Chronic alcohol consumption compromises gut barrier integrity and promotes endotoxemia: Implications for sepsis susceptibility in immunocompromised hosts.

BACKGROUND: Gut barrier integrity prevents microbial translocation and systemic infection. Chronic alcohol disrupts this barrier, but its role in infection susceptibility among immune-compromised hosts remains unclear. We investigated how chronic alcohol promotes gut barrier dysfunction, endotoxemia, and dysbiosis, predisposing to bacterial translocation and sepsis. METHODS: Twenty-four-week-old female FcγRIIb-/- and wild-type mice received oral gavage of 35% ethanol (4.2 g/kg/day) or water for 10 weeks. Gut barrier integrity was assessed by serum endotoxin, FITC-dextran permeability, ileal c

Extensive evolution and T cell escape by SARS-CoV-2 in a 2.5-year persistent infection of an immunocompromised host.

Prolonged virus-host interaction and suboptimal immunity during persistent SARS-CoV-2 infection of immunocompromised patients enables viral adaptation. We investigated viral evolution and immune escape during a 2.5-year persistent infection in a patient with multiple myeloma and rheumatoid arthritis receiving anti-CD20 therapy. Virus isolated 899-days post-infection revealed an ancestral B.31 lineage with extensive evolution (56 non-synonymous mutations across 20 viral proteins). Many mutations were private or convergent with those seen in other persistent infections and later variants. SARS-C

Oral wash PCR for the diagnosis of Pneumocystis jirovecii pneumonia in non-HIV immunocompromised hosts: a diagnostic accuracy study.

We assessed the Pneumocystis jirovecii PCR accuracy on non-invasive oropharyngeal wash (OW) specimens for the diagnosis of Pneumocystis jirovecii pneumonia (PJP) in non-HIV immunocompromised patients. In this retrospective study, 134 patients with suspected PJP undergoing both OW and deep respiratory sampling were included. PJP was defined by composite criteria. OW P. jirovecii PCR showed a sensitivity of 61.5% (95% confidence intervals [CI] 42.5-77.6) and a specificity of 97.2% (95% CI 92.2-99.1). Positive and negative predictive values were 84.2% and 91.3%, respectively. These results sugges

Nocardiosis in Immunocompromised Hosts: A Retrospective, Single-Center Analysis of Clinical and Microbiological Determinants of Central Nervous System Involvement.

BACKGROUND: Immunocompromised patients are at risk of disseminated nocardiosis, which can involve the central nervous system. The factors mediating dissemination to the central nervous system in this patient population have not been fully characterized. This study aimed to identify patient and pathogen-related variables associated with a higher risk of central nervous system infection among immunocompromised patients with nocardiosis. METHODS: Retrospective single-center analysis of all immunocompromised patients diagnosed with nocardiosis between 2013 and 2024. Patients without brain imaging

Isavuconazole as an optimal treatment option for multiple pathogens induced severe pneumonia in immunocompromised hosts: a case series report.

BACKGROUND: Treating severe pneumonia caused by multiple pathogens in immunocompromised hosts (ICHs) presents significant challenges. Isavuconazole (ISA), a next-generation triazole antifungal agent, has shown promise in managing fungal infections. However, clinical evidence regarding its efficacy in cases of complex infections involving multiple pathogens in ICHs remains limited. CASE PRESENTATION: This study describes a case series of three ICHs diagnosed with severe pneumonia, including invasive aspergillosis (IA). All three patients received ISA-based personalized antimicrobial regimens. …

Emergence of antibiotic resistance in immunocompromised host populations: A case study of emerging antibiotic resistant tuberculosis in AIDS patients.

OBJECTIVE: The evolution of antibiotic resistance is far outpacing the development of new antibiotics, causing global public health concern … e under conditions of both high and low HIV/AIDS prevalence. RESULTS: We demonstrate that HIV/AIDS-immunocompromised hosts can be responsible for a disproportionately greater contribution to emergence of resistance than would be expected based on population-wide HIV/AIDS prevalence alone. CONCLUSIONS: As such, the AIDS-immunocompromised have the potential become wells …

Characterization of Human Cytomegalovirus Genome Diversity in Immunocompromised Hosts by Whole-Genome Sequencing Directly From Clinical Specimens.

BACKGROUND: Advances in next-generation sequencing (NGS) technologies allow comprehensive studies of genetic diversity over the entire genome of human cytomegalovirus (HCMV), a significant pathogen for immunocompromised individuals. METHODS: Next-generation sequencing was performed on target enriched sequence libraries prepared directly from a variety of clinical specimens (blood, urine, breast milk, respiratory samples, biopsies, and vitreous humor) obtained longitudinally or from different anatomical compartments from 20 HCMV-infected patients (renal transplant recipients, stem cell transpl

Detection of (1, 3)-β-D-glucan in bronchoalveolar lavage and serum samples collected from immunocompromised hosts.

