Investigating the Decline of Fetal and Infant Mortality Rates in Alaska During 2010 and 2011.

INTRODUCTION: The U.S. infant mortality rate has been steadily declining since 2007. Although the downward trend has been notable in Alaska since 2006 when the rate was 6.9 infant deaths per 1000 live births, a dramatic drop in infant mortality occurred in 2010 and 2011 when the infant mortality rate fell to 3.8 infant deaths per 1000 live births during both years. The purpose of this study was to investigate the sudden decrease in fetal and infant mortality rates (FIMR) using the perinatal periods of risk (PPOR) method, an approach that has not been used previously in Alaska. METHODS: The st…

Association of arsenic with adverse pregnancy outcomes/infant mortality: a systematic review and meta-analysis.

BACKGROUND: Exposure to arsenic is one of the major global health problems, affecting > 300 million people worldwide, but arsenic's effects on human reproduction are uncertain. OBJECTIVES: We conducted a systematic review and meta-analysis to examine the association between arsenic and adverse pregnancy outcomes/infant mortality. METHODS: We searched PubMed and Ovid MEDLINE (from 1946 through July 2013) and EMBASE (from 1988 through July 2013) databases and the reference lists of reviews and relevant articles. Studies satisfying our a priori eligibility criteria were evaluated independently …

Socioeconomic position, health behaviors, and racial disparities in cause-specific infant mortality in Michigan, USA.

OBJECTIVES: Studies about racial disparities in infant mortality suggest that racial differences in socioeconomic position (SEP) and maternal risk behaviors explain some, but not all, excess infant mortality among Blacks relative to non-Hispanic Whites. We examined the contribution of these to disparities in specific causes of infant mortality. METHODS: We analyzed data about 2,087,191 mother-child dyads in Michigan between 1989 and 2005. First, we calculated crude Black-White infant mortality ratios independently and by specific cause of death. Second, we fit multivariable Poisson regression…

Obstructive sleep apnea and severe maternal-infant morbidity/mortality in the United States, 1998-2009.

STUDY OBJECTIVES: A recent trend in increasing rates of severe maternal morbidity and mortality despite quality improvements has been noted. The goal of this study is to estimate the national prevalence of obstructive sleep apnea (OSA) in pregnant women and examine associations between OSA and pregnancy-related morbidities, including in-hospital maternal mortality. DESIGN: A retrospective, cross-sectional analysis. SETTING: A nationally representative sample of maternal hospital discharges from 1998-2009. PATIENTS OR PARTICIPANTS: The analytic sample included 55,781,965 pregnancy-related in

Application of a global reference for fetal-weight and birthweight percentiles in predicting infant mortality.

OBJECTIVE: To determine whether the recently published A global reference for fetal-weight and birthweight percentiles (Global Reference) improves small- (SGA), appropriate- (AGA), and large-for-gestational-age (LGA) definitions in predicting infant mortality. DESIGN: Population-based cohort study. SETTING: The US Linked Livebirth and Infant Death records between 1995 and 2004. POPULATION: Singleton births with birthweight >500 g born at 24-41 weeks of gestation. METHODS: We compared infant mortality rates of SGA, AGA, and LGA infants classified by three different references: the Global Re…

Pregnancy loss and role of infant HIV status on perinatal mortality among HIV-infected women.

BACKGROUND: HIV-infected women, particularly those with advanced disease, may have higher rates of pregnancy loss (miscarriage and stillbirth) and neonatal mortality than uninfected women. Here we examine risk factors for these adverse pregnancy outcomes in a cohort of HIV-infected women in Zambia considering the impact of infant HIV status. METHODS: A total of 1229 HIV-infected pregnant women were enrolled (2001-2004) in Lusaka, Zambia and followed to pregnancy outcome. Live-born infants were tested for HIV by PCR at birth, 1 week and 5 weeks. Obstetric and neonatal data were collected after

Geographical variation in infant mortality, stillbirth and low birth weight in Northern Ireland, 1992-2002.

BACKGROUND: Improving the health of expectant mothers and reductions in health inequalities, are repeatedly prioritised in policy reports in England and Northern Ireland. Measurement of underlying rates, and geographical variation in rates, of adverse birth outcomes are tools in monitoring these priorities. METHODS: Northern Ireland data on stillbirths, infant mortality and low birth weight (1992-2002) were linked to board (n=4), district council (n=26) and 1991 census wards (n=568). Underlying variations in rates were estimated at each geographical level, unadjusted and controlling for year,…

Income inequality, residential poverty clustering and infant mortality: a study in Rio de Janeiro, Brazil.

