Management of inadequate keratinized gingiva and millers class III or IV gingival recession using two-stage free gingival graft procedure.

BACKGROUND: Prognosis in the management of Millers Class III or IV gingival recession is often fair to poor, which is further decreased by factors such as inadequate keratinized gingiva. Very few modalities show long-term success in the management of gingival recession with inadequate keratinized gingiva, free gingival graft (FGG) being one such technique. MATERIALS AND METHODS: Ten individuals with Millers Class III or IV recession and inadequate keratinized gingiva were recruited for the study. The first surgical procedure involved the FGG procedure to increase the width of keratinized ging…

C-Reactive Protein in Peripheral Blood of Patients with Chronic and Aggressive Periodontitis, Gingivitis, and Gingival Recessions.

CRP is a plasma protein that reflects a measure of the acute phase response to inflammation and is one of the markers of choice in monitoring this response. CRP can be used for the prediction and early detection of periodontal disease. The aim of this study was to compare and evaluate the systemic levels of CRP in the peripheral blood samples of patients with chronic and aggressive periodontitis, gingivitis, and gingival recessions and compare them with periodontal clinical parameters. All patients (N = 158) were examined prior to the initiation of periodontal treatment. Patients were divided …

Treatment of localized gingival recessions using gingival unit grafts: a randomized controlled clinical trial.

BACKGROUND: One of the success factors in periodontal plastic surgery is the synergistic relationship between involved tissues and vascular supply. Gingiva as a functional unit is unique with a specific vascular configuration and contains the supracrestal portion naturally created to survive over avascular root surfaces. The aim of this randomized controlled trial is to clinically evaluate the treatment of localized gingival recessions by using gingival unit grafts (palatal tissue involving marginal gingiva and papillae) compared with conventional palatal grafts. METHODS: Seventeen patients w…

Treatment of localized gingival recessions. Part II. Coronally repositioned flap with a free gingival graft.

Fourteen teeth with localized gingival recessions were treated using a coronally repositioned flap with a free gingival graft (Bernimoulin, 1973). The second step of the procedure was performed 1 month after the free gingival graft was done. Clinical measurements of the recession, sulcus depth and keratinized gingiva were taken preoperatively and at 30, 90 and 180 days after surgery. A mean reduction in the recession of 2.73 mm was obtained after 6 months, which was equivalent to a 64% decrease of the original recession. A significant increase in the width of the keratinized gingiva, averaging…

Effectiveness of Plasma-Rich Fibrin and De-Epithelialized Free Gingival Graft in the Treatment of Gingival Recessions.

Introduction/Aim: Soft tissue dehiscences such as gingival recessions are a very common problem that we face in modern periodontics. This clinical study aimed to analyze the effectiveness of surgical procedures using a de-epithelialized gingival graft (DGG) combined with a coronally advanced flap and to evaluate the application of plasma-rich fibrin (PRF). Methods: The study included 40 teeth (20 patients) with Miller class I and II gingival recessions. Twenty recessions (20 patients) were treated utilizing the de-epithelialized gingival graft in combination with the coronally advanced flap, a…

Effects of two toothbrushes on plaque, gingivitis, gingival abrasion, and recession: a 1-year longitudinal study.

Plaque removal is an important part of good oral hygiene. Many short-term studies have compared the effectiveness of an electric toothbrush in plaque removal to that of a conventional, manual toothbrush. Previous studies proved the electric toothbrush to be more effective on a short-term basis; however, little is known about its long-term effects. In a study that examined 32 patients over a 12-month period, the authors found electric toothbrushing to be more effective in plaque removal. The study measured the differences in abrasion, gingival recession, gingival bleeding, and plaque scores. Th…

Minimally Invasive Coronally Advanced Flap Techniques for Correction of Gingival Recession Defects: A Review.

For treatment of patients with isolated and/or multiple adjacent gingival recession defects, debate exists as to whether minimally invasive coronally advanced flap-based surgical techniques achieve improved root coverage compared with traditional evidence-based techniques. Correction of gingival recession defects requires mucogingival surgery utilizing tissue grafting and/or soft-tissue alternatives. Traditional mucogingival techniques for root coverage using autogenous tissue are associated with patient morbidity and discomfort. Considering the philosophical shift in daily practice from "pati…

Cryopreserved Umbilical Cord Allograft for Root Coverage of Gingival Recession Defects: A Case Series.

Gingival recession is a component of periodontal disease that leads to exposure of the root surface, root hypersensitivity, and esthetic concerns. Treatment of these cases is typically performed with autologous grafts harvested from the palate. However, this is associated with a second surgical site that increases surgical time and patient morbidity. A potential alternative is the use of cryopreserved umbilical-cord allografts, which are known to contain anti-inflammatory properties and have been used clinically by numerous medical specialties. In this observational series, three patients with…
