Diabetes mellitus and immunity.

The present report discusses the immunogenic properties of insulin: the factors upon which immunogenicity depends, the characteristics of insulin antibodies, clinical aspects (insulin allergy, immunologic insulin resistance, lipodystrophy, etc.), as well as the significance of highly purified insulin products. The authors then review the role of autoimmunity in the etiopathogenesis of the diabetic disease as results from the association of diabetes with other autoimmune diseases, lymphocytic islet cell infiltration, phenomena of cell mediated and humoral mediated autoimmunity (islet cell antib

Immunity and morbidity in infection due to Schistosoma haematobium.

It has been suggested that immunologic factors, rather than transmission factors, may play a particularly important role in the regulation of schistosome populations. Support for this view comes from several studies of the epidemiology of Schistosoma haematobium infection, although evidence is accumulating from both field and laboratory studies that immunity in schistosomiasis need not be a concomitant immunity. Correlations between immunologic parameters such as peripheral blood eosinophil counts and specific anti-worm immunoglobulin E (IgE) antibody levels, and apparent resistance to reinfec…

Malignant lymphomas of the skin: their differentiation from lymphoid and nonlymphoid cutaneous infiltrates that simulate lymphoma.

Malignant lymphomas of the skin, excluding mycosis fungoides, are pathologically, immunologically, and clinically heterogeneous. Varying patterns and degrees of cutaneous infiltration are encountered in all histologic subtypes of non-Hodgkin's lymphomas. Immunologic studies have shown relatively equal numbers of cases with B and T cell phenotypes, but true histiocytic lymphomas of the skin also occur. Patients may be of any age, and they may have lymphoma in any clinical stage. A low clinical stage and a low-grade histologic subtype are significant factors for long survival. The differential d

Neuroimmunology of tics and other childhood hyperkinesias.

The etiology of tics and other childhood hyperkinesias is unclear despite attempts to link these movement disorders to neurotransmitter abnormalities and genes. This article will review the studies that suggest that some movement disorders are the result of immunologic factors.

[Risk factors for chronic allograft failure in renal transplant recipients].

OBJECTIVE: To identify the potential risk factors for the development of chronic renal allograft failure in renal transplant recipients. METHODS: The data of 235 renal transplant recipients followed up for over 1 year were observed prospectively. And their risk factors were identified with multivariate Logistic regression analysis. RESULTS: Forty-seven patients were diagnosed as chronic allograft failure (CAF). Multivariate risk factor analyses revealed that early (OR, 2.12; P < 0.01) and late (OR, 4.54; P < 0.001) acute rejection episodes, HLA-mismatch ( ≥ 3), HLA antibody production, viral

Deep neck infection.

Deep neck infections present significant morbidity and mortality, particularly when associated with predisposing factors that impair a functional immunologic response. Familiarity with deep neck spaces and fascial planes is critical, because these form the basis for the emergent nature of the disease process. Common and potentially life-threatening complications include airway obstruction, jugular vein thrombosis, descending mediastinitis, sepsis, acute respiratory distress syndrome, and disseminated intravascular coagulation. The most common primary sources of deep neck infection are odontoge

Anergy in tuberculosis.

Anergy in tuberculosis is of considerable clinical and immunological interest. Although negative skin reactions may be secondary to improper tuberculin testing or to certain diseases, drugs, vaccinations and constitutional factors known to affect cell-mediated immunity, often no underlying explanation is apparent. Unexplained negative reactions occur with a highly variable frequency in a wide variety of tuberculous infections. Anergy in tuberculosis is usually generalized, is often accompanied by other immunologic abnormalities, and frequently disappears as the disease is treated. Most anergic

Factors that preclude the conclusion of a kidney transplant protocol from a living donor.

INTRODUCTION: A kidney transplant (KT) is a renal replacement therapy that offers greater survival and quality of life; however, it is not always successfully completed due to social, physical, medical, immunological factors, or lack of adherence. METHODS: This retrospective, analytical, longitudinal study included patients in the KT protocol from a living donor between May 2021 and November 2022. Factors preventing completion were evaluated. Comparative analyses and cumulative incidence curves were used to assess time to protocol completion or discontinuation. The analysis was performed usin

Autoimmune hepatitis in childhood: the role of genetic and immune factors.

