In 148 patients after major surgical procedures urinary endotoxin levels were determined and compared with bacteriological results. The study was designed as a screening study. Urine samples were collected once by suprapubic or transurethral catheters. In a first series of 49 patients urine bacteriology was positive (mainly, Gram-negative rods were found) in 3 cases. However, endotoxin determination was positive in these 3 patients and in a further 10 patients receiving antibiotic therapy for other reasons. Therefore, the following 99 patients were studied also by urinalysis by reagent strips

Bacillus cereus is a Gram-positive, rod-shaped facultative anaerobe commonly associated with gastrointestinal infections. Although typically regarded as a blood culture contaminant, B. cereus can cause severe infections, including bacteremia, in high-risk patients. We report a case of a 39-year-old male with a history of intravenous drug use (IVDU) presenting with fevers and found to have B. cereus bacteremia. Cultures remained persistently positive for B. cereus despite 7 days of therapeutic vancomycin. Gentamicin was initiated for Gram-positive synergistic effect with vancomycin. This combin

Rat bite fever (RBF) is a rare infectious zoonotic disease caused by two bacterial species: the Gram-negative rod Streptobacillus moniliformis and the Gram-negative coiled rod Spirillum minus. The association between RBF and skin vasculitis and arthritis has been observed. The aim of this paper was to present a case of rat-bite fever with symptoms of skin vasculitis and arthritis, associated with high titers of ANCA antibodies and anti-endothelial cell antibodies suggestive of primary vasculitis. The patient was successfully treated with antibiotics and non-steroidal anti-inflammatory drugs, l

A 63-year-old man with follicular lymphoma was administered standard R-CHOP chemotherapy. Six days after the second course of chemotherapy, the patient developed fever and chills. Blood cultures yielded rod-shaped gram-negative bacteria, but no further identification was obtained. High fever and chills returned on the fifth and sixth days after the third and fourth courses of R-CHOP, respectively. These blood cultures were also positive. Since we detected spiral-shaped gram-negative rods, we performed a prolonged culture during the febrile period after the fourth course of R-CHOP. This reveale

A 65-year-old Japanese male farmer with advanced gastric adenocarcinoma and multiple hepatic metastases was admitted to our hospital. Blood culture results were positive on day 5, and Gram-positive rods were detected. According to the results of Ziehl-Neelsen staining and a cultured colony of this bacterium, we suspected a mycobacterial infection. Suspecting a rapidly growing mycobacterium (RGM), we started multidrug therapy with levofloxacin, clarithromycin, and ethambutol, and the patient recovered from the bloodstream infection. Further gene examination (16S rRNA, hsp65, and sodA) revealed …

The isolation and characterization of a new methanogenic bacterium, Methanobacterium arbophilicum, is described. Isolation from wetwood enrichment cultures, that were obtained from methane-positive trees, required a medium containing inorganic salts, vitamins, and an atmosphere consisting of an 80:20 mixture of hydrogen-carbon dioxide. Isolates of M. arbophilicum were gram-positive, non-motile short rods that occurred singly, in pairs, or chains. The organism was found to be an autotroph and a strick anaerobe, and to have a pH optimum of 7.5-8.0. The optimal temperature for growth was 30 to 37

Background: There are concerns with inappropriate prescribing of antibiotics in hospitals especially broad spectrum in Pakistan and the subsequent impact on antimicrobial resistance rates. One recognized way to reduce inappropriate prescribing is for empiric therapy to be adjusted according to the result of culture sensitivity reports. Objective: Using culture sensitivity reports to optimize antibiotic prescribing in a teaching hospital in Pakistan. Methods: A retrospective observational study was undertaken in Ghurki Trust Teaching Hospital. A total of 465 positive cultures were taken from pa

Myroides species are rare opportunistic pathogens that are not part of the normal human flora and can cause infections, particularly in immunocompromised individuals. They are aerobic, non-motile, yellow-pigmented, oxidase-positive, gram-negative, rod-shaped bacteria commonly found in environmental sources such as water and soil. In this case presentation, Myroides odoratimimus is reported as the causative agent of a bloodstream infection in a patient with a prolonged ICU stay due to chronic obstructive pulmonary disease (COPD), type 2 respiratory failure, and acute renal failure.

We report a case of septic shock caused by Slackia exigua, an obligatory anaerobic gram-positive rod, in an 82-year-old woman with diabetes. Dental assessment revealed a palatal lesion and untreated periodontitis. Although a resident species in the oral cavity and associated with localized disorders, S. exigua can cause extra-oral diseases, which can be fatal in individuals with risk factors, such as diabetes. Thus, control of oral lesions caused by S. exigua is important to prevent systemic infection.

Erysipelothrix rhusiopathiae is a facultative anaerobic Gram-positive rod that occurs widely in nature and is best known in veterinary medicine for causing swine erysipelas. In humans, infections are rare and mainly considered as occupationally acquired zoonosis. A case of E. rhusiopathiae bacteremia most likely associated with home freshwater aquarium handling is reported. The route of transmission was probably a cut with the dorsal fin of a dead pet fish. A short review of clinical presentations, therapeutic considerations and pitfalls of E. rhusiopathiae infections in humans is presented.

Anthrax is a disease caused by Bacillus anthracis, a gram-positive, aerobic, spore-forming rod. It is also used as a biological weapon. The primary care physician will be the public's first line of defense in the event of a widespread anthrax exposure and will play a crucial role providing postexposure prophylaxis. Understanding the disease process will allow the primary care physician to play an essential role in protecting and educating the public. Accurate diagnosis and quick treatment will save many lives and help calm the panic following a biological attack.

Listeria monocytogenes is a gram-positive bacillus that causes food poisoning. Listeriosis causes gastrointestinal infections and occasionally leads to fatal bacteremia in older adults. The symptoms of Listeria infections are non-specific and difficult to diagnose. We describe a case of Listeria bacteremia in an 82-year-old Japanese woman who had handled raw venison one month prior to becoming ill, but had not consumed any. No other possible sources of infection were identified. She presented with a fever without any focal symptoms. Computed tomography revealed enteritis with mucosal damage. B