The incidence of invasive fungal infections (IFI) has increased in recent years, especially among immunocompromised hosts (ICH). In 2003, the Fungitell(®) assay received FDA clearance for the presumptive diagnosis of IFI using serum and detects (1-3)-β-D-glucan, which is a major cell wall component of certain fungi (e.g., Candida, Aspergillus, and Pneumocystis). The goal of the current study was to assess the performance of the assay on bronchoalveolar lavage (BAL) fluid and serum to identify IFI in ICH. Patients were classified as having proven, probable, possible, or no IFI according to the …

Prevention and treatment of influenza in high-risk groups: children, pregnant women, immunocompromised hosts, and nursing home residents.

The pediatric population experiences preventable hospitalizations and serves as a reservoir for influenza and its transmission to other children as well as adults. As a consequence, the Advisory Committee on Immunization Practices has recommended initiating influenza immunization of children as young as 6 months of age through 23 months of age and, recently, up to 5 years of age. However, immunization of older children has not yet become a priority of the US Public Health Service. As a consequence, the importance of antiviral agents, particularly neuraminidase (NA) inhibitors, cannot be overem

Multiple cavitating masses in an immunocompromised host with rheumatoid arthritis-related interstitial lung disease: an unusual expression of cytomegalovirus pneumonitis.

Cytomegalovirus is a known opportunistic infection in immunocompromised individuals. We document multiple cavitating lung masses caused by cytomegalovirus in a patient with rheumatoid arthritis-induced interstitial lung disease on immunosuppressant medication, an unusual expression of CMV pneumonitis. With increasing use of immunosuppressive therapy and increase in AIDS, pulmonary cytomegalovirus infection is expected to be a more frequently encountered disease and familiarity with its uncommon radiological manifestations will be advantageous.

Persistent SARS-CoV-2 Infection in an Immunocompromised Host Treated Successfully With the Japanese Herbal Medicine, Mao-to: A Case Report.

BACKGROUND Persistent COVID-19 in immunocompromised patients, such as those with B-cell depletion or hematologic malignancies, is an important clinical challenge. Clinically, cases that do not respond to guideline-based antivirals, such as molnupiravir, remdesivir, and nirmatrelvir/ritonavir, are occasionally encountered, but effective therapeutic alternatives remain scarce. We report a case of persistent severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection unresponsive to guideline-based antivirals, for which the traditional Japanese herbal medicine Mao-to (Ma-huang-tang) le

Recurrent Campylobacter jejuni bacteraemia with independent acquisition of carbapenem resistance in three ST9611 isolates: a case series in immunocompromised hosts.

OBJECTIVES: Campylobacter jejuni is a leading cause of bacterial gastroenteritis but rarely causes bacteraemia, except in immunocompromised hosts. Carbapenem resistance in C. jejuni remains uncommon, and the clinical evolution of resistance during infection has not been well described. METHODS: We report a case series of three immunocompromised adults with recurrent C. jejuni bacteraemia at a tertiary hospital in Singapore between 2024 and 2025. RESULTS: All three patients had underlying haematologic or autoimmune disorders associated with hypogammaglobulinaemia. Each experienced multiple ep…

Newer antibiotics for drug-resistant Gram-negative infections in immunocompromised hosts: from pivotal trials to high-risk practice.

Newer antibiotics have expanded treatment options for drug-resistant Gram-negative infections, but registration evidence is dominated by syndrome-based trials and post-approval evidence by heterogeneous observational cohorts. Translation to immunocompromised patients remains uncertain because profound neutropenia, transplantation, cell-targeted therapy, organ dysfunction, impaired source control, and limited immune-mediated clearance change both the probability and consequences of treatment failure. This critical narrative Review integrates 27 pivotal-trial protocol identifiers included in a p

A case of invasive Aspergillus niger spondylodiscitis with epidural abscess following COVID-19 infection in an immunocompromised host with literature review.

INTRODUCTION AND IMPORTANCE: Aspergillosis is defined as an opportunistic infection that may spread hematogenously. COVID-19 infection has not been reported as a direct cause or risk factor. Its treatment (e.g. corticosteroids) significantly increases the risk for invasive infections. The respiratory system remains the main target, and the Aspergillus fumigatus is the most responsible subtype. Other species like Aspergillus (A) flavus, A. niger, and A. nidulans follow in frequency. Other included sites are the skeletal muscular system and the entire spine leading to spondylodiscitis. Only a to

[Etiology of sepsis occurring in the immunocompromised host and its prevention. 1. Analysis of the bacterial portal of entrance in experimental mice with bacteremia].

Nosocomial infections due to aerobic gram-negative bacilli are life-threatening problems for immunocompromised patients. Most of these infections might arise from the patient's own flora, especially from their faecal flora. The purpose of this study was to investigate the bacterial portal of entry in experimental mice with bacteremia which were treated by cyclophosphamide (CY). It was clarified that almost all of the isolates from blood of CY and antibiotics treated mice were identical with their own faecal flora, and that organisms were isolated in some cases only from the portal vein but not
