Can elective surgery for mandibular and zygomatic complex fractures reduce overall hospital stay without compromising outcomes? Analysis of administrative datasets by the GIRFT programme.

When patients attend the emergency department with facial fractures that require surgery and are immediately admitted, surgery can be delayed as theatre time is prioritised for other more urgent patients. One solution is to send the patient home and admit them as an elective patient at a later date. The aim of this study was to investigate the outcomes of patients admitted directly and those seen as elective patients following fracture of the mandible or zygomatic complex. Data were taken from the hospital episodes statistics (HES) dataset for 2011-2018, and all hospital admissions for mandibu

Antimicrobial stewardship interventions reduce the time to the first antibiotic administration in septic patients in ICUs: regional multicenter study in 7 Latin American high-complexity hospitals.

Delay in the initial administration of antimicrobials is one of the strongest predictors for mortality for septic patients in the intensive care unit (ICU). Given the different logistics among hospitals for antibiotic administration, this delay can take hours. As antibiotic administration involves care coordination and the participation of different team members, education and the antimicrobial stewardship (AMS) program play a key role in reducing these times. This study evaluated the time between the initial prescription and the effective administration of antibiotics for ICU patients in seve

The relationship between timing of screening for gestational diabetes mellitus and maternal and fetal outcomes: A retrospective cohort study linking primary care electronic and hospital administrative data.

BACKGROUND: Gestational diabetes (GDM) is associated with adverse outcomes including a large-for-gestational age (LGA) baby, which in turn is associated with downstream childhood obesity. Appropriate timing of GDM screening is important for prompt initiation and optimization of medical management, potentially mitigating the risk of those outcomes. The present study explored the association between the timing of GDM screening and macrosomia, LGA, shoulder dystocia and caesarean section. METHODS: This retrospective cohort study linked primary care prenatal data and intrapartum data from a provi

Abuse potential and analgesic efficacy of intravenous hydromorphone bolus administration among hospitalized patients with cancer pain: A double-blind, double dummy, randomized crossover trial.

BACKGROUND: There is much concern that opioids administered as intravenous (iv) bolus for pain relief may inadvertently increase their risk for abuse. However, there is insufficient data to support this. The authors compared the abuse liability potential, analgesic efficacy, and adverse effect profile of fast (iv push) versus slow (iv piggyback) administration of iv hydromorphone among hospitalized patients requiring iv opioids for pain. METHODS: In this double-blind, double dummy, randomized, 2 × 2 crossover trial, patients with ≥4 cancer-related pain were randomly assigned to receive either

The use of epinephrine in out-of-hospital cardiac arres : A retrospective study of the effects of administration timing and cumulative doses on outcome in a physician-staffed emergency medical service system.

BACKGROUND: Epinephrine administration during cardiopulmonary resuscitation (CPR) has been a long-standing recommendation, but the evidence is controversial. This study investigated effects of epinephrine administration in a physician-staffed emergency medical service (EMS) system and for the first time addressed the quality of chest compressions. METHODS: Complete datasets of adult patients who suffered out-of-hospital cardiac arrest (OHCA) and received CPR were retrospectively analysed. Factors (time of collapse, bystander CPR, EMS arrival, initial cardiac rhythm, suspected cause of OHCA, an

Infant Whole-Cell Versus Acellular Pertussis Vaccination in 1997 to 1999 and Risk of Childhood Hospitalization for Food-Induced Anaphylaxis: Linked Administrative Databases Cohort Study.

BACKGROUND: Evidence suggests that children who had received an initial priming dose of whole-cell pertussis (wP) vaccine, rather than acellular pertussis (aP) vaccine, had a lower risk of developing IgE-mediated food allergy, the most common cause of anaphylaxis-related hospital presentations of childhood. OBJECTIVE: To assess the association between wP versus aP vaccination in infancy and subsequent hospital presentations for anaphylaxis. METHODS: This study was preregistered under PMID 34874968. Making the business case for an addiction medicine consult service: a qualitative analysis.

BACKGROUND: As the drug poisoning crisis worsens in North America and opioid use disorder (OUD)-related hospital admissions increase, policymakers and hospital administrators are beginning to recognize the important role of hospitals in the OUD care continuum. This study explores and describes how U.S. addiction medicine physicians created and presented business propositions to hospital administrators to support the development of addiction medicine consult (AMC) services. METHODS: Fifteen qualitative interviews were completed with board-certified or board-eligible addiction medicine physicia…

Association between hospital-level factors and early rehabilitation initiation in patients with acute ischemic stroke: a multilevel analysis using a Japanese administrative database from a rural prefecture.

