[Significance of different kinds of correction of the hemothorax volume in surgical treatment of patients with lung and pleura tuberculosis].

In the article, with 72 patients, taken as an example, with tuberculosis of respiratory organs the authors present their clinical experience with using various methods of surgical correction of the hemithorax volume (by means of extrapleural, intrapleural extra-intrapleural correcting thoracoplasties and diaphragm plasty) during performing resections on the lungs and/or pleura. Substantiation is given to the necessity of correcting hemithorax volume in patients with tuberculosis of respiratory organs in performing resections of great volume.

Changing dogmas: history of development in treatment modalities of traumatic pneumothorax, hemothorax, and posttraumatic empyema thoracis.

Development of treatment modalities for chest wounds and traumatic empyema thoracis is reviewed in the light of war experience. Mortality from thoracic injury was more than 50% before World War I and was about 25% during World War I. It came down to 10% in World War II and was about 5% during the Korean War. It improved further during the Vietnam War, until it ranged at 2% to 4%, where no further improvement could be imagined. Thoracic surgery was born in the field hospitals of World War I. Established drainage methods and standardized anesthesia made thoracotomy a standard procedure in World

[Successful surgical treatment of a patient with tension pneumopericardium and left hemothorax caused by multiple rib fractures].

A 62-year-old man underwent operation for tension pneumopericardium and left hemothorax caused by multiple rib fractures. He sustained other multiple injuries, such as acute subdural hematoma, compound fractures of the left leg. At left thoracotomy, massive hemorrhage from the left apical lung disrupted by the first and second fractured ribs was encountered and pericardium was seen to be bulging and tensely inflated. Incision of the pericardial sac resulted in expulsion of air with a subsequent fall in central venous pressure and heart rate. And after repair of the tear of the laceration of th…

Fatal hemothorax following complicated VV-ECMO cannulation in an anticoagulated patient with severe COVID-19 pneumonia.

Venovenous extracorporeal membrane oxygenation (VV-ECMO) is a lifesaving procedure for patients with acute respiratory distress syndrome (AR … hypotension and bradycardia, which progressed to cardiac arrest. Ultrasonography identified a right hemothorax, and a chest tube was inserted. The patient died despite attempts at resuscitation. Postmortem computed tomography showed a large hemothorax and a guidewire penetrating the RIJV into the right pleural cavity. An autopsy was performed two days post-m …

Hemothorax resulting from hemorrhagic degeneration of giant uterine leiomyomata: A rare variant of pseudo-Meigs' syndrome.

Though rare, hemothorax can occur without trauma, coagulopathy, or apparent vascular injury. Similar to the proposed mechanism of Meigs' syndrome or hepatic hydrothorax, small diaphragmatic defects may allow peritoneal fluid to traverse the diaphragm into the pleural space. While typically this is associated with transudative pleural effusions, there have been case reports of hemoperitoneum crossing the diaphragm into the pleural space. Here, we present a case in which multiple giant leiomyomata with hemorrhagic necrosis led to hemoperitoneum that translocated across the diaphragm into the ple…

Can Intrapleural Tissue Plasminogen Activator and Deoxyribonuclease Be Used to Treat Persistent Hemothorax After Robotic Lobectomy?

Hemothorax is a serious complication following thoracic surgery, often resulting from vessel injury or rib fractures, and is typically managed with chest tube drainage. Persistent or loculated hemothorax, referred to as retained hemothorax, may require more invasive interventions, such as thoracotomy. Although the intrapleural administration of tissue plasminogen activator (tPA) and deoxyribonuclease (DNase) has shown promise in managing pleural infections, its use for hemothorax remains controversial due to bleeding risks. We present a case of a 74-year-old female who developed a retained hem…

Hemothorax caused by injury of musculophrenic artery after ultrasound-guided percutaneous liver biopsy: a case report.

BACKGROUND: Hemorrhage is the most common major complication after liver biopsy. Hemothorax is one type of bleeding and is very rare and dangerous. Several cases of hemothorax subsequent to liver biopsy have been documented, primarily attributed to injury of the intercostal artery or inferior phrenic artery and a few resulting from lung tissue damage; however, no previous case report of hemothorax caused by injury of musculophrenic artery after liver biopsy has been reported. CASE PRESENTATION: A 45-year-old native Chinese woman diagnosed with primary biliary cirrhosis due to long-term rednes…

Hemothorax associated with anticoagulation after placement of implantable cardioverter defibrillator: possible similarity to postinfarction Dressler's syndrome.

Massive hemothorax developed after placement of an implantable cardioverter defibrillator (ICD) in two patients who received postoperative anticoagulants. The possible relationship of this complication to polyserositis after ICD implantation is discussed as are the possible adverse sequelae of early anticoagulation after ICD implantation.

Life-threatening hemothorax after radiofrequency ablation for subcapsular hepatocellular carcinoma in a patient with previous rib injury.

