Reconstruction of the weight-bearing surface of the foot is a challenging problem for the reconstructive surgeon. Because local tissues are not usually available for reconstruction, distant tissue transfers are often necessary. In an attempt to prevent migration of the heel pad, 11 patients underwent a combined Syme's amputation and Achilles tendon tenodesis between December 1989 and April 1992. Ten patients healed the Syme's amputation, and one patient failed to heal the surgical wounds and required a below-knee amputation. The instep neurovascular flap, based on the medial plantar artery and innervated by the medial plantar nerve, is in our opinion the treatment of choice for resurfacing heel skin defects. The flap is usually raised as an island flap in the non weight-bearing surface of the foot. It provides heel cover with thick skin combined with a perfectly adapted texture. Conservation of sensibility offers a protective advantage. Four patients, treated with the flap, are discussed.
