HIV-infected persons entering the criminal justice system (CJS) often experience suboptimal healthcare system engagement and social instability, including homelessness. We evaluated surveys from a multisite study of 743 HIV-infected jail detainees prescribed or eligible for antiretroviral therapy (ART) to understand correlates of healthcare engagement prior to incarceration, focusing on differences by housing status. Dependent variables of healthcare engagement were: (1) having an HIV provider, (2) taking ART, and (3) being adherent (≥95% of prescribed doses) to ART during the week before inca

As social and economic conditions are key determinants of HIV, the United States 'National HIV/AIDS Strategy (NHAS)', in addition to care and treatment, aims to address mental health, unemployment, food insecurity, and housing instability, as part of its strategic plan for the 'Ending the HIV Epidemic' initiative. Although mechanistic models of HIV play a key role in evaluating intervention strategies, social conditions are typically not part of the modeling framework. Challenges include the unavailability of coherent statistical data for social conditions and behaviors. We developed a method,

OBJECTIVE: The aim of the study was to investigate how social housing with high locomotion activity affects experimental osteoarthritis (OA) in rats.
DESIGN: Rats were housed either conventionally in type IV cages in pairs or in rat colony cages (RCC) on 4 levels interconnected by jump holes or staircase in groups of 48. OA was induced by anterior cruciate ligament transection and resection of the medial meniscus (ACLT + tMx), medial meniscal tear (MMT) or destabilization of the medial meniscus (DMM). Functional changes were characterized by continues tracking of individual activity and catwa

Family homelessness is shaped by social and structural determinants of health, including poverty, housing insecurity, fragmented services, and unequal access to support. Families with infants and young children are particularly vulnerable to the health and developmental effects of housing instability. Public Health Nurses (PHNs) are uniquely positioned at the interface of health services, family life, and community systems. Through universal reach, relational continuity, developmental surveillance, safeguarding, advocacy, and interagency coordination, PHNs help families remain connected to car

IntroductionWhile prior research has examined individual health-related social needs (HRSNs) in relation to preventive care, few studies have investigated how cumulative social needs affect cancer screening behaviors. This study examines how individual and combined HRSNs affect breast and cervical cancer screening completion.MethodsParticipants included 1,964 female patients overdue for breast or cervical cancer screening, receiving care at Federally Qualified Health Centers. The dependent variable was the completion of the appropriate screening test within 90 days of SMS text message reminder

Pressure injuries are challenging chronic wounds because healing depends on systemic and patient-specific factors in addition to local wound care. Patients with Crohn's disease may be particularly vulnerable to impaired healing because chronic inflammation, nutritional compromise, prior intestinal surgery, infection, and limited mobility can interfere with tissue repair. Sustaining improvement after hospital discharge may be further complicated by social barriers that limit continuity of wound care. We report the case of an adult man with advanced fistulizing Crohn's disease who was admitted f

Provider organizations are increasingly held accountable for health care spending in vulnerable populations. Longitudinal data on health care spending and use among people experiencing episodes of homelessness could inform the design of alternative payment models. We used Medicaid claims data to analyze spending and use among 402 people who were continuously enrolled in the Boston Health Care for the Homeless Program (BHCHP) from 2013 through 2015, compared to spending and use among 18,638 people who were continuously enrolled in Massachusetts Medicaid with no evidence of experiencing homeless

OBJECTIVE: Research on homelessness among persons with severe mental illness tends to focus on aspects of demand, such as risk factors or structural and economic forces. The authors address the complementary role of supply factors, arguing that "solutions" to residential instability-typically, a series of institutional placements alternating with shelter stays-effectively perpetuate homelessness among some persons with severe mental illness.
METHODS: Thirty-six consecutive applicants for shelter in Westchester County, New York, in the first half of 1995 who were judged to be severely mentally

Patients experiencing homelessness (PEH) continue to experience acute and chronic medical vulnerabilities in the United States. In the acute setting, these disparities increase hospital readmission rates, utilization of optimal treatment, and mortality. In the chronic setting, these disparities increase housing instability, medical deterioration, and mortality. These vulnerabilities are exacerbated in New York City (NYC), with PEH experiencing longer hospital stays, increased hospitalization costs, and disproportionate mental health and substance use hospitalizations. Over the past few decades

