OBJECTIVES: To perform a complex review of HELLP syndrome and its treatment from up to date scientific literature. Study of the interresting treatement regime of the patient.
METHODS: Complex analysis of the syndrome, systematic search of medical scientific databases and Slovak Medical Library. Analysis of the life threatening state of the patient, retrospective analysis of the diagnostics, treatment, acute management, complications and clinical results.

Preeclampsia and the variant HELLP syndrome are systemic conditions associated with vascular changes resulting in vasoconstriction. BACKGROUND: HELLP syndrome is an important complications in pregnancy which increases maternal and fetal mortality. This disease usually remits with supportive treatment which includes prescription of corticosteroid, magnesium sulfate, stabilization of mother and pregnancy termination. HELLP (hemolysis, elevated liver enzymes and low platelets) syndrome is an obstetric complication with heterogeneous presentation, multisystem involvement and variable prognosis, but which usually resolves after delivery. We report a case of HELLP syndrome with subcapsular hematoma of the liver causing extrapericardial cardiac tamponade. The subcapsular hematoma and cardiac tamponade were managed conservatively.

BACKGROUND: HELLP syndrome is associated with high morbidity and mortality which is attributed most commonly to intracranial hemorrhage (ICH) and/or stroke. The presence of other complications further worsens the prognosis.

HISTORY AND ADMISSION FINDINGS: A 35-year-old woman (gravida 4, para 3) was known to have mixed connective tissue disease (MCTD-Sharp's syndrome) for five years, treated with immunosuppressives. She was in the 20 + 4 week of an unwanted pregnancy.
DIAGNOSIS: On admission she had marked eclampsia with severe hypertension (180/130 mmHg), laboratory tests revealing hemolysis, elevated liver enzymes and thrombocytopenia, indicating an HELPP syndrome (severe eclampsia with hemolysis, elevated liver function and low platelets).

OBJECTIVE: We reviewed the impact of intravenous high-dose corticosteroid administration for preterm hemolysis, elevated liver enzymes, low platelets (HELLP) syndrome on vaginal delivery rate and degree of clinically significant thrombocytopenia.
METHODS: Retrospective analysis of 1991-2000 HELLP syndrome (platelets < 100,000/uL, lactate dehydrogenase > 600 IU/L, aspartate aminotransferase and/or alanine aminotransferase > 70 IU/L) data focusing on labor inductions for gestations of less than 34 weeks and increase in platelet count sufficient to permit regional anesthetic techniques.

Serum levels of aminotransferases, lactate dehydrogenase, gammaglutamyl transferase, alkaline phosphatase, albumin and conjugated bilirubin, measured in 54 women at a median of 31 months (range 3-101) after pregnancies complicated by the HELLP syndrome, were not elevated. 120 cases of severe preeclampsia without HELLP Syndrome (hemolysis, elevated liver enzymes and low platelets), 120 cases of HELLP Syndrome without eclampsia and 119 eclamptic patients without HELLP were analyzed. We attempted to predict patients at risk for HELLP syndrome (hemolysis, elevated liver enzymes, and low platelet count) by using the D-dimer test to screen 81 preeclamptic gravidas and 12 control subjects. Among preeclamptic patients, 43 had positive and 38 had negative results of D-dimer tests. All controls had negative results.