BACKGROUND: Early rehabilitation after acute ischemic stroke improves functional outcomes and is recommended by clinical guidelines. However, initiation of early rehabilitation varies substantially across hospitals, and the contribution of hospital-level factors remains unclear. OBJECTIVE: To examine the association between hospital-level factors and early rehabilitation initiation, adjusting for patient-level characteristics. METHODS: We conducted a multicenter cross-sectional study using the Japanese Diagnosis Procedure Combination database from 43 hospitals in Yamagata Prefecture between

Receipt of social services intervention in childhood, educational attainment and emergency hospital admissions: longitudinal analyses of national administrative health, social care, and education data in Wales, UK.

BACKGROUND: Research consistently finds poorer health and educational outcomes for children who have experienced out-of-home care relative to the general population. Few studies have explored differences between those in care and those in receipt of intervention from social services but not in care. Children receiving social services interventions often experience Adverse Childhood Experiences (ACEs), and deprivation, which are known to negatively impact outcomes. We aimed to estimate the association of different social services interventions with educational outcomes and hospital admissions,

Health Services Use and Outcomes for Hospital Admissions With a Major Cardiovascular Event Recorded in Health Care Administrative Data in Patients Receiving Maintenance Hemodialysis: A Retrospective Cohort Study.

BACKGROUND: Administrative data are used in studies of hemodialysis care to report cardiovascular-related hospitalizations. Showing recorded events are associated with significant health care resource use and poor outcomes would confirm that administrative data algorithms identify clinically meaningful events. OBJECTIVE: The objective of this study was to describe the 30-day health service use and outcomes when a hospital admission with myocardial infarction, congestive heart failure, or ischemic stroke is recorded in administrative databases. DESIGN: This is a retrospective review of linked

Estimating national prevalence of life-limiting conditions amongst infants, children, and adolescents using administrative hospital data in the absence of a unique health identifier; evidence from Ireland.

BACKGROUND: Estimating the total number of children with a life-limiting, complex chronic condition or serious illness has been identified as an important first step in determining how many children might need palliative care in a country or region. Relevant estimates have not been calculated for Ireland, an exercise complicated by the country's lack of a fully operational unique patient identifier in healthcare data. METHODS: This study estimated prevalence of life-limiting conditions amongst children aged 0-19 in Ireland in 2019 and 2024, demonstrating methods in the absence of a unique ide

First dose of misoprostol administration at home or in hospital for medical abortion between 12-22 gestational weeks in Sweden (PRIMA): a multicentre, open-label, randomised controlled trial.

BACKGROUND: Medical abortion after 12 gestational weeks often requires a stay in hospital. We hypothesised that administering the first misoprostol dose at home could increase day-care procedures as compared with overnight care procedures, shorten inpatient stays, and improve patient satisfaction. METHODS: This multicentre, open-label, randomised controlled trial was done at six hospitals in Sweden. Participants were pregnant people aged 18 years and older who were undergoing medical abortion at 85-153 days of pregnancy. Randomisation was done in blocks 1:1 to mifepristone administered in-cli

Zoledronic acid significantly improves pain scores and quality of life in breast cancer patients with bone metastases: a randomised, crossover study of community vs hospital bisphosphonate administration.

Patients with bone metastases from breast cancer often experience substantial skeletal complications -- including debilitating bone pain -- which negatively affect quality of life. Zoledronic acid (4 mg) has been demonstrated to reduce significantly the risk of skeletal complications in these patients and is administered via a short, 15-min infusion every 3 weeks, allowing the possibility for home administration. This study compared the efficacy and safety of zoledronic acid administered in the community setting vs the hospital setting in breast cancer patients with > or =1 bone metastasis rec

Medicare program; hospital inpatient prospective payment systems for acute care hospitals and the long-term care hospital prospective payment system and fiscal year 2015 rates; quality reporting requirements for specific providers; reasonable compensation equivalents for physician services in excluded hospitals and certain teaching hospitals; provider administrative appeals and judicial review; enforcement provisions for organ transplant centers; and electronic health record (EHR) incentive program. Final rule.

We are revising the Medicare hospital inpatient prospective payment systems (IPPS) for operating and capital-related costs of acute care hospitals to implement changes arising from our continuing experience with these systems. Some of these changes implement certain statutory provisions contained in the Patient Protection and Affordable Care Act and the Health Care and Education Reconciliation Act of 2010 (collectively known as the Affordable Care Act), the Protecting Access to Medicare Act of 2014, and other legislation. These changes are applicable to discharges occurring on or after October