Autoimmune hepatitis (AIH) is a rare chronic inflammatory disease of the liver, which affects a group of patients who lost their immunological tolerance to antigens of the liver. It is clinically characterized by hypergammaglobulinemia, elevated liver enzymes, presence of autoantibodies and histological changes. Although being rare in children, it represents a serious cause of chronic hepatic disease that can lead to cirrhosis and hepatic failure. Clinical findings, exclusion of more common liver disorders and the detection of antibodies antinuclear antibodies, smooth muscle antibodies and ant

[Immunologic risk analysis of blood transfusion: 1991-1998].

The immunologic risk associated to erythrocyte transfusions is bound to the polymorphism of blood group systems and to the respect of blood transfusion regulations. The results of three studies are presented, which were carried out respectively by the French Society of Blood Transfusion, the National Institute of Blood Transfusion and the National Haemovigilance Network. Two hundred and twenty-seven cases of immunologic accidents are analysed using the Kaplan's interpretation model and the traditional method of process analysis. The results show three critical factors in the occurrence of this

Recurrent parotitis and sialectasis in childhood. Clinical, radiologic, immunologic, bacteriologic, and histologic study.

Twenty children with juvenile recurrent parotitis, between 3 months and 16 years of age at onset, were followed up over a period of 7 to 22 years. Radiologic, histopathologic, cytologic, immunologic, and bacteriologic studies were performed to investigate the cause of sialectasis, commonly found in juvenile recurrent parotitis, and the pathogenesis of the disease. It was considered that a combination of a congenital malformation of portions of the salivary ducts and infections ascending from the mouth following dehydration of the children are contributory to the pathogenesis of the disease. Immunologic reactions associated with anemia, thrombocytopenia, and coagulopathy in experimental African trypanosomiasis.

Rats infected with Trypanosoma brucei rhodesiense developed anemia, thrombocytopenia, and hypocomplementemia. Anemia, thrombocytopenia, and sharp reductions in parasitemia were associated with elevated titers of cold-active hemagglutinin, antibody to fibrinogen/fibrin-related products, and immunoconglutinin. Depletion of lytic complement, prolonged partial thromboplastin times, and presence of fibrin monomers in the blood occurred at the time anemia and significant elevations in precipitable immune complexes were observed. Terminally, consumption of immunologic factors coincided with accelerat…

Recurrent mucocutaneous lymph node syndrome (Kawasaki disease): a report of three cases.

Three children are reported who each had two documented febrile episodes, compatible with the diagnostic criteria of mucocutaneous lymph node syndrome (Kawasaki disease). The recurrence of this syndrome in these three children, sporadically, may indicate that Kawasaki disease is related to host immunologic factors.

Optimal monitoring strategies for guiding when to switch first-line antiretroviral therapy regimens for treatment failure in adults and adolescents living with HIV in low-resource settings.

BACKGROUND: One of the critical clinical decisions made in antiretroviral therapy (ART) is when to switch from an initial regimen to another due to treatment failure. This complex process requires consideration of multiple factors including: (1) what type of monitoring (e.g. clinical, immunologic, virologic) is available to guide switching; (2) establishing criteria for treatment failure (e.g. viral load >10,000 copies/mL); (3) integrating data from different types of monitoring; (4) making a decision; and, if possible, (5) follow-up and monitoring to determine patient outcomes. The initial st

[Predisposing and triggering factors in erysipelas].

The authors studied 29 patients with erysipela and considered the immunologic deficit and the filariasis as the principal predisponents factors while local traumatisms and regional superficial mycosis were considered as the most frequent unleashing agents. After treatment of these factors the authors did a follow-up by 1 1/2 year and only 2 patients presented recurrence of the disease in coincidence with recurrence of their feet mycosis.
