The cause of sexually acquired genital ulceration was studied in 240 consecutive migrant mineworkers attending a sexually transmitted disease clinic in Carletonville, Transvaal. Chancroid proved to be the most common cause of genital ulcer disease, with Haemophilus ducreyi being isolated from 164 patients (68%). In 60% of cases the disease was locally acquired and in the remaining 40%, the infection was acquired in Lesotho, Botswana, Natal, Transkei and Malawi. INTRODUCTION: Donovanosis (granuloma inguinale) is a bacterial infection caused by Klebsiella granulomatis that occurs mainly in the genital area and is primarily sexually transmitted; it is seen predominantly in the tropics. Pseudoelephantiasis of the penis, an unusual sequel of longstanding donovanosis with otherwise characteristics clinical expression, is reported in a young man. The diagnosis was confirmed by the demonstration of Donovan bodies in Giemsa stained tissue smears and tissue sections.

One hundred and sixteen patients with granuloma venereum were treated with cotrimoxazole two tablets twice daily. All the patients responded; most of them required treatment for 10-15 days. Only two patients had recurrences, which were probably due to reinfections. No side effects of the drug occurred. Co-trimoxazole is considered to be the treatment of choice for granuloma venereum because of its high efficacy, few side effects, easy administration, and absence of any risk of masking concomitant syphilis.

Co-trimoxazole (Septrin, Wellcome) was prescribed in a dose of two tablets twice daily for the treatment of 10 patients with Donovanosis. All the patients responded well to this treatment, and the ulcers healed completely within 10 days in eight patients and within 14 days in the remaining two. Ten days' treatment with 40 tablets of co-trimoxazole is suggested as sufficient to treat Donovanosis. No adverse reactions were noted in any patient.
