of cases where laparoscopic gastrostomy was performed via a Witzel approach without gastropexy.
METHODS: A retrospective chart review was performed of all patients at a tertiary academic medical cente

BACKGROUND: Emergency general surgery (EGS) conditions account for over 3 million or 7.1% of hospitalizations per year in the US. Patients are increasingly transferred from community emergency departments (EDs) to larger centers for care, and a growing demand for treating EGS conditions mandates a better understanding of how ED clinicians transfer patients. We identify patient, clinical, and organizational characteristics associated with interhospital transfers of EGS patients originating from EDs in the United States.

BACKGROUND: The devastating effects of the opioid epidemic are well documented. We implemented a surgeon/pharmacist opioid reduction initiative at an academic medical center that incorporated multimodal pain therapy in an attempt to reduce total inpatient opioids prescribed. We hypothesized that less opioids would be used postoperatively without affecting pain scores or length of stay.
METHODS: This single-center observational cohort analysis included patients admitted to the acute general surgical service and had one of 10 emergent general surgical (nontrauma) procedures. Patients who underw
