The influence of cost on avoidance recovery after fear extinction with response prevention.

Eliminating avoidance responses is crucial for the treatment of anxiety disorders. However, previous experimental research indicates that conventional therapies, such as exposure with response prevention, may fail to fully eradicate avoidance when it does not entail a cost. This is evident in studies where individuals, after completing an extinction with response prevention (ERP) phase, are given the opportunity to avoid again. This study investigated the impact of introducing a cost for avoidance on the persistence of avoidance responses after an ERP phase. After fear conditioning, participan

Prolonged exposure in routine outpatient care: a naturalistic study of feasibility, safety and effectiveness.

Background: Prolonged Exposure (PE) is a first-line evidence-based treatment for Posttraumatic Stress Disorder (PTSD), with well-established efficacy in randomised controlled trials. However, its implementation in routine care remains limited, particularly in outpatient clinics where evidence-based psychotherapies are not widely adopted. Objective: This study aimed to evaluate the feasibility, safety and effectiveness of PE therapy delivered in a real-world outpatient setting.Method: Data were obtained from 95 adults diagnosed with PTSD using the Clinician-Administered PTSD Scale for DSM-5 (CAP

Predicting treatment outcome for complex posttraumatic stress disorder using the personalized advantage index.

ABSTRACTBackground: Ample studies have demonstrated the effectiveness of psychotherapy for posttraumatic stress disorder (PTSD). However, large individual variation in treatment outcome remains unsolved and treatment options for complex posttraumatic stress disorder (CPTSD) are debated. There is a need for exploring methods for matching patients with treatment they will most likely benefit from. Objective: To develop a personalized advantage index (PAI) based on relevant clinical and demographic predictors of outcome from exposure therapy and skills-training for CPTSD.Method: Data from a previo

Co-existence of Capgras syndrome and OCD in children: Case studies.

IntroductionCapgras syndrome (CS) is characterized by the delusional belief that a person, usually a close relative, has been replaced by an imposter. This study focuses on the co-occurrence of CS and Obsessive-Compulsive Disorder (OCD) in children.MethodsWe present two cases of children diagnosed with CS and OCD treated at our inpatient child psychiatric unit.Cases presentationWe describe the cases of an 11-year-old male who believed his parents were demonic entities and a 12-year-old female who believed her parents were robots. Both children exhibited additional psychiatric manifestations su

Exposure-based treatment for co-occurring posttraumatic stress disorder and obsessive compulsive disorder in veterans: The feasibility of massed models.

Exposure-based therapies are widely accepted as the gold-standard intervention for both obsessive compulsive disorder (OCD) and posttraumatic stress disorder (PTSD). Despite their co-occurrence, little research has explored exposure-based treatment in individuals who experience both OCD and PTSD. At an academic medical center mental health program, four different treatment approaches combining exposure and response prevention (ERP) and prolonged exposure (PE) therapies were piloted for veterans with co-occurring OCD and PTSD. We present each approach with a brief case study. Two sequential mod

Effectiveness of intensive exposure therapy for persistent post-concussion symptoms: an aggregated single-case design approach.

The fear-avoidance model suggests that catastrophising and avoidance behaviours can maintain persistent post-concussion symptoms (PPCS) after mild traumatic brain injury (mTBI). Although exposure therapy has shown preliminary success in this population, its effectiveness in mTBI remains unproven. This study investigated the therapeutic effects of this therapy across different settings. Twenty participants with PPCS took part in five concurrent multiple-baseline, multi-phase single-case experimental design studies. Phases included: baseline (A), exploration (B), active exposure (C), booster (D)

Implementation and Preliminary Outcomes of an Exposure-Based Summer Camp for Pediatric OCD and Anxiety.

