Outcomes for Completely Displaced and Rotated Lateral Humeral Condyle Fractures in Children: A Multicenter Study.

INTRODUCTION: Song V lateral humeral condyle fractures represent a displaced and rotated fragment, historically treated with open reduction and percutaneous pinning (ORPP) or screw fixation (ORSF). We aim to describe outcomes of Song V fractures and analyze the effect of fixation and reduction type on outcomes. METHODS: This retrospective review assessed data from 6 level 1 trauma centers from 2005 to 2019. Inclusion criteria were children aged 1 to 12 with a Song V fracture. Patient clinic and surgical notes until 1 year after the primary intervention were reviewed. Radiographic healing data

The radial nerve palsy caused by embedding in the humeral shaft fracture - a case report.

This paper examines a case of radial nerve palsy in a 13-year-old girl caused by nerve embedded in a humeral shaft fracture. An X-ray showed a mild angular-displaced humeral shaft fracture. Two weeks later, an intramedullary Ender nailing was performed. Because of brain injury due to traffic accident, the patient lost consciousness for one month. Once she regained consciousness, radial nerve palsy was discovered and closely observed. As the radial nerve palsy had not healed after five months, the radial nerve was explored and found to be interposed between fracture sites and totally entrapped

The comparison of functional bracing of humeral shaft fractures: thermoplastic custom against preprepared commercial braces.

BACKGROUND: Functional humeral bracing of midshaft humeral fractures has been widely accepted as a gold standard for nonoperative treatment. Despite reported high union rates, there is no proven superiority of any orthosis. Here we aim to compare the outcomes after the use of custom-made thermoplastic vs. commercial humeral brace with regard to time to union, nonunion rates, types of nonunion, and conversion to surgery. METHODS: Patients with humeral fractures treated between 2018 and 2021 were identified retrospectively by electronic records. Only diaphyseal humerus fractures (AO 12) were in

Long-stemmed humeral components in primary shoulder arthroplasty.

BACKGROUND: Long-stemmed humeral components are often associated with revision shoulder arthroplasty. However, long-stemmed humeral components will likely prove useful in selected patients with extra large shoulders and in those with bone loss from nonarthroplasty causes and in humeral fractures. This study was developed to examine the frequency of use of longer humeral stems, identify the indications for their use, define the results, and enumerate the complications encountered. MATERIALS AND METHODS: Thirty-five primary shoulder arthroplasties were followed-up clinically and radiographicall

Double crush syndrome due to plating of humeral shaft fracture.

Median nerve injury is rarely associated with the humeral shaft fracture. A Sixty two year old woman with a displaced humeral shaft fracture, developed a symptomatic carpal tunnel syndrome after plating with a screw protruding medially. 16 months later, the implants were removed and the symptoms gradually improved without carpal tunnel release surgery. A double crush syndrome resulted due to the proximal compression by the medially protruding screw and the distal compression by carpal tunnel. The proximal decompression produced by removal of the screw led to relief of the symptoms.

Operative versus conservative management of distal humerus fractures in patients aged 65 years and older: a target-trial-informed retrospective cohort study of five-year mortality.

INTRODUCTION: Older patients with distal humerus fractures die more often after conservative than after operative treatment in unadjusted comparisons; whether this reflects treatment or the selection of healthier patients for surgery is unresolved. We estimated the association between operative versus conservative management and five-year all-cause mortality in patients aged ≥ 65 years. MATERIALS AND METHODS: Retrospective comparative cohort study at two tertiary trauma centres (2012-2025). Cases were ascertained by automated screening of codes and free text, then adjudication of all uncertai

Posterolateral Locked Plate and Percutaneous Medial Screw Fixation for Distal Humerus Fractures in Elderly.

Background: A high incidence of ulnar nerve-related complications has been reported in open reduction and internal fixation for distal humerus fractures (DHFs). To minimise ulnar nerve damage, we used a percutaneous medial screw combined with a posterolateral plate in the elderly. The aim of this study was to evaluate the postoperative complications and functional outcomes of this method. Methods: Data from patients aged over 65 who underwent this surgical procedure for DHFs at a single Level I trauma centre from 2013 to 2021 were extracted. Postoperative complications, reoperations, mean rang

The 3D-Printed Custom Elbow Prosthesis for Salvage Treatment of Complex Intra-Articular Distal Humerus Fracture Malunion.

Intra-articular fractures of the distal humerus are complex injuries that often require surgery with the goal of restoring elbow range-of-motion and function. Open reduction and internal fixation has been the preferred surgical modality; however, restoration of the medial and/or lateral columns can be complicated in fractures involving a major loss of the articular surface and bony structure. Over the past decade, 3-dimensional (3D) printing has made significant advances in the field of orthopedic surgery, specifically in guiding surgeon preoperative planning. Recently, the incorporation of 3D

Complex revision elbow arthroplasty for periprosthetic Holstein-Lewis fracture with delayed onset radial nerve entrapment after skydiving injury.

Holstein-Lewis fractures are spiral fractures in the distal third of the humeral shaft, which carry increased risk of radial nerve palsy compared with other humeral shaft fractures; these fractures are usually managed conservatively. However, there is no case in the existing literature that describes the management of Holstein-Lewis fractures with radial nerve entrapment in a patient with a history of total elbow arthroplasty (TEA). Here, we present the first documented case of a complex revision elbow arthroplasty for Holstein-Lewis fracture with delayed onset radial nerve entrapment after a

Sequential Opposite-pattern Supracondylar Fractures in the Same Elbow of a Child.

Supracondylar humerus fractures in children are most commonly of the extension type, whereas flexion-type injuries are rare. Sequential occurrence of extension- and flexion-type supracondylar fractures in the same elbow is extremely uncommon. We report a child who sustained a Gartland type II extension-type supracondylar fracture at 5 years of age, managed conservatively with above-elbow casting. He recovered fully and resumed normal activities. Ten months later, at age 6 years, he sustained a new fall on the same limb and presented with a Gartland type III flexion-type supracondylar fracture.
