'That's kind of under my work blanket'-redeployment experiences of children's hospital staff during the covid-19 pandemic: a qualitative interview study.

BACKGROUND: During COVID-19 pandemic, a rapid readjustment to continued delivery of healthcare was required. Redeployment is an intentional process to mobilise human resources by reassigning a healthcare worker to a new role or new work location, to achieve sustainable delivery of patient care. We report redeployment experiences of staff from a specialist children's hospital during first and second waves of the United Kingdom COVID-19 pandemic. METHODS: This study focuses on a specialist children's hospital where redeployment occurred externally to adult intensive care units and other COVID-1

Perspectives and practices of counselling, consent and debriefing for cesarean sections: a mixed-methods study in two Indian states.

BACKGROUND: Effective counselling, consent, and debriefing (CCD) are essential for respectful, quality cesarean section (CS) services but limited evidence exists. We explored providers and clients' perceptions and practices regarding CCD for CS in India. METHODS: A mixed-methods study was conducted in seven purposively selected secondary level hospitals (five public, two private) across two states. Key-informant interviews and in-depth interviews (N = 108) with providers, women and their relatives, and community workers were conducted in all seven hospitals. Focused ethnography and record rev

The road to zero preventable birth injuries.

BACKGROUND: The Seton Family of Hospitals' experience in developing and implementing transformational practices in labor and delivery (L&D) units aimed at reducing the rate of birth trauma at our facilities was previously reported. METHODS: Seton began its individual perinatal safety effort in earnest in October 2003. The endeavor brought together the four hospitals that offer obstetrical services, resulting in the establishment of an interdisciplinary team. The team meets monthly to develop and monitor best practices that are then shared, executed, and validated by each respective site's per

Dynamic responses in the human methylome to exertional heat exhaustion, heat injury, and heat stroke.

Exertional heat illness encompasses a continuum from heat exhaustion (EHE) to heat stroke (EHS), yet the molecular mechanisms remain poorly understood. DNA methylation offers a stable epigenetic signature linking environmental stress to gene regulation and long-term physiological outcomes. We profiled genome-wide DNA methylation in blood from active-duty service members hospitalized for EHE (n = 36), heat injury (EHI; n = 18), or EHS (n = 50). Blood was collected longitudinally and analyzed using the Illumina 850 K array, with normalization and time-course clustering (TCseq) to identify co-reg

Fear and loathing in New England: examining the health-care perspectives of homeless people in rural areas.

Little anthropological research has been conducted on the health care perspectives of homeless people in rural areas. The aim of the present study is to elicit health-care beliefs, and examine overall health experience among a sample of current or recently homeless people in rural New Hampshire. Thirteen qualitative interviews were conducted, 11 with single men. Interviews were audio-recorded and transcribed, and analyzed using thematic analysis. Despite a massive burden of disease and illness, almost all participants reported an abiding aversion to doctors, hospitals and professional health c

Crisis, competition and care: Market disruption in elective gynecology in the Netherlands due to COVID-19.

BACKGROUND: Regulated healthcare markets rely on provider competition to improve efficiency and access. During the COVID‑19 pandemic, Dutch hospitals were urged to prioritise emergency COVID care, while independent treatment centres (ITCs) were largely exempt. This reallocation of capacity substantially reduced hospitals' ability to deliver elective gynecological care and may have affected medium‑run market dynamics between provider types. OBJECTIVES: This study examines how the COVID-19 pandemic affected revenues, treatment volumes, and surgical care proportions in benign gynecological care

Leading across boundaries: establishment of the UCLH community outreach intrathecal baclofen refill service.

BACKGROUND: Intrathecal baclofen (ITB) is a recognised treatment for severe spasticity. ITB users need to attend a specialist clinic regularly for their pump to be refilled and reprogrammed. Many patients travel significant distances and require hospital transport which is inconvenient, stressful and costly. These challenges inspired the University College London Hospitals (UCLH) spasticity team to explore establishing a community outreach clinic, staffed by the UCLH team, to provide care closer to home for this complex group. METHOD: By mapping the patient cohort, using their general practit

Beyond HAART: unveiling the reality of antiretroviral therapy knowledge among pregnant women living with HIV in Kaduna State, Nigeria.

To ensure the prevention of mother-to-child-transmission of HIV (MTCT), 81% of pregnant women received antiretroviral therapy (ART) in 2021 worldwide. Despite the global efforts to prevent MTCT of HIV, significant knowledge gaps persist among these women including a lack of basic knowledge of HIV and ART. This study aimed to determine the levels of HIV knowledge, knowledge of ART, and attitude toward ART in HIV-positive pregnant women in Kaduna State. It was a hospital-based, cross-sectional study among 318 HIV-positive pregnant women attending antenatal care in the nine selected hospitals usi

Cancer rehabilitation and palliative care: critical components in the delivery of high-quality oncology services.

Palliative care and rehabilitation practitioners are important collaborative referral sources for each other who can work together to improve the lives of cancer patients, survivors, and caregivers by improving both quality of care and quality of life. Cancer rehabilitation and palliative care involve the delivery of important but underutilized medical services to oncology patients by interdisciplinary teams. These subspecialties are similar in many respects, including their focus on improving cancer-related symptoms or cancer treatment-related side effects, improving health-related quality of

Outcomes of a randomised controlled trial of a complex genetic counselling intervention to improve family communication.

When an inherited genetic condition is diagnosed in an individual it has implications for other family members. Privacy legislation and ethical considerations can restrict health professionals from communicating directly with other family members, and so it is frequently the responsibility of the first person in a family to receive the diagnosis (the proband) to share this news. Communication of genetic information is challenging and many at-risk family members remain unaware of important information that may be relevant to their or their children's health. We conducted a randomised controlled

Acute Pain Medicine in the United States: A Status Report.

BACKGROUND: Consensus indicates that a comprehensive, multimodal, holistic approach is foundational to the practice of acute pain medicine (APM),but lack of uniform, evidence-based clinical pathways leads to undesirable variability throughout U. S. healthcare systems. Acute pain studies are inconsistently synthesized to guide educational programs. Advanced practice techniques involving regional anesthesia assume the presence of a physician-led, multidisciplinary acute pain service, which is often unavailable or inconsistently applied. This heterogeneity of educational and organizational standards

An assessment of the feasibility of telephone and email consultation in a chest clinic.

One hundred and sixty-four consecutive patients attending a busy respiratory outpatient service were asked how acceptable was the concept of alternating face to face consultation with consultation by either telephone or email. The patients were then assessed as to their suitability for such non-traditional methods of consultation. Thirty patients (18.3%) were not agreeable to other forms of consultation and five could not speak English. One hundred and thirty-three (84%) had a suitable daytime telephone number for consultation purposes, but only 34 (21%) had email access, with this being commo

Linking Healthcare Quality and Knowledge Management in Traditional and Ethnomedicine-Oriented Hospitals: A Scoping Review and Implementation Framework.

Traditional and ethnomedicine-oriented hospitals operate at the intersection of culturally embedded clinical knowledge, contemporary biomedical evidence, regulatory requirements, and patient expectations. Their quality challenges therefore extend beyond conventional compliance and include the preservation, validation, translation, and safe application of knowledge. This scoping review mapped how healthcare quality management and knowledge management have been conceptualized and operationalized in hospital and health service settings relevant to traditional, complementary, integrative, Indigeno
