[Epidemiology of listeriosis in Germany: a changing but ignored pattern].

Listeriosis is emerging as a food-borne disease. Although the incidence of clinical presentations is low, this infectious disease with a rather poor prognosis deserves attention. Approximately 20-30% of patients with recognized listeriosis die in spite of an appropriate antibiotic regimen. In the last two years the number of registered cases of listeriosis in Germany has more than doubled. This increase is clearly due exclusively to the rising number of infections in elderly persons, while the number of infections in newborns has remained constant. Genotyping does not show a conclusive correla…

Demographic characteristics of patients with extrapulmonary tuberculosis in Germany.

The aim of the present study was to determine the demographics of patients with extrapulmonary tuberculosis in Germany. Data on 26,302 tuberculosis cases from a national survey carried out during the period 1996-2000 were analysed. The crude proportion of tuberculosis patients with extrapulmonary manifestations was 21.6%. Extrapulmonary tuberculosis was most likely among females, children aged <15 yrs and persons originating from Africa and Asia. Females tended to be more likely to have any form of extrapulmonary tuberculosis than males, except pleural tuberculosis. The strength of this associ…

[A controversial definition of the limitations on reproductive medicine in Germany].

In principle, all kinds of sterility therapy can be defined as "medicine of desire". Stimulation, insemination, in-vitro fertilisation and ICSI are established methods and paid for by statutory health insurers. Some types of sterility therapy, though, are forbidden in Germany and thus form part of a "medicine of desire". These methods are only practised in foreign countries: oocyte donation, pre-implantational genetic diagnosis, embryo selection, surrogate motherhood, therapeutic cloning and preparation of embryonic stem cells. Taking oocyte donation as an example, the question of whether it i…

[Paediatric laryngotracheal stenosis: pattern of care in Germany].

BACKGROUND: Stenoses of the larynx and upper trachea in the pediatric population require a multidisciplinary approach and center-based treatment. Patterns of care and treatment strategies in Germany were evaluated using a standardized questionnaire. MATERIAL AND METHODS: A questionnaire was transmitted by telefax to 141 departments of Otorhinolaryngology (ORL) and to 214 pediatric (PED) departments. Using primarily multiple-choice questions variables regarding caseload, surgical techniques, cooperating institutions, criteria for surgical indications and subjective assessment of success rates …

Regional variation in outpatient antibiotic prescribing in Germany.

OBJECTIVE: According to recent surveys, outpatient antibiotic prescribing in Germany has been comparatively low among European countries. We assessed regional variation in outpatient antibiotic use within Germany both for overall use as well as for specific antibiotic drug classes. METHODS: Prescription data for the year 2003 covering approximately 90% of the total population were analysed using the ATC/WHO defined daily dose (DDD) methodology. Data were expressed in DDD per 1,000 persons covered by the insurance and day (DID). RESULTS: Outpatient antibiotic prescribing in 2003 was 13.6 DID …

[Nation-wide registration of limb deficiencies in Germany].

AIM: A detailed registration of the frequency and the aetiology of limb deficiencies has been established for the first time in Germany. METHOD: According to the ESPED model (Documentation of rare paediatric diseases in Germany) we send out at an interval of three months a questionnaire for registration of live births, still births and their limb deficiencies to 1 073 gynaecological hospitals. In case of a positive response for a limb deficiency a second questionnaire is sent for details about pregnancy, birth and family. The limb defects are registered according to the ICD-10 classification …

Cases of rabies in Germany following organ transplantation.

UNLABELLED: Management of periacetabular metastatic bone disease (MBD) is challenging, specifically if associated with bone loss or fracture. The aim of this study was to evaluate the complications and outcomes after undergoing peri-acetabular reconstruction using an 'ice-cream cone' pedestal cup endoprostheses for the most severe cases of (impending) pathological acetabular fractures. Fifty cases with severe periacetabular disease were identified. Acetabular defects were classified using the Metastatic Acetabular Classification (MAC). Pre- and post-operative mobility was assessed using the Ea

Epidemiology of staphylococcal scalded skin syndrome in Germany.

Staphylococcal scalded skin syndrome (SSSS) is a blistering skin disorder caused by Staphylococcus aureus. The "Dokumentationszentrum schwerer Hautreaktionen", a unique population-based registry for severe skin reactions, included SSSS during a time period of 2 y in Germany. Statistical calculations indicated a low overall incidence between 0.09 and 0.13 cases per 1 million inhabitants per year with 95% confidence interval of [0-4]. The age distribution showed two clusters; one in young children and one in adults. The mortality rate was much lower in children than in adults. Young age was the …

A proposal for an anonymous living organ donation in Germany.

