[The role of a shortening osteotomy of the first phalanges associated with soft-tissue release in the surgical treatment of hallux valgus].

The Authors report the results of 120 hallux valgus operated on 76 patients from January 1983 to June 1988. The operative procedure associated bunionectomy, tenotomy of the abductor hallucis which was resected from the base of the first phalanx and from the lateral sesamoid, shortening osteotomy of the proximal phalanx and finally adjustment of the sesamoids under the head of the first metatarsal. When there was an important metatarsus varus, a basilar osteotomy of the first metatarsal was associated (10 cases about 120). The results were appreciated functionally (pain, range of motion, easine

[Hallux valgus treated by Scarf osteotomy of the first metatarsus and the first phalanx associated with an adductor plasty. Apropos of 50 cases with a 2-year follow up].

PURPOSE OF THE STUDY: Fifty hallux-valgus were treated with Scarf Osteotomy of the first metatarsal, associated to a phalangeal varisation or shortening osteotomy and an adductor plasty. Patients were evaluated with a minimum follow-up of two years. MATERIAL AND METHODS: Forty five females and two males were operated with an average age of fifty years. The pre operative metatarsus varus was of 15 degrees 8. Mean alignment of metatarsal bar was 31 degrees 4. The cuneo-metatarsal joint was twenty two times spheric and twenty eight times plane. The average metatarso-phalangeal great toe valgus w

Operative correction of arch collapse with forefoot deformity: a retrospective analysis of outcomes.

BACKGROUND: There are many forefoot deformities, including hallux valgus, forefoot overload, and hammertoe that are treated as unrelated problems with multiple different techniques. Currently, there has been renewed interest in the role of a gastrocnemius contracture on foot deformities. Our objective was to review a specific surgical treatment plan for forefoot deformities classified by us as Type 2 arch collapse and evaluate the outcomes. MATERIALS AND METHODS: We retrospectively reviewed the charts of 374 patients who underwent foot procedures to treat deformity classified as a Type 2 arch

Lateralising step-DMMO (MIS) - a midshaft modification of the lateralising DMMO for a powerful correction of a pes adductus during surgical treatment of a hallux valgus.

Surgical treatment of hallux valgus can be very difficult when it occurs together with pes adductus. The medially displaced metatarsal bones prevent a sufficient shifting of the first metatarsal during hallux valgus correction. This can lead to inherently high reoccurrence rates. Open surgical corrections lead to a profound soft tissue trauma and prolonged swelling, while requiring relief from weight-bearing in the affected foot. In this paper, we aim to introduce a useful midshaft modification of the lateralising Distal Metatarsal Minimal-invasive Osteotomy (lat. DMMO) to perform an effective

Optimal posterior screw placement configuration in Sanders 2B calcaneal fractures: A biomechanical study.

BACKGROUND: Calcaneal fractures can be high energy intra-articular injuries associated with joint depression. Challenges to fracture reduction include lateral wall blow out, medial wall overlap, comminution and central bone loss. Secondary deformity such as hindfoot varus alters foot biomechanics. Minimally invasive approaches with indirect reduction of the calcaneal tuberosity to maintain the reduction using posterior screws is routinely being used in the treatment of joint depression fractures. Biomechanically, optimum screw numbers and configuration is not known. Biomechanical studies have

[Hallux valgus--an atavism?].

In biology, atavism is generally understood as a biological phenomenon which brings to sight a recurrent phenotypic character which was lost for extinct generations. Phylogenic evolution of homo sapiens demonstrates today that the first plantar ray in tetrapods has always been a very stable ray and the lateral foot experienced a progressive adduction to the talo-metatarsal axis, together with a remodeling of the lateral column of the foot and a progressive valgus of the calcaneus beneath the talus. The foot appears as a strongly modified extremity, starting at a prehensile foot organ, common t

Hypermobility of the first ray.

Hypermobility of the first ray is one of the causative components in common foot problems (such as hallux valgus) with a large intermetatarsal angle and metatarsus primus varus. Although not always associated with hallux valgus, hypermobility is a predisposing factor for this deformity, especially in conjunction with extrinsic factors, such as disruption of the plantar first metatarsal cuneiform ligament and tendon-muscle imbalance. Hypermobility is also frequently found in adolescents with hallux valgus, especially when associated with a large intermetatarsal angle. Motion at the first metata

Distal chevron osteotomy with lateral release for treatment of hallux valgus deformity.

