INTRODUCTION: Opioid use disorder is a chronic relapsing condition with major global public health impacts. Despite rising opioid use and overdose rates, data availability remains uneven, limiting understanding of treatment and harm reduction services worldwide. This study aims to provide a global overview of current practices in treatment and harm reduction for opioid use disorder.
METHODS: A global survey was conducted through the International Society of Addiction Medicine's Global Expert Network (ISAM-GEN) with participation from 42 addiction medicine societies/organizations representing …

BACKGROUND: Homelessness results from, and exacerbates, various social determinants of health, including poverty, racism, and inadequate healthcare access, which are further compounded by physical and mental health challenges. The street medicine movement seeks to address these disparities by providing direct medical care and harm reduction services to unsheltered homeless in low-barrier settings. The development of trust is critical to providing this form of care. Few studies have sought to understand the factors influencing trust development in street medicine encounters.
METHODS: This case…

BACKGROUND: Community-based harm reduction for opioid use may be crucial for mitigating maternal and fetal harms by reducing information bias, fostering trust, and connecting individuals to essential treatment and services. This scoping review examines community-based harm reduction strategies and policies addressing maternal opioid use occurring in nonclinical settings, including stigma reduction, public health education, service access, and integration. The objectives of this scoping review are to 1) delineate community-based harm reduction approaches; 2) contextualize findings; 3) identify …

BACKGROUND: Harm reduction vending machines (HRVMs) that dispense safe injection equipment and other supplies have operated globally for more than 30 years, yet few operate in the U.S., particularly in the rural epicenters of drug-related harms. This study explores likelihood of using HRVMs and correlates thereto among people who inject drugs in rural Appalachian Kentucky.
METHODS: Respondent-driven sampling and outreach were used to recruit participants who were age 18 or older, resided in an Appalachian Kentucky county, and had used opioids and/or injected drugs to get high in the past 30 d…

A central challenge in the opioid crisis in the US has been the underuse of lifesaving treatment and harm reduction programs. Conventional explanations focus on limited access to care when services are not convenient, affordable, or timely. However, these factors exist downstream from other fundamental causes of low utilization. In this article, we focus on the upstream challenges of scaling effective programs to reach more people who would benefit. The opioid crisis spurred numerous inventive approaches such as buprenorphine administered by paramedics and vehicles equipped to dispense methado…

BACKGROUND: People who inject drugs (PWID) face barriers to treatment due to drug use stigma. Reduced treatment access might be exacerbated if PWID's other social identities (e.g., race), are part of another historically marginalized group regarding healthcare access. Research suggests harm reduction strategies are efficacious for PWID, yet they remain under-supported at the policy level in the U.S. The current study examines associations between racial attitudes, motivation to act nonprejudiced, attitudes toward PWID and harm reduction policies, and whether these vary by race of PWID.

The opioid epidemic has exposed a gulf in mental health research, treatment, and policy: Most patients with comorbid trauma-related disorder (TRD) and opioid use disorder (OUD) (TRD + OUD) remain undiagnosed or unsuccessfully treated for the combination of TRD symptoms and opioid use. TRD treatments tend to be psychotherapies that are not accessible or practical for many individuals with TRD + OUD, due to TRD treatment models not systematically incorporating principles of harm reduction (HR). HR practices prioritize flexibility and unequivocally improve outcomes and save lives in the treatment…

BACKGROUND: Xylazine, an adulterant in local drug supplies, has been detected in approximately 30% of opioid samples submitted for testing i … mong PWUD, and they consider xylazine an unwanted adulterant. We also found that PWUD who use other harm reduction measures are more willing to use xylazine test strips. The increase in xylazine warrants additional community-level interventions such as wound management and local testing infrastructure. Further research is needed to understand better the impacts …

BACKGROUND: Xylazine is in 99% of the fentanyl supply in Philadelphia, PA and is on the rise throughout the United States. Perspectives about this emerging crisis among people who use opioids (PWUO) and harm reduction workers are lacking.
METHODS: We conducted focus groups with PWUO and harm reduction workers in Philadelphia, PA. Discussions focused on xylazine's impact on PWUO. Transcripts were coded for thematic analysis.
RESULTS: Themes across focus groups included xylazine withdrawal, wounds, safety concerns, and overdose response implications. Participants emphasized extreme xylazine wi…

BACKGROUND: Anabolic androgenic steroids (AAS) are synthetic forms of testosterone frequently used as performance enhancing drugs among gay, bisexual, and queer (GBQ) men. Despite widespread use, associated harms, and the likely existence of an AAS use disorder, there is no medical consensus on standards of care for people who use AAS, with most medical providers focusing exclusively on abstinence. Individuals using AAS have developed community-based harm reduction strategies to mitigate these harms.
METHODS: This paper is a sub-analysis of qualitative data obtained through semi-structured in…

Juvenile idiopathic arthritis (JIA) is a chronic autoimmune condition in children that often requires long-term pain management, which can include opioid use. In the context of Canada's ongoing overdose crisis, youth with JIA face risks due to potential opioid dependency and exposure to toxic drug supplies. This commentary proposes an integrated approach combining chronic disease management with harm reduction strategies specifically tailored for JIA patients. By incorporating multidisciplinary care, opioid stewardship, and harm reduction measures, this approach aims to address the dual challe…

BACKGROUND: Rural areas in the United States (US) are ravaged by the opioid overdose epidemic. Oconee County, an entirely rural county in northwest South Carolina, is likewise severely affected. Lack of harm reduction and recovery resources (e.g., social capital) that could mitigate the worst outcomes may be exacerbating the problem. We aimed to identify demographic and other factors associated with support for harm reduction and recovery services in the community.
METHODS: The Oconee County Opioid Response Taskforce conducted a 46-item survey targeting a general population between May and Ju…

Background: Harm reduction includes treatment and prevention approaches rather than abstinence, as a public health strategy for mitigating the opioid epidemic. Harm reduction is a new strategy for many healthcare professionals, and gaps in knowledge and practices may lead to barriers to optimal treatment. Our objective was to identify and describe gaps in physicians' knowledge, education, and practice in harm reduction strategies related to opioid overdose. Methods: We searched the PubMed, CINAHL, and Web of Science databases for articles published between 2015 and 2021, published in English, …

BACKGROUND: Introduction of new tobacco products in the United States, including those that may be lower on the risk continuum than traditional combustible cigarettes, requires premarket authorization by the US Food and Drug Administration and information on the potential impact of the products on consumer behaviors. Efficient recruitment and data capture processes are needed to collect relevant information in a near-to-real-world environment.
OBJECTIVE: The aim of this pilot study was to develop and test a protocol for an actual use study of a new tobacco product. The product included in thi

Two recent randomized controlled efficacy trials showed that harm-reduction treatment for alcohol use disorder (AUD)-or patient-driven treatment that does not require abstinence and instead supports decreased alcohol-related harm and improved quality of life (QoL)-is efficacious for adults experiencing homelessness and AUD. The present study provides qualitative and quantitative analysis of one component of harm-reduction treatment, participants' harm-reduction goal-setting, within these two trials. Aims of this secondary, dual-trial study (Trial 1 N = 208, Trial 2 N = 86) were to describe par