BACKGROUND: Population-level tracking of hospital use patterns with integrated care organizations in patients experiencing homelessness has been difficult. A California law implemented in 2019 (Senate Bill 1152) aimed to ensure safety for this population after discharge from the hospital by requiring additional documentation for patients experiencing homelessness, which provides an opportunity to evaluate hospital use by this population.
METHODS: In a large integrated health system in California, patients experiencing homelessness were identified through documentation change requirements asso

The number of families living in shelters in the Paris region (France) has increased by a factor of three in 10 years. In 2013, a survey was performed on homeless families in order to characterize their living conditions, their health needs, and the developmental problems in children. This probability survey was conducted in 17 languages among 801 homeless families sheltered in emergency centers for asylum-seekers, emergency housing centers, social rehabilitation centers, and social hotels in the Paris region. Among the 772 families that provided data on food security only 14.0% were with food

Scholars are beginning to use the concept medicalization of poverty to theorize how the United States spends large amounts of money on illnesses related to poverty but invests much less in preventing these illnesses and the conditions that create them (e.g., economic insecurity, housing instability, continuous exposure to violence, and racism). This study examines the connection between poverty, disease burden and health-related costs through the in-depth interviews of 86 Black mothers living in neighborhoods with high levels of violence on the South Side of Chicago. The rippling costs of pove

Multiple chemical sensitivity (MCS), a recognized disability, is a chronic, multisystem condition in which exposure to common chemical substances provokes adverse health effects and functional impairment. Although population-level data and emerging clinical research support the pathophysiological basis of MCS, individuals living with the condition frequently encounter barriers to recognition and accommodation across employment, healthcare, and housing systems. This study examines patterns of accommodation requests, refusals, and associated socioeconomic impacts among adults living with MCS in

OBJECTIVE: To investigate the housing situation, service use and substance use among patients with substance use disorders seeking psychiatric treatment.
METHODS: Structured interview among 540 psychiatric inpatients including day clinics in a psychiatric centre in Berlin, Germany.
RESULTS: Patients under instable housing conditions were significantly more often diagnosed with a comorbid psychotic disorder and were significantly younger when first psychiatric or substance use treatment occurred, on average before onset of homelessness.
CONCLUSION: Homelessness among people with substance us

IMPORTANCE: Childhood housing insecurity has dramatically increased in the US in recent decades, but whether an association with adverse mental health outcomes exists after adjusting for repeated measures of childhood poverty is unclear.
OBJECTIVE: To test whether childhood housing insecurity is associated with later anxiety and depression symptoms after adjusting for time-varying measures of childhood poverty.
DESIGN, SETTING, AND PARTICIPANTS: This prospective cohort study included individuals aged 9, 11, and 13 years at baseline from the Great Smoky Mountains Study in western North Caroli

BACKGROUND: Research conducted in urban areas has highlighted the impact of housing instability on people who inject drugs (PWID), revealing that it exacerbates vulnerability to drug-related harms and impedes syringe service program (SSP) use. However, few studies have explored the effects of houselessness on SSP use among rural PWID. This study examines the relationship between houselessness and SSP utilization among PWID in eight rural areas across 10 states.
METHODS: PWID were recruited using respondent-driven sampling for a cross-sectional survey that queried self-reported drug use and SS

Transgender women experience high prevalence of homelessness, which can affect their likelihood of acquiring HIV infection and can lead to poor medical outcomes. CDC analyzed data from the National HIV Behavioral Surveillance Among Transgender Women to identify whether personal characteristics and social factors affecting transgender women were associated with duration of homelessness during the past 12 months. Longer duration and chronic homelessness might indicate greater unmet needs, which increases their likelihood for acquiring HIV infection. Ordinal logistic regression was conducted to c

BACKGROUND: After release from incarceration, individuals are rarely connected to primary care or to social services despite bearing a disproportionate burden of poor health (e.g., chronic illness) and structural determinants of health (e.g., housing instability). The Rhode Island Transitions Clinic (RITC) works to fill this gap, particularly for patients with complex needs, by linking individuals to primary care and social services. However, prior work has not formally assessed how successful the TCN is at connecting patients to social services. Our objective was to assess the relationship be

Methicillin-resistant Staphylococcus aureus (MRSA) infection is a leading cause of hospitalization and medical visits among individuals experiencing homelessness and also among persons who inject drugs (PWID), populations with significant overlap in urban centers in the USA. While injection drug use is a risk factor for MRSA skin infections, MRSA is also known to transmit easily in crowded, public locations in which individuals have reduced personal hygiene. Individuals in urban centers who experience homelessness or drug addiction may spend significant amounts of time in environments where MR