Despite the high prevalence of anxiety disorders in children and adolescents and the existence of effective evidence-based treatments for them, access to psychological care remains a major public health concern. Summer camps may provide an effective treatment avenue for youth who might not otherwise have access to care. This study describes the design and implementation of Fear Facers, a semistructured, 5-day, daytime exposure-therapy-based summer camp designed for youth with a primary diagnosis of obsessive-compulsive disorder (OCD), social anxiety, separation anxiety, or a specific phobia. The association between fear extinction, the ability to accomplish exposure and exposure therapy outcome in specific phobia.

Great interest exists in maximizing exposure therapy efficacy in anxiety disorders. At the same time, reduced frequency and shortened duration of exposure sessions are required to meet the specific regularities in routine care settings. Extinction has emerged as the key mechanism of exposure treatment in anxiety disorders. Examining exposure treatment processes from the perspective of extinction learning might provide novel insights into variability in exposure treatment duration and outcome. The present study sought to examine the functional link between fear extinction, the ability to accomp

Exposure and response prevention therapy augmented with naltrexone in kleptomania: a controlled case study using galvanic skin response for monitoring.

BACKGROUND: Kleptomania is a disease that shares features with obsessive compulsive spectrum disorders (OCD) and with substance abuse disorders (SAD). This is underlined by therapeutic approaches in kleptomania ranging from cognitive behavioural therapy and selective serotonin reuptake inhibitors that are effective in OCD, and opioid antagonists that are currently being used in SAD. However, almost no literature exists about exposure and response prevention (ERP) therapy in kleptomania. Furthermore, there is a clear lack of objective markers that would allow a therapeutic monitoring. AIM: To

Does Exposure and Response Prevention Behaviorally Activate Patients With Obsessive-Compulsive Disorder? A Preliminary Test.

Exposure and response prevention (ERP) and behavioral activation (BA) are effective treatments for obsessive-compulsive disorder (OCD) and depression, respectively. Patients with OCD often exhibit depression; furthermore, ERP for OCD is associated with reduced depressive symptoms. To our knowledge, no study has examined whether ERP itself functions to behaviorally activate patients with concurrent OCD and depressive symptoms. This prospective study was designed to test the hypotheses that (a) OCD exposure hierarchy completion, increased BA, and depressive symptom reduction would all be related

Exposure therapy for social anxiety disorder in people who stutter: An exploratory multiple baseline design.

BACKGROUND & OBJECTIVES: Social anxiety disorder (SAD) is a debilitating condition, and approximately half of adults who stutter have SAD. Cognitive-behavioral therapy (CBT) has shown promise in decreasing social anxiety symptoms among adults who stutter, but exposure, arguably the essential component for successful CBT for SAD, has been understudied and underemphasized. Aims of this study were to develop an exposure therapy protocol designed specifically for people who stutter and have SAD and evaluate its potential efficacy in reducing social anxiety and stuttering severity using a multiple

The unique and conditional effects of interoceptive exposure in the treatment of anxiety: A functional analysis.

Interoceptive exposure (IE; exposure focused on anxiety about somatic sensations) is a well-established component of treatments for panic disorder (PD), but little is known about the specificity of its effects or individual response patterns resulting from this intervention. This study investigated the utility of IE in the treatment of PD with claustrophobia, examining its mechanisms in isolation and in combination with situational exposure. Ten adults with PD and claustrophobia were treated with a flexible single-case approach. Participants received up to 6 sessions of IE; nonresponders recei

Perceived burdensomeness and suicide ideation in adult outpatients receiving exposure therapy for anxiety disorders.

Perceived burdensomeness is considered a proximal risk factor for suicide ideation. However, there is a lack of prospective studies. Furthermore, it is unclear in as much psychotherapy for anxiety disorders is associated with a decrease in suicide ideation. A total of 105 adult outpatients suffering from panic disorder, agoraphobia, or specific phobia received manualized exposure-therapy. Perceived burdensomeness was considered as predictor of suicide ideation concurrently, after the fourth and the tenth therapy session and posttreatment - controlling for baseline symptom distress, suicide ide

Automatic mechanisms for measuring subjective unit of discomfort.

