Understanding the Impact of Antiretroviral Therapy and Other Factors on HPV Clearance in Cisgender Women With HIV/HPV Coinfections in the DC Cohort.

BACKGROUND: Women with HIV are at an elevated risk for HPV. Antiretroviral therapy (ART) effectively treats HIV; however, there is no noninvasive pharmacologic treatment for HPV. Previous studies have shown varied effects of ART class on HPV clearance, with some demonstrating an association. Our objective was to evaluate the association between ART class and HPV clearance among women with HIV/HPV coinfections. METHODS: We analyzed electronic health records from participants with HPV in the DC Cohort Longitudinal HIV Study from time at HPV detection to January 1, 2024. We investigated time to

The effect of a treatment switch to integrase Strand transfer inhibitor-based regimens on weight gain and other metabolic syndrome-related conditions.

OBJECTIVE: This study aimed to assess weight gain associated with treatment switching to INSTI-based regimens in people living with HIV (PLWH) and to determine whether it is accompanied by worsening features of hypertension, dyslipidemia, or hyperglycemia. METHODS: In this two-center retrospective observational study, we assessed weight gain and metabolic features in PLWH who switched to an INSTI-based regimen (study group) as compared to patients who remained on a non-INSTI regimen (control group) over a 24-month follow-up period. RESULTS: One-hundred seventy-four PLWH were included in the

[Universal modular system for in vitro screening of potential inhibitors of HIV-1 replication].

Here we describe a system based on recombinant lentiviral vectors for the safe screening of potential anti-HIV drugs. The system allows to evaluate the sensitivity of HIVl-1 reverse transcriptase and integrase (wild-type as well as mutant forms of these enzymes detected in drug-resistant virus isolates) towards different drugs and substances, but also to screen inhibitors of other stages of HIV-1 life cycle.

High levels of transmitted drug resistance to INSTIs and NNRTIs in Chile: A nationwide prevalence study.

BACKGROUND: Human Immunodeficiency Virus (HIV) morbi-mortality and transmission have decreased due to antiretroviral therapy (ART). However, treatment failure still occurs when acquiring a strain with resistance mutations. HIV transmitted drug resistance (TDR) is increasing worldwide, and its prevalence in Chile was estimated at 10.4 % in 2018. We aimed to determine the prevalence of TDR and, its associated mutations to evaluate the need to incorporate genotyping studies in ART-naïve people living with HIV in Chile. METHODS: Cross-sectional study conducted in eleven health centers and seven r

Progressive emergence of an S153F plus R263K combination of integrase mutations in the proviral DNA of one individual successfully treated with dolutegravir.

OBJECTIVES: The development of HIV drug resistance against the integrase strand transfer inhibitor dolutegravir is rare. We report here the transient detection, by near full-genome ultradeep sequencing, of minority HIV-1 subtype B variants bearing the S153F and R263K integrase substitutions in the proviral DNA from blood cells of one patient who successfully initiated dolutegravir-based ART, over 24 weeks. Our objective was to study the effects of these substitutions. METHODS: Strand transfer and DNA-binding activities of recombinant integrase proteins were measured in cell-free assays. Cell-

Different Pathways Conferring Integrase Strand-Transfer Inhibitors Resistance.

Integrase Strand Transfer Inhibitors (INSTIs) are currently used as the most effective therapy in the treatment of human immunodeficiency virus (HIV) infections. Raltegravir (RAL) and Elvitegravir (EVG), the first generation of INSTIs used successfully in clinical treatment, are susceptible to the emergence of viral resistance and have a high rate of cross-resistance. To counteract these resistant mutants, second-generation INSTI drugs have been developed: Dolutegravir (DTG), Cabotegravir (CAB), and Bictegravir (BIC). However, HIV is also able to develop resistance mechanisms against the secon

In Vitro and In Silico Potential Inhibitory Effects of New Biflavonoids from Ochna rhizomatosa on HIV-1 Integrase and Plasmodium falciparum.

The aim of this study was to identify bioactive secondary metabolites from Ochna rhizomatosa with potential inhibitory effects against HIV and Plasmodium falciparum. A phytochemical study of O. rhizomatosa root barks resulted in the identification of three new biflavonoids (1-3), along with four known ones (4-7). Compound 7 (Gerontoisoflavone A) was a single flavonoid present in the rootbark of the plant and was used as a reference. Compound 1 (IC50 = 0.047 µM) was the only one with a noteworthy inhibitory effect against HIV-1 integrase in vitro. Chicoric acid (IC50 = 0.006 µM), a pure competi

Sociodemographic, Ecological, and Spatiotemporal Factors Associated with Human Immunodeficiency Virus Drug Resistance in Florida: A Retrospective Analysis.

BACKGROUND: Persons living with human immunodeficiency virus (HIV) with resistance to antiretroviral therapy are vulnerable to adverse HIV-related health outcomes and can contribute to transmission of HIV drug resistance (HIVDR) when nonvirally suppressed. The degree to which HIVDR contributes to disease burden in Florida-the US state with the highest HIV incidence- is unknown. METHODS: We explored sociodemographic, ecological, and spatiotemporal associations of HIVDR. HIV-1 sequences (n = 34 447) collected during 2012-2017 were obtained from the Florida Department of Health. HIVDR was catego

Recent Developments in HIV Antivirals: The Prospect of Prophylactic Drugs to Change the Pandemic.