In Germany, living organ donation of paired and usually not regenerating organs is restricted by law to related individuals, as well as persons who 'obviously entertain an especially intimate personal relationship'. When this law was adopted in 1997, the intention of the legislator was to guarantee the free will of the donor and to exclude any trade of organs. Since then the transplantation of cadaveric organs has not increased. Additional organs were donated from living donors. However, for a number of reasons only a limited array of transplantation centers use living organ donation as a supp…

The seroepidemiology of Lyme borreliosis in zoo animals in Germany.

We conducted the first seroepidemiological study to evaluate the exposure of zoo animals to Borrelia burgdorferi s.l. in German zoos and wildlife parks. A total of 1487 individuals representing 148 ungulate and carnivore species belonging to 19 families were examined using a non-species dependent ELISA. Specific antibodies were detected in 154 (10.4%) animals; 168 (11.3%) sera produced borderline results. The percentage of seropositive individuals was related to species and origin (zoo), and increased with age of the animals. Sex and season did not influence seroprevalence. Examination of 600

[Current epidemiology of carcinoma of the esophagus and cardia in Germany].

BACKGROUND AND OBJECTIVE: In the past 20-30 years the incidence of adenocarcinoma (AC) of the esophagus and the cardia has risen in western countries, while that of squamous cell carcinoma (SC) of the esophagus has largely stayed constant. Current data in the epidemiology of these cancers in Germany were analysed. METHODS AND MATERIAL: The data of the epidemiological cancer register of the Saarland (Old State of former West Germany) and the combined registers of the New States of Germany (former East Germany) and Berlin were collected. Because of the excellent registration procedures, data fr…

[Anisakiasis of the stomach--a case report from Germany].

Anisakiasis or "herring worm disease" is one of the most important parasitic diseases of the gastrointestinal tract in Japan. In 1988 Lorenz and Warzok published 8 cases of intestinal anisakiasis in Eastern Germany. In 1988 Spehn et al. reported a case of gastric anisakiasis in an AIDS patient. Here, we describe a case of gastric anisakiasis in Germany with an impressive serious clinical course. The symptoms--acute abdominal cramps, severe chest pain, diarrhoea, sub-febrile temperatures and leucocytosis--followed 4 h after consumption of raw herring, which was homemade pickled in vinegar. The …

Epidemiology of classical swine fever in Germany in the 1990s.

In Germany, 424 outbreaks of CSF in domestic pigs and a great number of cases in wild boar were recorded between 1990 and 1998. Most of the federal states ('Bundesländer') were affected. Epidemiological data from field investigations combined with genetic typing allowed to distinguish seven unrelated epidemics and a number of sporadic outbreaks in domestic pigs. Detailed epidemiological data was available for 327 outbreaks. It was found that 28% of these were primary outbreaks. Most of them were due to indirect or direct contact to wild boar infected with CSF virus or swill feeding. Infected w…

Incidence of Lyme borreliosis in the Würzburg region of Germany.

To assess the incidence of Lyme borreliosis in Central Europe, a 12-month, prospective, population-based surveillance study of Lyme borreliosis was conducted in the Wurzburg region of central Germany, following an aggressive awareness campaign. The diagnosis of Lyme borreliosis required the presence of (i) erythema migrans (diameter > or =5 cm); (ii) lymphocytoma; or (iii) another specific manifestation including Lyme arthritis, neuroborreliosis, carditis or acrodermatitis chronica atrophicans in conjunction with serological confirmation. A total of 313 cases of Lyme borreliosis was diagnosed,…

[Tick-encephalitis after a vacation in Southern Germany].

A 38-year-old man was admitted with signs of meningoencephalitis. Tick-borne encephalitis was considered because the patient reported a tick bite during a vacation in South Germany and because of the biphasic course of the symptoms. Serology for tick-borne encephalitis was positive: this constituted the first reported positive serology outcome in the National Institute for Public Health and Environment in the Netherlands. Tick-borne encephalitis is an infection with a flavivirus transmitted by an infected tick, which is endemic in parts of Central and Eastern Europe. The neurological symptoms …

Epidemiology of alveolar echinococcosis in southern Germany (Bavaria).

Alveolar echinococcosis is considered to be the most dangerous endemic parasitic disease for man in Central Europe. In Germany, unlike the neighbouring countries of Switzerland, Austria and France, only limited data on the prevalence and incidence of echinococcosis are available. Therefore, a retrospective cross-sectional study was conducted in order to investigate the epidemiology of echinococcosis in Bavaria, one of the two southern states of Germany. A standardised questionnaire was sent to all hospitals in Bavaria requesting information about patients seen from 1985 to 1989. In a second st…
