Construction of a stable nipple valve with processed dermal sheep collagen for continent ileostomy and urostomy.

Complications of the nipple valve system used for continent urostomy and ileostomy basically consist of incomplete (or complete lack of) adhesion of the invaginated serosal surfaces of the valve. A novel concept in fixation of the valve by means of the intussusception of a newly developed biomaterial (processed dermal sheep collagen; PDSC) was tested. The implanted PDSC is characterized by induction of fibroblast invasion and formation of new collagen fibers, initial high tensile strength, bio-inertion and (slow) biodegradation. It was implanted between the serosal surfaces of the invagination

Colonic metaplasia of ileostomies. Biological significance for ulcerative colitis patients following total colectomy.

Two patients who had undergone proctocolectomy for ulcerative colitis developed lesions in their ileal stoma that appeared to be inflammatory polyps morphologically similar to those encountered in the large intestine of ulcerative colitis patients. One of these patients eventually developed mucinous adenocarcinoma in the ileal stoma. The ileal mucosa adjacent to the neoplasm had morphologic features of large-bowel mucosa and was richly populated by sulfomucin-containing goblet cells, which are characteristic of large-bowel mucosa. Sulfomucin-containing goblet cells were also found in the infla

Colonic metaplasia with colonic-type polyps on an ileostomy stoma in polyposis coli. Report of a case.

A unique case is reported in which chronic physical and chemical irritation of an ileostomy stoma (after proctocolectomy for polyposis coli) was associated with colonic metaplasia and formation of colonic-type tubular adenomas on the external surface of the ileostomy. The remainder of the terminal ileum and the upper gastrointestinal tract were normal, with no evidence of Gardner's syndrome. This case demonstrates that even metaplastic colonic epithelium is susceptible to the formation of adenomas in polyposis coli patients.

Ileostomy and excisional surgery for chronic inflammatory disease of the colon: a survey of one hospital region.

Four hundred and fifty-three patients who underwent ileostomy and excisional large bowel surgery for chronic inflammatory disease at two teaching hospitals and 35 non-teaching hospitals within the North East Metropolitan Hospital Region during the years 1955-66 were surveyed through a study of hospital case records. The surgical results and complications are presented in part I; a study of the health of these patients after their discharge from hospital is made in part II in an attempt to answer the question posed by many ileostomists, ;Are we as healthy as other people?'

Comparative Outcomes of Loop Ileostomy and Loop Transverse Colostomy Following Curative Resection for Rectal Cancer.

INTRODUCTION: Temporary diversion of the fecal stream with a loop stoma is commonly performed to safeguard against an anastomotic failure and cause further complications. Commonly used stomas include loop ileostomy (LI) and loop transverse colostomy (LTC). Both of these techniques have their own complication profiles. This study highlights the comparison of patient outcomes and quality of life in these two stoma groups following curative resection for rectal cancer. METHODS: This randomized comparative study was carried out in the Colorectal Surgery Unit of Alexandria University Hospitals, Al

Micro-power negative pressure wound technique reduces risk of incision infection following loop ileostomy closure.

BACKGROUND: Prophylactic loop ileostomy is an effective way to reduce the clinical severity of anastomotic leakage following radical resection of rectal cancer. Incisional surgical site infection (SSI) is a common complication after ileostomy closure. AIM: To evaluate the efficacy and safety of the micro-power negative pressure wound technique (MPNPWT) in preventing incisional SSI. METHODS: This was a prospective, randomized controlled clinical trial conducted at a single center. A total of 101 consecutive patients who underwent ileostomy closure after rectal cancer surgery with a prophylact

Protective ileostomy creation after anterior resection of the rectum: Shared decision-making or still subjective?

AIM: The choice of whether to perform protective ileostomy (PI) after anterior resection (AR) is mainly guided by risk factors (RFs) responsible for the development of anastomotic leakage (AL). However, clear guidelines about PI creation are still lacking in the literature and this is often decided according to the surgeon's preferences, experiences or feelings. This qualitative study aims to investigate, by an open-ended question survey, the individual surgeon's decision-making process regarding PI creation after elective AR. METHOD: Fifty four colorectal surgeons took part in an electronic

Minimally Invasive Correction of Prolapsed, Gangrenous Distal Limb of Loop Ileostomy to End-Loop Stoma.

INTRODUCTION: Prolapse can be a complication of loop stomas. A prolapsed stoma which cannot be reduced or complicated with strangulation needs surgical correction. This case report describes a minimal access correction of a prolapsed gangrenous distal limb of ileostomy. Presentation of Case. A 67-year-old male patient was diagnosed with a lower rectal carcinoma, staged T3N1M0. Following neoadjuvant chemoradiation, he underwent a laparoscopic anterior resection with a defunctioning loop ileostomy. One month later, he presented with prolapse of the distal limb of the ileostomy. The limb was gang

Colonic transit in the empty colon after defunctioning ileostomy: do we really know what happens?

BACKGROUND: There is disagreement amongst surgeons about the use of oral mechanical bowel preparation (MBP) prior to low anterior resection with diverting ileostomy. Colonic transit in the early post-operative period is an important factor in determining the role of MBP, as propagation of any stool remaining in the defunctioned colon may exacerbate morbidity in the event of anastomotic leak. We studied colonic transit time in the first 7 days following low anterior resection with diverting ileostomy. METHODS: We conducted a prospective observational study of patients with rectal cancer underg

Morbidity related to defunctioning ileostomy closure after ileal pouch-anal anastomosis and low colonic anastomosis.

