Early Disseminated Lyme Disease with Carditis Complicated by Posttreatment Lyme Disease Syndrome.

Lyme disease is an infectious disease caused by the bacterium Borrelia burgdorferi. When untreated, infection may spread to the heart, nervous system, and joints. Cardiac involvement usually manifests as abnormalities of the conduction system and bradycardia. Treatment of Lyme disease is generally effective, with a subset of patients experiencing persistent, sometimes long-term symptoms called posttreatment Lyme disease syndrome.

Bradycardia: sinus and AV node dysfunction.

The surface electrocardiogram (ECG) holds many clues with regard to the etiology of bradycardia and site of atrioventricular (AV) block. Bedside maneuvers may prove helpful in cases of 2:1 AV block or situations where the data is not all concordant. Wenckebach conduction may occur in any region of the heart, and there are nonpathologic mimickers of Mobitz II AV block as well. The surface ECG may aid in the inference of etiology for better than expected or slowed rather than blocked AV conduction. Sinus node dysfunction may present in several forms and often accompanies other conduction system

Progressive High-Grade Atrioventricular Block in a Young Adult: Distinguishing Reversible Causes From Lev-Lenègre Disease.

High-grade atrioventricular (AV) block in young adults is uncommon and poses a diagnostic challenge when distinguishing reversible causes from intrinsic conduction system disease. We present a 27-year-old previously healthy male who presented with dizziness and was found to have a complete heart block with a wide QRS escape rhythm requiring temporary pacing. Initial evaluation revealed metabolic abnormalities and positive Coxsackie serology; however, correction of these factors did not result in improvement of conduction. Echocardiography demonstrated a normal cardiac structure. Given the poss

Conduction System Hamartoma: Autopsy Case Series.

Conduction system hamartoma is a benign hamartomatous lesion arising from Purkinje and Purkinje-like cells of the heart. We aimed to investigate the cases that we detected during postmortem histopathological examination. The histopathology reports of the cases autopsied between 2012 and 2022 were reviewed retrospectively. The cases were evaluated in terms of histopathological features, demographic data, autopsy findings, causes of death, microbiological results, and accompanying cardiac anomalies. There was a total of 4 cases. The female-to-male ratio was 3/1. The mean age of the cases was 5.8

Cardiac resynchronization considerations in left bundle branch block.

Cardiac resynchronization therapy (CRT) via biventricular pacing (BiVP) is an established treatment for patients with left ventricular systolic heart failure and intraventricular conduction delay resulting in wide QRS. Seminal trials demonstrating mortality benefit from CRT were conducted in patients with wide left bundle branch block (LBBB) pattern on electrocardiogram (ECG) and evidence of clinical heart failure. The presence of conduction block was assumed to correlate with commonly applied criteria for LBBB. More recent data has challenged this assertion, revealing that LBBB pattern may in

Case report: Challenges and implications of conduction system pacing in pediatrics: Case series.

Cardiac electrical stimulation in children usually is needed in the setting of complete congenital atrioventricular block, atrioventricular block after heart surgery, and bradycardia associated with some specific channelopathies. In cases of atrioventricular block, the high percentage of ventricular stimulation raises concern on the deleterious effects of chronic stimulation of the right ventricle. In recent years, physiologic stimulation has developed as a valid approach for adult patients and a great interest has risen in offering conduction system pacing also to the pediatric population. We

A case of self-induced digoxin poisoning: with His bundle studies of the site of heart block.

A case of digoxin overdose was described. Despite vigorous gastric lavage, the serum digoxin which was 0.8 ng/ml on admission rose to 7 ng/ml four hours later. The main side-effects were symptoms of nausea and anorexia and conduction defects including complete atrioventricular dissociation requiring pacemaker insertion. Opportunity was taken to study the site of heart block with His bundle electrography during the conduction disturbances. This showed that the site of block was situated above the His-Purkinje system in the atrioventricular (A-V) node.

Observations on the antidromic type of circus movement tachycardia in the Wolff-Parkinson-White syndrome.

In the differential diagnosis of tachycardias showing a wide QRS complex and having a 1 to 1 relation between ventricular and atrial events, a supraventricular tachycardia with anterograde conduction over an accessory pathway and retrograde conduction by way of the specific conduction system must be considered. Five patients showing this type of circus movement tachycardia were studied by programmed electrical stimulation of the heart. Sudden changes in the tachycardia cycle length were observed in these patients that were based on changes in the VH interval. This finding suggested a change in

Development of the cardiac conduction system in atrioventricular septal defect in human trisomy 21.

In patients with atrioventricular septal defect (AVSD), the occurrence of nonsurgical AV block has been reported. We have looked for an explanation in the development of the AV conduction system. Human embryos with AVSD and trisomy 21 and normal embryos were examined (age 5-16 wk gestation). Antibodies to human natural killer cell-1 (HNK-1), muscle actin (HHF-35), and collagen VI were used to delineate the conduction system. As in normal hearts, HNK-1 transiently stains the AV conduction system, the sinoatrial node, and parts of the sinus venosus in AVSD. A large distance is present between th

[Transient total atrioventricular dissociation induced by junctional premature depolarization].