In this paper, we propose an approach to investigate the hypothesis that the residential concentration of poverty affects health status more … tration across census tracts within a neighborhood. The effects of geographic poverty clustering on infant mortality related variables (early neonatal mortality rate; post-neonatal mortality rate; proportion of adolescent mothers; and fertility rate among adolescents) were estimated by partial correlation coefficients, controlling for the neighborhood poverty rate …

Effects of malaria during pregnancy on infant mortality in an area of low malaria transmission.

Malaria during pregnancy reduces birth weight, and low birth weight is a major determinant of infant mortality. The authors estimated the impact of malaria during pregnancy on infant mortality in a Karen population living in Thailand. Between 1993 and 1996, a cohort of 1,495 mothers and their infants was followed weekly from admission of the mother to antenatal clinics until the first birthday of the infant. Both falciparum malaria and vivax malaria during pregnancy were associated with low birth weight but did not shorten gestation. Febrile illness in the week before delivery was associated w…

Baby Sleep Project Protocol: a realist evaluation of an intervention to reduce preventable infant mortality.

INTRODUCTION: In the UK, approximately 300 infants each year die suddenly and unexpectedly, with most deaths remaining unexplained. Population-wide 'Safer Sleep' messages have brought rates down but remaining deaths now predominantly occur within families experiencing poverty. Many of these deaths may be preventable as the majority have known, avoidable risks present. New resources and tools for health professionals and families have been developed to address modifiable barriers to implementing safer sleep advice in this priority group of families. This study aims to understand how the Baby Sl

Troponin I, CK-MB, and inotropic score in hypoxic-ischemic encephalopathy and associated infant mortality.

PURPOSE: Cardiovascular dysfunction is common in hypoxic-ischemic encephalopathy (HIE), which is one of the leading causes of multi-organ failure in neonates. We aimed to assess troponin I and creatine kinase myocardial band (CK-MB) levels, inotropic score (IS) in HIE patients, and their associations with HIE staging and mortality. METHOD: The medical records of all HIE infants admitted to our unit between 2016 and 2018 were retrospectively analyzed. Demographic characteristics of the infants, seizures, anticonvulsive therapies, maximum inotrope doses, and the derived IS (dopamine dose [µg/kg

Inequality of opportunity in infant mortality in South Asia: A decomposition analysis of survival data.

Early-life environments into which newborn babies are born play principal roles in their development. This study explores inequalities in infant mortality that are rooted in household and parental socio-economic backgrounds in five South-Asian countries: Afghanistan, Bangladesh, India, Nepal and Pakistan. Considering multidimensional aspects of socio-demographic and socio-economic status, this study explores disparities in the trajectory of survival rates across infants with dissimilar circumstantial backgrounds over the first 12 months of their lives. This study proposes a new method to first…

Geographic distribution of live births and infant mortality from congenital anomalies in Brazil, 2012-2017.

In the 2010-2014 period, the mean prevalence of congenital anomalies (CA) in the world was estimated at 398/10,000 births. CA are an important cause of mortality, disability, and comorbidity. Thus, the present study aims to describe the geographical and temporal distributions of live births and infant mortality (IM) due CA (IM-CA) in Brazil, from 2012 to 2017. The data used in this study is available at the Department of Informatics of the Unified Health System (DATASUS). The prevalence of CA at birth was 81.67/10,000 (95% CI 80.46-82.88), and the IM-CA rate was 27.97/10,000 (95% CI 27.95-28.0…

Effect of Concomitant Birth Defects and Genetic Anomalies on Infant Mortality in Tetralogy of Fallot.

BACKGROUND: A substantial proportion of infants born with tetralogy of Fallot (TOF) die in infancy. A better understanding of the heterogeneity associated with TOF, including extracardiac malformations and chromosomal anomalies is vital to stratifying risk and optimizing outcomes during infancy. METHODS: Using the North Carolina Birth Defects Monitoring Program, infants diagnosed with TOF and born between 2003 and 2012 were included. Kaplan-Meier survival curves were used to estimate cumulative 1-year mortality, stratified by the presence of concomitant birth defects (BDs) and chromosomal ano

Territorial differences in infant mortality in Latvia in the first decade of the third millennium.

OBJECTIVE: Infant and child mortality are some of the most substantial indicators of country welfare. Infant mortality (IM) in Latvia is constantly the highest among 25 Member States of the European Union. Since the regaining of independence in 1991, IM has decreased by almost 50%, however, it is still high enough to cause concern that the country will not be able to meet the UN Millennium Development Goals to decrease IM in Latvia by 2015. The Medical Faculty at the University of Latvia has conducted several studies identifying correlations between IM and GDP, total expenditure on health, une…