Radiofrequency ablation is a well-established treatment for early-stage hepatocellular carcinoma. Although generally safe, serious complications may occur, particularly when an intercostal approach is required. We report a rare case of life-threatening hemothorax caused by intercostal artery injury following radiofrequency ablation for a subcapsular hepatocellular carcinoma in a patient with a prior rib fracture. Approximately 1 hour after the procedure, the patient developed acute chest symptoms and hemodynamic instability. Contrast-enhanced computed tomography revealed a massive right-sided …

Delayed life-threatening hemothorax following blunt thoracic trauma: A rare and catastrophic evolution in a polytrauma patient.

BACKGROUND: Delayed hemothorax, although infrequent, constitutes a life-threatening complication in the context of blunt thoracic trauma. The temporal dissociation between injury and symptom escalation challenges early detection, and the pathophysiological mechanisms often remain elusive. We present a case of an 18-year-old female who developed massive right-sided hemothorax six days after initial injury, necessitating emergent thoracotomy. CASE PRESENTATION: The patient, a victim of a high-energy vehicular collision, presented with multiple rib fractures, bilateral pneumothorax, and vertebra…

Bronchial artery pseudoaneurysm presenting with hemoptysis and hemothorax after pulmonary lobectomy: a case report and literature review.

BACKGROUND: Bronchial artery pseudoaneurysm is a rare vascular disorder, and cases of bronchial pseudoaneurysms reported after lung surgery are even rarer. The number of reported cases is very limited due to the unclear pathogenesis, lack of diagnostic criteria and treatment guidelines, and nonspecific clinical manifestations. CASE PRESENTATION: The paper reports a case of a patient with primary lung adenocarcinoma who developed hemoptysis, chest and back pain, and right hemothorax after lobectomy. Due to the lack of experience in treatment, we suspected that the above symptoms were caused by…

Factors Associated With Successful Video-Assisted Thoracoscopic Surgery for Traumatic Hemothorax in Children: A Cross-Sectional Study.

INTRODUCTION: Due to the rarity of traumatic hemothorax in children, no studies have evaluated factors associated with successful video-assisted thoracoscopic surgery (VATS) as definitive management. METHODS: We conducted an exploratory cross-sectional analysis of pediatric patients in the Trauma Quality Programs database from 2008 to 2017 with traumatic hemothorax managed with primary VATS. Those with early resuscitative thoracotomy for cardiac arrest were excluded. We stratified patients by blunt or penetrating mechanism and estimated absolute differences (ADs) and 95% confidence intervals …

Life- threatening hemothorax due to the inferior pulmonary ligament injury without obvious organ injuries: a case report.

Traumatic hemothorax is usually associated with obvious organ injuries, such as rib fractures, pulmonary injuries, and other mediastinal injuries. We present a rare case in which a 42-year- old Korean man who fell off of a roof, approximately 3 meters in height, resulting in a life-threatening hemothorax without obvious injuries to the thoracic organs. Chest CT showed a large amount of hemothorax in the right side of the thoracic cavity, and an active bleeding, presumably from the posterior intercostal or the phrenic artery, with a focal aneurysmal change. The emergency thoracotomy was perform…

Report of a rare case: occult hemothorax due to blunt trauma without obvious injury to other organs.

Traumatic hemothorax commonly occurs accompanied by organ damage, such as rib fractures, lung injury and diaphragm rupture. Our reported patient was a 61-year-old man who fell down from a stepladder about 1 meter in height, resulting in a heavy blow to the left abdomen. He consulted a clinic because of left chest pain the next day and was transported to the emergency center of our hospital on diagnosis of hemothorax with hemorrhagic shock. On computed tomography scanning with contrast medium, left hemothorax without rib fracture, diaphragm rupture or obvious organ injury was evident. We found o…

Validation of a safe volumetric cut-off for observation of traumatic hemothorax: A Western Trauma Association multicenter study.

BACKGROUND: Traumatic hemothorax (HTX) contributes significantly to trauma morbidity and is frequently managed with tube thoracostomy (TT). However, TT placement has complications and some HTXs can be successfully observed. Optimal observation criteria remain unclear, and management practices vary. This multicenter study aimed to characterize current practices, determine observation failure rates, and identify predictors of safe observation. METHODS: We conducted a prospective observational study from July 2023 to June 2025 across 16 trauma centers. Adult patients with computed tomography-con…

Spontaneous synchronous bilateral hemothorax as the only finding in primary pleural angiosarcoma: a case report and a literature review.

Angiosarcoma is a rare malignancy of vascular origin, mostly originating from skin, soft tissues, and breast, but rarely also from the pleura. We present the case of a 55-year-old man who referred to our hospital for a spontaneous bilateral hemothorax. The CT angiography did not show any source of active bleeding; plus, no pleural or lung masses were observable. Cytological and microbiological analyses made on a sample of pleural fluid resulted negative. Despite numerous blood transfusions and thoracenteses, the patient deceased from hemorrhagic shock ten days later and the diagnosis of primar…