The authors describe a case of temporary complete atrioventricular dissociation in prognostically less serious conduction disorders in the area of the atrioventricular node manifested clinically as an atrioventricular block grade 1. Dissociation episodes were induced by premature junctional depolarization and maintained, thanks to retrograde latent conduction into the area of the atrioventricular junction and the small difference of frequencies of the sinus and ectopic substitution rhythm.

[Electric stimulation of the heart in ventricular tachycardia: achievements and prospects].

Cardiac electrostimulation was conducted in 19 patients to prevent and control paroxysmal ventricular tachycardia. In 8 of those, an attempt was made to prevent tachycardia by continuous endocardial ventricular stimulation in the demand regimen. In 11 patients, a radio-frequency stimulator was implanted, its electrode positioned in the right ventricle to control current tachycardiac attacks. A 3-5-second stimulation at frequencies of 180-220 impulses per minute was sufficient to control the attack in 8 of the patients, while programmed ventricular stimulation only was needed to produce the eff

His bundle recordings in atrioventricular nodal alternating Wenchebach periods ending in 5:1 atrioventricular block coexisting with paroxysmal atrioventricular nodal block.

Atrioventricular nodal alternating Wenckebach periods ending in 5:1 atrioventricular block occurring during rapid atrial rhythms were explained by postulating the presence of block in three levels of the atrioventricular node. This pattern of conduction occurred in ten of 11 patients who either had received ouabain or verapamil (nine patients) or who had organic atrioventricular nodal disease (two patients). In contrast, this pattern of conduction occurred in only one of eight nonmedicated patients without organic atrioventricular nodal disease. The frequent association of this pattern with pa

Intraventricular dissociation due to complete intraventricular block.

Intraventricular dissociation due to intraventricular block occurred in a patient with cardiac arrest due to massive cerebral hemorrhage. The electrocardiogram showed complete atrioventricular block with three different types of accelerated ventricular rhythm. The ectopic beats did not interfere with each other because of the existence of an area of complete block surrounding the areas in which impulse formation occurred. Hence, the corresponding QRS complexes did not result from depolarization of all the ventricular muscle mass but only of certain regions. The latter were large enough to prod

Symptomatic improvement in a patient with sick sinus syndrome after the onset of stable atrial flutter.

A patient with sick sinus syndrome (SSS) presented with episodic lightheadedness and weakness. The electrocardiographic features were marked supraventricular bradyarrhythmias and paroxysmal atrial flutter. The symptoms lasted for four years and disappeared with the onset of stable atrial flutter which has persisted for the past seven years. Over the 11-year period of observation, there has been progressive involvement of the His-Purkinje system manifested by the development of left anterior fascicular block, right bundle-branch block, and prolongation of the HV conduction time. The patient has

Case Report Detailing an Inferior Myocardial Infarction, Third-Degree Heart Block, and Cardiogenic Shock.

An infarction in the right coronary artery affects the inferior wall of the heart and can also cause impedance to the cardiac conduction system. The right coronary artery perfuses the sinoatrial and atrioventricular nodes, and a loss of blood flow contributes to a breakdown in the communication system within the heart, causing associated bradycardias, heart blocks, and arrhythmias. This case report details the prehospital and emergency care of a middle-aged man who experienced an inferior myocardial infarction, concomitant third-degree heart block, and subsequent cardiogenic shock, with succes

Severe pulmonary valve stenosis associated with advanced atrioventricular conduction disease: A rare case report.

Congenital pulmonary valve stenosis is a common cause of right ventricular (RV) outflow tract obstruction. Advanced atrioventricular (AV) block in congenital heart disease is usually associated with abnormalities of AV septation or conduction system malposition. An association between isolated pulmonary valve stenosis and advanced AV block has not previously been reported. A 16-year-old male presented with exertional dyspnea and recurrent presyncope. Electrocardiography (ECG) revealed 2:1 AV block. Transthoracic echocardiography demonstrated severe valvular pulmonary stenosis with a peak gradi

Pleural and pericardial effusions with cardiac conduction system and myocardial involvement: A rare presentation of sarcoidosis.

Sarcoidosis is a granulomatous immune disorder that can infiltrate many organ systems. When the cardiac system is involved, the myocardium and conduction system are frequently affected. We report the case of a patient presenting with complete heart block following cardioversion from atrial flutter accompanied by pleural and pericardial involvement whose diagnosis of sarcoidosis was subsequently made on pathological examination. Pericardial effusion and pleural effusion are rare manifestations of sarcoid, and the both of them happening simultaneously (less than 10 case reports) in conjunction w
