The value of using loupe magnification and methylene blue dye in intra-operative identification of thyroglossal duct tract.

BACKGROUND: This was a comparative randomised study to find out the value of using loupe magnification and methylene blue dye in identification of the thyroglossal duct and to compare between them. PATIENTS AND METHODS: Twenty-two patients who presented with infrahyoid thyroglossal duct cyst were subjected to excision of the cyst with two methods for identification of thyroglossal duct track during the operation. Data were anlysed for identification of multiple tracts, the tract relation to the hyoid bone, incidence of complications and operative time. RESULTS: We found that multiple tracts …

Cadaver Study on Transoral Thyroid Chondrolaryngoplasty and Transoral Sistrunk Operation, Possibility from Transoral Thyroidectomy.

OBJECTIVE: Transoral approaches in surgery are gaining global popularity. Transoral endoscopic and robotic thyroidectomies are well established due to the benefit of being truly scarless. Thyroid chondrolaryngoplasty reduces protrusion of the thyroid notch in transgender patients. The Sistrunk operation involves removal of the thyroglossal cyst and the central portion of the hyoid bone. In this cadaveric study, we developed a transoral thyroidectomy approach and assessed the feasibility of applying this technique to thyroid chondrolaryngoplasty and Sistrunk operation with the proper usage of a…

The relationship between severity of obstructive sleep apnoea/hypopnoea syndrome (OSAHS) and lateral cephalometric radiograph values: a clinical diagnostic tool.

BACKGROUND: This prospective cross-sectional cohort study examined the relationship between radiographic anatomy and the severity of OSAHS. … ity of OSAHS as measured by the Epworth score. Overjet, lower lip length, and the distance from the hyoid bone to a point B on the mandible all increased significantly with increasing severity of OSAHS, as measured by the AHI. The hyoid bone was found to rotate counter clockwise as the severity of OSAHS increased, as a result the distance between the most an …

[Laryngeal fractures due presumably to tracheal intubation in resuscitation].

In three autopsy cases performed in our department, we observed the throat-skeleton fractures occurring presumably during the tracheal intubation in resuscitation. Case 1. 63-year-old man died of acute hemorrhagic pancreatitis shortly after the quarrel with a suspected person. The autopsy examination showed the fractured hyoid bone with haemorrhage at the fracture site. The question whether direct pressure on his neck by the suspected person results the fracture of the hyoid bone was investigated. Case 2. 75-year-old man treated for senile dementia was clubbed with a walking stick by the other…

Cervical posture analysis in dental students and its correlation with temporomandibular disorder.

OBJECTIVE: To evaluate the relationship between temporomandibular disorders (TMD) and craniocervical posture in the sagittal plane measured from lateral radiographs of the head. METHODS: The sample was comprised of 80 randomly selected students of dentistry at the Federal University of Rio Grande do Norte. Research Diagnostic Criteria for TMD (RDC/TMD) was used to evaluate the signs and symptoms of TMD. Lateral radiographs of each individual were used to measure the position of the hyoid bone, the craniocervical angle, and the occiput-atlas distance. A chi-square test was used to evaluate the…

Effect of surgical reduction of the tongue on oral stereognosis, oral motor ability, and the rest position of the tongue and mandible.

The oral ability to recognize forms and oral motor ability were studied by means of two specific tests in 27 subjects, 10 to 23 years of age, before and after tongue reduction because of macroglossia. Recordings were made before and 6 and 12 months after the operation. At the same time the natural position of the head and of the cervical column, the craniocervical relation, the position of the tongue and the hyoid bone, and the rest position of the mandible were studied with profile roentgen-cephalometry. The surgical reduction of the tongue had a minor influence on the subject's performance i…

Failure of OPHL type IIb due to undiagnosed Eagle syndrome.

A 52-year-old man with glottic-supraglottic tumour underwent open partial horizontal laryngectomy (OPHL) IIb. On the 12th day postoperative, laryngoscopy showed necrotic tissue at the level of pexy and an increased distance between tongue base and neoglottis; the neck CT showed cricoid arch rupture and rupture of the pexy. By re-examining the preoperative CT images, the ossification of stylohyoid ligament (Eagle syndrome) was detected and supposed as the possible cause of cricoid rupture due to its traction on the hyoid bone and therefore on the pexy. The stylohyoid ligaments were cut at their…

Bedside Ultrasound to Identify and Predict Severity of Dysphagia Following Ischemic Stroke: Human Versus Artificial Intelligence.

OBJECTIVE: Dysphagia is a significant ischemic stroke complication that can lead to aspiration. Identification of at-risk patients can be logistically difficult and costly. Researchers investigated whether quantitative ultrasound assessment of hyoid bone movement during induced swallowing would predict failure of videofluoroscopy (VFS) or fiberoptic endoscopic evaluation of swallowing (FEES), as determined by a penetration-aspiration scale (PAS) score. Additionally, ability of a machine learning (ML) algorithm to predict PAS success or failure from real-time ultrasound video recordings was ass…

Genioglossus Advancement and Hyoid Surgery.

The structure and dimensions of the mandible, tongue, and hyoid complex are important variables in the pathophysiology of obstructive sleep apnea at the hypopharyngeal level. Genioglossus advancement is based on mandibular osteotomy, which brings the genioglossus muscle (GGM) forward and prevents posterior collapse during sleep. The genioglossus advancement technique has recently undergone several modifications; each has attempted to minimize surgical morbidity while improving the incorporation and advancement of the GGM. The hyoid bone has been of interest in sleep apnea and apnea-related sur…

Unusual variations of the stylo-hyoid complex: case report and review of literature.

BACKGROUND: According to several descriptions, the hyoid apparatus consists of the styloid process (SP), stylohyoid ligament, and lesser horn of the hyoid. However, elongation of the SP plays the most important role in the development of clinically important symptoms, called Eagle syndrome. Several theories clarifying SP elongation have been proposed. Some researchers have described a congenital etiology of this finding, whereas others have postulated that ossification of the stylohyoid ligament may result in such elongation. Nevertheless, regardless of the origin, elongated SPs can provoke sy

Expansion hyoidplasty: a potential surgical procedure designed for selected patients with obstructive sleep apnea syndrome. Experimental canine results.

Some patients with "idiopathic" obstructive sleep apnea syndrome have an inspiratory, circumferential, passive collapse of the hypopharyngeal lumen as the mechanism of airway obstruction. Permanent tracheotomy has offered documented dramatic resolution of the airway obstruction with reversal of the associated cardiopulmonary sequelae. Tracheotomy is not, however, without psychological and physical complications detracting from its use. In hope of developing a surgical alternative, the expansion hyoidplasty was conceived. The hyoid bone is transected just medial to each lesser cornu. The trisec…