From 1986 to 1990, 42 feet in 30 patients had a combined chevron osteotomy and lateral release to correct their painful bunion deformity. Seventeen patients, 23 feet, were available for follow-up. The purpose of this study was to evaluate the outcome of this procedure and to determine the incidence of avascular necrosis. The patients were examined by an independent examiner, radiographs obtained, and questionnaires filled out. Avascular necrosis was determined using plain films read by a musculoskeletal radiologist who did not know the outcomes. The average age was 45 years at the time of the

Asymmetry of the leg alignment affects trunk bending in the coronal plane after unilateral total knee arthroplasty.

Unilateral total knee arthroplasty (TKA) would produce asymmetric changes of lower extremity in patients with bilateral varus deformity. Our purpose was to investigate whether asymmetry of the leg alignment would affect trunk bending in the coronal plane after unilateral TKA. Twenty patients (mean 76 years old) with bilateral end-stage knee osteoarthritis (OA) participated. Spine images during relaxed standing were obtained on pre- and postoperative day 21. As a result, the shoulder tilted more to the TKA side and the pelvis inclined more to the contralateral OA side. These results suggested t

[Treatments associated with scarf osteotomy for hallux valgus correction].

In view of the multiplicity of the proposed techniques for hallux valgus correction, we recommend to evaluate scarf osteotomy associated or not with phalangeal osteotomy and/or Weil osteotomy. We conducted a retrospective study of 29 patients, including a bilateral case, undergoing scarf osteotomy of the first ray with associated treatments in 80% of cases between 2011 and 2016. Results were analyzed on the basis of patients' satisfaction, Groulier index and radiological measurements. The overall Groulier score showed good objective assessment of the end results based on radiological and anato

Pathologic Disorders of the Foot in Professional Female Flamenco Dancers.

BACKGROUND: The foot is the main element of artistic creation in flamenco dancing. At the professional level, the foot undergoes high musculoskeletal demands, predisposing the development of podiatric pathologic disorders in this group. The principal objective of this study was to determine the most common foot lesions in professional female flamenco dancers. METHODS: In a cross-sectional observational study of 44 female professional flamenco dancers, the participants completed a short questionnaire about their demographic features, number of hours danced per week, and years of professional a

Multifactorial Non-union After First Metatarsophalangeal Arthrodesis Revised With Structural Iliac Crest Autograft: A Case Report and Comprehensive Literature Review.

INTRODUCTION: First metatarsophalangeal (MTP-I) arthrodesis is a reliable procedure for end-stage hallux rigidus and selected complex first-ray deformities. Non-union is uncommon but clinically important, particularly when associated with pain, osteolysis, implant fatigue, and loss of correction. Revision is challenging because failure is usually multifactorial rather than the consequence of an isolated implant problem. CASE REPORT: A 62-year-old woman with a 45 pack-year smoking history underwent MTP-I arthrodesis for advanced hallux rigidus with hallux varus and associated lesser-toe deform

Biomechanical Cadaveric Evaluation of the Role of Medial Column Instability in Hallux Valgus Deformity.

BACKGROUND: Medial column instability is a frequent finding in patients with flatfeet and hallux valgus, within others. The etiology of hallux valgus is multifactorial, and medial ray axial rotation has been mentioned as having an individual role. Our objective was to design a novel cadaveric foot model where we could re-create through progressive medial column ligament damage some components of a hallux valgus deformity. METHODS: Ten fresh-frozen lower leg specimens were used, and fluorescent markers were attached in a multisegment foot model. Constant axial load and cyclic tibial rotation (

[Lesser toe deformities. Definition, pathogenesis, and options for surgical correction].

Whereas in the past resection arthroplasty was - in analogy to hallux valgus surgery - the preferred therapy to correct lesser toe deformities, the point of view has undergone a change. Much interest is directed toward functional aspects that require reconstructive management. Whenever possible the integrity of joint play should be saved. Above all the metatarsophalangeal joint of the lesser toes is worth being preserved to prevent a severe disturbance of the biomechanics of the foot. Tendon transfers and subtle corrective osteotomies such as the Weil procedure allow restricting resection proc

High tibial osteotomy for osteoarthritis of the knee with varus deformity utilizing the hemicallotasis method.

INTRODUCTION: A hemicallotasis method has been developed utilizing an external fixator as high tibial osteotomy (HTO), and satisfactory results of this method with the external fixator have been reported. This external fixator has a universal joint that moves in all directions. We have recently designed a hemicallotasis device for this operation. METHODS: HTO for the knee with varus deformity utilizing the hemicallotasis method was performed on 44 knees. The patients had a mean age at operation of 65 years (range 49-82 years), a mean follow-up period of 68 months (range 36-119 months), and a