The human immunodeficiency-1 virus was identified in 1983 as the etiological agent of AIDS. Four years later, the first HIV-1 antiviral drug was approved: the nucleoside analog zidovudine inhibiting the viral reverse transcriptase (RT). Subsequently, non-nucleoside inhibitors of RT, viral protease, viral integrase, and viral entry inhibitors were approved as antiviral drugs. Combining these drugs into a highly active antiretroviral therapy (HAART) decreased the viral load in chronically infected patients and suppressed AIDS defining symptoms. HAART became a medical success story, but the chron

HIV-1 drug resistance and genetic diversity in a cohort of people with HIV-1 in Nigeria.

OBJECTIVE: This study was designed to provide information on the genetic diversity of HIV-1 and drug resistance mutations in Nigeria, as there is limited understanding of variants circulating in the country. METHODS: We used an advanced next-generation sequencing platform, Primer ID, to: investigate the presence of high and low abundance drug resistance mutations; characterize preexisting Integrase Strand Transfer Inhibitor (INSTI) mutations in antiretroviral therapy (ART)-experienced but dolutegravir-naive individuals; detect recent HIV-1 infections and characterize subtype diversity from a

Is the HIV-2 pandemic going extinct 40 years after virus discovery?

A close but genetically divergent retrovirus named HIV-2 was discovered in 1986 in West African patients with AIDS negative or indeterminate for HIV-1 antibodies. Viral replication is lower in HIV-2, being plasma viremia often undetectable or low. As result, HIV-2 transmission either by sex or vertically, is lower compared to HIV-1. Alongside, CD4+ T-cell declines occur slowly and clinical manifestations of immunodeficiency may appear after 15 years in HIV-2 patients. Over 10% of HIV-2 carriers may remain asymptomatic lifelong. The susceptibility of HIV-2 to antiretroviral drugs is generally l

High concordance of human immunodeficiency virus-1 genotypic drug resistance generated from paired cerebrospinal fluid and plasma in antiretroviral therapy -naive or -experienced patients.

BACKGROUND: The development of human immunodeficiency virus (HIV) drug resistance significantly impairs patients' quality of life. However, the HIV-1 drug resistance patterns in the central nervous system (CNS) have been poorly studied. OBJECTIVE: We aimed to compare HIV-1 genotypes and drug resistance mutations (DRMs) derived from the cerebrospinal fluid (CSF) and plasma of antiretroviral therapy (ART)-naive or -experienced patients. METHODS: The matched CSF and plasma samples from 59 patients with HIV were subjected to HIV proteinase (PR), reverse transcriptase (RT), and integrase (IN) gen

Structural Implications of Genotypic Variations in HIV-1 Integrase From Diverse Subtypes.

Human immunodeficiency virus type 1 (HIV-1) integrase (IN) integrates viral DNA into the host genome using its 3'-end processing and strand-transfer activities. Due to the importance of HIV-1 IN, it is targeted by the newest class of approved drugs known as integrase strand transfer inhibitors (INSTIs). INSTIs are efficient in maintaining low viral load; however, as with other approved antivirals, resistance mutations emerge in patients receiving INSTI-containing therapy. As INSTIs are becoming increasingly accessible worldwide, it is important to understand the mechanism(s) of INSTI susceptib

Raltegravir versus efavirenz in antiretroviral-naive pregnant women living with HIV (NICHD P1081): an open-label, randomised, controlled, phase 4 trial.

BACKGROUND: Although antiretroviral regimens containing integrase inhibitors rapidly suppress HIV viral load in non-pregnant adults, few published data from randomised controlled trials have compared the safety and efficacy of any integrase inhibitor to efavirenz when initiated during pregnancy. We compared safety and efficacy of antiretroviral therapy with either raltegravir or efavirenz in late pregnancy. METHODS: An open-label, randomised controlled trial was done at 19 hospitals and clinics in Argentina, Brazil, South Africa, Tanzania, Thailand, and the USA. Antiretroviral-naive pregnant

Mechanistic and pharmacological analyses of HIV-1 integration.

Significant advances have transpired in the human immunodeficiency virus type 1 (HIV-1) integration field in recent years. Considering its essential nature, integrase has long been a target of interest for antiviral drug development. The most significant advance was the approval of the Merck compound raltegravir, the first licensed integrase inhibitor, in October 2007. Another milestone was the identification and characterization of specific nucleoprotein complexes that mediate integrase 3' processing and DNA strand transfer activities in vitro. Genome-wide distribution analyses have furthermo

First characterization of insertions in the HIV-1 integrase-coding region from people with HIV.

OBJECTIVES: This study aimed to gain a better understanding of the role of insertional mutations in the integrase (IN)-coding sequence of 13 HIV-1-infected people. RESULTS: Here, we present the first documentation of amino acid insertion in the IN-coding sequence of HIV-1-infected people at positions 168, 253, 255 and 260. The consequences of these mutations in terms of viral replication and resistance to INSTIs were analysed using virological and biochemical assays and 3D modelling. Analysis of viral genomes by quantitative PCR demonstrated that insertional mutations reduce reverse transcrip