PURPOSE: Defunctioning ileostomies are widely performed in order to prevent or treat anastomotic leakage after colorectal surgery. The aim of the present study was to determine morbidity related to stoma closure and to identify predictive factors of a complicated postoperative course. METHODS: A consecutive series of 138 patients were retrospectively analyzed after stoma reversal. Data collection included general demographics and surgery-related aspects. Morbidity related to stoma closure was retrieved from our prospectively collected registry of complications. RESULTS: In 74 of 138 patients

Diverting loop ileostomy after restorative proctocolectomy: predictors of poor outcome and poor quality of life.

AIM: Diverting loop ileostomy is used to minimize the impact of anastomotic complication after restorative proctocolectomy (RPC). However, the ileostomy itself may have complications and therefore affect quality of life (QOL). The aim of this study was to analyse the predictors of complications of the ileostomy formation and closure and of the QOL of these patients. METHOD: Forty-four consecutive patients who underwent RPC were enrolled. Records of the ileostomy follow-up were retrieved from a prospectively collected database and QOL was assessed with the Stoma-QOL questionnaire. Ileostomy si

Intestinal diversion (colostomy or ileostomy) in patients with severe bowel dysfunction following spinal cord injury.

BACKGROUND: Spinal cord injury (SCI) affects motor and sensory nervous integrity resulting in paralysis of lower or both upper and lower extremities, as well as autonomic nervous system function resulting in neurogenic bowel. SCI leads to diminished or lost sensations of the need to defecate or inability to distinguish the presence of gas versus liquid versus solid stool in the rectal vault. Sensory loss, incomplete evacuation of stool from the rectal vault, immobility, and reduced anal sphincter tone increase the risk of fecal incontinence. Gastrointestinal symptoms are associated with depres

Magnetic Duodeno-Ileostomy as a Conversion Procedure After Sleeve Gastrectomy: First Single-Centre Experience in Colombia.

BACKGROUND: Magnetic duodeno-ileostomy (MAGDI) uses magnetic compression to create a side-to-side duodeno-ileal anastomosis without sutures or staples at the anastomotic site. Outcomes differ between primary and revisional settings. We describe our initial experience with MAGDI as a conversion procedure after sleeve gastrectomy. METHODS: Three consecutive patients underwent MAGDI after laparoscopic sleeve gastrectomy for insufficient clinical response or recurrent weight gain, following multidisciplinary evaluation. Procedures were laparoscopic, with magnetic docking 300 cm proximal to the il

Safety of early diverting loop ileostomy reversal after sigmoid colectomy with primary anastomosis for perforated diverticulitis.

BACKGROUND: Evidence increasingly supports sigmoidectomy with primary anastomosis (SPA) and diverting loop ileostomy over Hartmann's procedure for perforated diverticulitis in stable patients. Prompt diverting loop ileostomy reversal (DLIR) is often preferred by patients; however, optimal timing after index surgery remains unclear. The objective of this study is to examine the association of DLIR timing with clinical outcomes and costs. METHODS: Retrospective analysis was performed using National Readmissions Database (2010-2020) of all adults who underwent emergent sigmoidectomy with primary

Short- and Long-term Outcomes After Continent Ileostomy: An Often Forgotten Option for Patients After Proctocolectomy.

BACKGROUND: Continent ileostomy is an alternative for reconstruction after proctocolectomy when IPAA is not feasible. Due to technical difficulties and patient preference, few centers still perform these operations. Recent literature focuses on ileostomies that are as old as 40 years, and there is a dearth of information on newly constructed continent ileostomies. OBJECTIVE: To evaluate outcomes and patient satisfaction in a group of new continent ileostomy patients. DESIGN: Retrospective case series. SETTING: Urban academic center. PATIENTS: All patients undergoing continent ileostomy cre

Ileostomy or Ileal Pouch Anal Anastomosis: Shared Decision-Making in the Surgical Management of Ulcerative Colitis.

Up to 30% of patients with ulcerative colitis will ultimately require surgery. Generally, surgery for ulcerative colitis consists of total proctocolectomy with either end ileostomy or ileal pouch anal anastomosis, which may be performed in 1 to 3 stages. There are risks and benefits specific to undergoing permanent ileostomy creation or ileal pouch anal anastomosis, with each procedure resulting in different and dramatic changes in a patient's day-to-day life. Because most patients are eligible for either procedure, patient preferences are an important consideration when creating a treatment p

Protein-based oral rehydration solutions for patients with an ileostomy: A randomised, double-blinded crossover study.

BACKGROUND & AIM: Patients with an ileostomy are at increased risk of dehydration and sodium depletion. Treatments recommended may include oral rehydration solutions (ORS). We aimed to investigate if protein type or protein hydrolysation affects absorption from iso-osmolar ORS in patients with an ileostomy. METHODS: This was a randomised, double-blinded, active comparator-controlled 3 × 3 crossover intervention study. We developed three protein-based ORS with whey protein isolate, caseinate or whey protein hydrolysate. The solutions contained 40-48 g protein/L, 34-45 mmol sodium/L and had an
