Forty heart transplantations were performed at the Toronto Western Hospital, University of Toronto, from October 1987 to December 1989. Each heart extracted from a recipient was examined with the view of using the aortic valve as a homograft for another patient requiring aortic valve replacement. Of the 40 explanted hearts, 26 had normal aortic valves that were potentially suitable for homografting, and 14 had aortic valves judged as unsuitable. Of the potentially suitable valves, four were preserved for ex vivo arrhythmia studies requiring aortic root perfusion and four were damaged during ha

Transcatheter heart valve interventions have transformed the outcomes of patients with valvular heart disease (VHD) who are unfavourable candidates for surgery. Technological advances have allowed extension of these interventions to younger or lower risk patients and those with other forms of VHD and may in the future permit earlier treatment of VHD in less symptomatic patients or those with moderate disease. The balance of risks and benefits is likely to differ between lower and higher risk patients, and more evidence is needed to evaluate the net benefit of transcatheter technology in these

OBJECTIVES: This study sought to evaluate the feasibility of minimally invasive transapical repeat valve-in-a-valve (VinV) implantation.
BACKGROUND: Reoperative heart valve replacement for degenerated xenografts is associated with an increased surgical risk.
METHODS: Conventional Carpentier Edwards porcine aortic (n = 5) and mitral (n = 2) valve prostheses were implanted in 7 pigs. Transapical VinV implantation of a pericardial xenograft fixed within a 23-mm stainless steel, balloon expandable stent (Cribier Edwards, Edwards Lifesciences, Irvine, California) was then performed under fluorosc

After 5 years the European Society of Cardiology and the European Association for Cardio-Thoracic Surgery have released an update on the guidelines for the management of valvular heart diseases. In recent years published results of randomized trials in patients with aortic valve stenosis have resulted in updated recommendations for catheter-based prosthesis implantation (TAVI), which is now extended to patients presenting without a low risk for conventional surgical valve replacement. In mitral or tricuspid valvular disease, the recommendations for catheter-based therapies are less strong beca

Transcatheter mitral valve replacement is increasingly being used as a treatment for high-risk patients who have native mitral valve disease; however, no comprehensive studies on its effectiveness have been reported. We therefore searched the literature for reports on patients with native mitral valve disease who underwent transcatheter access treatment. We found 40 reports, published from September 2013 through April 2017, that described the cases of 66 patients (mean age, 71 ± 12 yr; 30 women; 30 patients with mitral stenosis, 34 with mitral regurgitation, and 2 mixed) who underwent transcat

We describe the case of a 38-year-old man with a history of metastatic testicular cancer who had undergone multiple thoracic surgical procedures, including tricuspid valve replacement with a bioprosthetic valve as a result of tricuspid involvement of his malignancy. He presented at our outpatient cardiology clinic with worsening fatigue, shortness of breath, and peripheral edema, investigation of which revealed severe tricuspid bioprosthesis stenosis with central regurgitation. Because of the patient's medical history, he was considered to be a high-risk surgical candidate. Therefore, transcat

The timing of surgical intervention and prosthesis selection in mildly symptomatic aortic and mitral valve disease remain an area of clinical uncertainty. Symptom-based referral often occurs after the onset of adverse ventricular remodeling, whereas earlier surgery may improve long-term outcomes but introduces procedural risk. This article synthesized contemporary evidence to delineate optimal strategies in this intermediate-risk population. In aortic stenosis, randomized trials including RECOVERY and AVATAR demonstrate that early surgical aortic valve replacement reduces heart failure events

To study the long-term results of the St. Jude medical prosthesis in the mitral position, 520 patients were evaluated who had undergone mitral valve replacement with St. Jude medical (SJM) prosthesis. Eighteen patients or 3.5% died in the early postoperative period (within 30 days after operation). The total follow-up was 2872 patient-years and late deaths occurred in 23 cases. Postoperative cerebral embolism occurred in six, bleeding complication in five, and thrombosed valve in two patients. Postoperative anti-coagulant therapy was initiated with both dipyridamole (300 mg/day) and warfarin p

The improved pediatric-cardiological diagnostics and cardio-surgical therapy result in a numerical increase of women reaching reproductive age. Pregnancy considerably strains heart and circulation, which is countered by various cardiorespiratory mechanisms. Today mothers are not endangered vitally (exception: Eisenmenger-syndrome), but congestive heart failure, thromboembolic complications and rhythm disturbances may occur. Spontaneous abortion rate is 20-25%, prematurity and underweight are obligatory in cyanotic mothers. On the basis of an extensive review of literature the specific risks du