Current practice in Virtual Reality Exposure Therapy (VRET) is that therapists ask patients about their anxiety level by means of the Subjective Unit of Discomfort (SUD) scale. With an aim of developing a home-based VRET system, this measurement ideally should be done using speech technology. In a VRET system for social phobia with scripted avatar-patient dialogues, the timing of asking patients to give their SUD score becomes relevant. This study examined three timing mechanisms: (1) dialogue dependent (i.e. naturally in the flow of the dialogue); (2) speech dependent (i.e. when both patient

Session-level effects of prolonged exposure therapy with and without topiramate in veterans with posttraumatic stress disorder and alcohol use disorder.

Posttraumatic stress disorder (PTSD) and alcohol use disorder (AUD) commonly co-occur and are associated with worse outcomes and treatment engagement than either disorder alone. Research and clinical practice guidelines support treating PTSD + AUD concurrently, with the combination of trauma-focused psychotherapy and pharmacotherapy demonstrating promise. Topiramate (TOP) can reduce AUD symptoms with emerging evidence that it may also attenuate PTSD symptoms. In a recent randomized controlled trial, veterans (N = 100) receiving prolonged exposure (PE) with TOP (PE + TOP) demonstrated greater r

Treatment expectancy and credibility as predictors of concentrated exposure treatment outcomes in patients with difficult-to-treat obsessive-compulsive disorder.

BACKGROUND: Treatment readiness factors, such as treatment credibility and expectancy, are postulated to be predictors of outcomes within the context of cognitive behavioral therapy (CBT). Concentrated exposure therapy (cET) is a form of short-term, intensive, exposure-based CBT that has shown promising results. This study investigated whether treatment expectancy and credibility predict cET treatment outcomes in patients with difficult-to-treat (nonresponders and patients with relapse following CBT) obsessive-compulsive disorder (OCD). METHODS: A total of 163 patients underwent 4 days of cET

Virtual reality-based cue exposure therapy reduces psychological craving in men with methamphetamine use disorder: a randomized controlled trial.

Reducing psychological craving is critical for preventing relapse in methamphetamine use disorder (MUD). This study aimed to evaluate the efficacy of virtual reality (VR)-based cue exposure therapy (CET) and cue exposure with aversion therapy (CETA) in reducing methamphetamine craving in men with MUD. In this randomized controlled trial, 89 men with MUD were assigned to three groups: VR-based cue exposure therapy (CET, n = 30), VR-based cue exposure combined with aversion therapy (CETA, n = 29), and neutral scenes (NS, n = 30). The intervention comprised 16 sessions over 8 weeks. Primary outco

Efficacy of exposure scenario in virtual reality for the treatment of acrophobia: A randomized controlled trial.

BACKGROUND AND OBJECTIVES: Individuals with acrophobia (fear of heights) can experience severe anxiety or panic attacks when they are located at height. This randomized controlled study aimed to verify the effects of a novel scalable virtual reality-based exposure (VR exposure) tool in individuals with acrophobia, by exposing them to a predefined set of situations they usually tend to avoid. METHODS: Forty-three adults were randomly assigned to one of the two groups: the experimental group or the waitlist group. Both groups attended initial short online education. The experimental group conse

Delivery of Exposure and Response Prevention Among Veterans with Obsessive-Compulsive Disorder.

Veterans with obsessive-compulsive disorder (OCD) often face barriers to receiving evidence-based treatments such as exposure and response prevention (ERP). Through retrospective review of electronic medical records, this study examined the rates of ERP delivery in a national sample of 554 veterans newly diagnosed with OCD in the Veterans Health Administration between 2016 and 2017. Results indicated that only 4% of veterans (n = 22) received any ERP treatment; and, of those, 16 veterans received "true ERP." Veterans who received any ERP were younger than those who did not. ERP was primarily d