INTRODUCTION: The increase in the prevalence of aortic stenosis due to an aging population has led to an increasing number of surgical aortic valve replacements. Over the past 20 years, there has been a major shift in preference from mechanical to bioprosthetic valves. However, despite efforts, there is still no "ideal" bioprosthesis. It is crucial to understand the structure, biology, and function of native heart valves to design more intelligent, strong, durable, and physiological heart valve tissues.
METHODS: A comprehensive review of the literature was performed to identify articles repor

AIMS: A comprehensive description of transcatheter heart valve (THV) failure has not been performed. We undertook a systematic review to investigate the aetiology, diagnosis, management, and outcomes of THV failure.
METHODS AND RESULTS: The systematic review was performed in accordance with the PRISMA guidelines using EMBASE, MEDLINE, and Scopus. Between December 2002 and March 2014, 70 publications reported 87 individual cases of transcatheter aortic valve implantation (TAVI) failure. Similar to surgical bioprosthetic heart valve failure, we observed cases of prosthetic valve endocarditis (P

The Starr-Edwards ball and cage valves were among the first and most commonly used mechanical valve devices. These valves offered a novel design that would become one of the mainstays for replacement of severely diseased heart valves in the early second half of the twentieth century. We describe the case of a patient with a Starr-Edwards ball and cage valve in the aortic position that was replaced 40 years earlier who was admitted with concerns for symptoms of new volume overload. Transthoracic and transesophageal echocardiography demonstrated a functional mechanical aortic valve with no evide

A 49-year-old woman with a congenital quadricuspid aortic valve presented in cardiogenic shock due to severe aortic regurgitation. She underwent transfemoral transcatheter aortic valve replacement with a 29 mm self-expanding prosthesis. The procedure was technically successful with minimal paravalvular leak and significant symptomatic improvement. However, her long-term course was complicated by medication nonadherence, valve thrombosis, thromboembolic coronary events, and progressive heart failure. This case highlights the anatomical and technical challenges of transcatheter valve therapy in

We set to examine the mid-term outcome after aortic valve replacement (AVR) with Soprano pericardial stented bioprosthesis measuring <or=20 mm. Sixty-eight patients underwent AVR between June 2003 and January 2006 (50 women; median age 77 years; range 60-89 years). Preoperatively, 60 patients (88.2%) were in New York Heart Association (NYHA) class III/IV. The mean EuroSCORE was 6.7+/-2.3. Supra-annular aortoplasty was performed in 21 patients (30.9%), out of which 11 patients received an 18 mm prosthesis (55%). The median follow-up was 45.5 months (0.1-62 months). The 30-day mortality was 4.4%

BACKGROUND: Severe aortic stenosis (AS) with concomitant mitral annular calcification (MAC) presents substantial procedural and surgical challenges, and simultaneous transcatheter treatment remains rare. This case adds to the emerging literature by demonstrating a hybrid approach using transfemoral transcatheter aortic valve replacement (TAVR) and transapical transcatheter mitral valve replacement (TMVR)-in-MAC supported by patient-specific 3D-printed simulation and prosthesis modification.
CASE SUMMARY: A 68-year-old woman with severe obesity, diabetes, chronic kidney disease, prior thoracic

Management of aortic valve disease follows traditional algorithms: initial gross diagnosis, noninvasive imaging and quantitation, possible catheterization or angiography, and medical treatment until symptoms of prognosis prediction indicates surgery. Most advances in this subject reflect improved technology and additional data collection therefrom, and subsequent analysis. Noninvasive imaging, especially Doppler echocardiography and nuclear magnetic resonance imaging, enjoy continued refinement. Diastolic dysfunction, a particular pathophysiologic feature of aortic valve disease, has become mo

BACKGROUND: Valvular heart disease affects more than 100 million people worldwide and is associated with significant morbidity and mortality. The prevalence of at least moderate valvular heart disease is 2.5% across all age groups, but its prevalence increases with age. Mitral regurgitation and aortic stenosis are the most frequent types of valvular heart disease in the community and hospital context, respectively. Surgical valve replacement (or mitral valve repair) is the standard of care for treating heart valve disease. However, the replacement of a prosthetic heart valve can lead to compli
