Prehospital activation of a coordinated multidisciplinary hospital response in preparation for patients with severe hemorrhage: A statewide data linkage study of the New South Wales "Code Crimson" pathway.

BACKGROUND: Hemorrhage is a leading cause of preventable death in trauma. Prehospital medical teams can streamline access to massive transfusion and definitive hemorrhage control by alerting in-hospital trauma teams of suspected life-threatening bleeding in unstable patients. This study reports the initial experience of an Australian "Code Crimson" (CC) pathway facilitating early multidisciplinary care for these patients. METHODS: This data-linkage study combined prehospital databases with a trauma registry of patients with an Injury Severity Score greater than 12 between 2017 and 2019. Four

Unsupervised learning for large-scale corneal topography clustering.

Machine learning algorithms have recently shown their precision and potential in many different use cases and fields of medicine. Most of the algorithms used are supervised and need a large quantity of labeled data to achieve high accuracy. Also, most applications of machine learning in medicine are attempts to mimic or exceed human diagnostic capabilities but little work has been done to show the power of these algorithms to help collect and pre-process a large amount of data. In this study we show how unsupervised learning can extract and sort usable data from large unlabeled datasets with m

The Application of Mobile Telehealth System to Facilitate Patient Information Presentation and Case Discussion.

Background:Most secondary transmission of COVID-19 is occurring in a hospital setting. To decrease person-to-person contact, health care providers have built many isolation wards. However, out-of-hospital professionals cannot access patient information, which has greatly reduced the efficiency of treatment; it is inconvenient for health care professionals to issue a case discussion with professionals from other wards. This article mainly introduces a mobile telehealth system (MTS) applied to facilitate patient information presentation and case discussion.Materials and Methods:The MTS searches

Linking household surveys and health facility assessments to estimate intervention coverage for the Lives Saved Tool (LiST).

BACKGROUND: Calls have been made for improved measurement of coverage for maternal, newborn and child health interventions. Recently, methods linking household and health facility surveys have been used to improve estimation of intervention coverage. However, linking methods rely the availability of household and health facility surveys which are temporally matched. Because nationally representative health facility assessments are not yet routinely conducted in many low and middle income countries, estimates of intervention coverage based on linking methods can be produced for only a subset of

A systems neurophysiology approach to voluntary event coding.

Mechanisms responsible for the integration of perceptual events and appropriate actions (sensorimotor processes) have been subject to intense research. Different theoretical frameworks have been put forward with the "Theory of Event Coding (TEC)" being one of the most influential. In the current study, we focus on the concept of 'event files' within TEC and examine what sub-processes being dissociable by means of cognitive-neurophysiological methods are involved in voluntary event coding. This was combined with EEG source localization. We also introduce reward manipulations to delineate the ne

Work-related illness and injury claims among nationally certified athletic trainers reported to Washington and California from 2001 to 2011.

BACKGROUND: Little is known about the work-related injury and illnesses experienced by certified athletic trainers (AT). METHODS: The incidence and characteristics of injury/illness claims filed in two workers' compensation systems were described from 2001 to 2011. Yearly populations at risk were estimated from National Athletic Trainers' Association membership statistics. Incidence rate ratios (IRR) were reported by job setting. RESULTS: Claims were predominantly for traumatic injuries and disorders (82.7%: 45.7% sprains/strains, 12.0% open wounds, 6.5% bruises) and at these body sites (bac

Roles of nonclinical and clinical data in prediction of 30-day rehospitalization or death among heart failure patients.

BACKGROUND: Selecting heart failure (HF) patients for intensive management to reduce readmissions requires effective targeting. However, available prediction scores are only modestly effective. We sought to develop a prediction score for 30-day all-cause rehospitalization or death in HF with the use of nonclinical and clinical data. METHODS AND RESULTS: This statewide data linkage included all patients who survived their 1st HF admission (with either reduced or preserved ejection fraction) to a Tasmanian public hospital during 2009-2012. Nonclinical data (n = 1,537; 49.5% men, median age 80 y

Detective work in drug-induced liver injury: sometimes it is all about interviewing the right witness.

Diagnosing drug-induced liver injury (DILI) relies primarily on history taking. We report 4 cases in which DILI was missed or the drug was misidentified when physicians relied solely on patient history. We reviewed 27 cases referred with possible DILI from August 1, 2009, to February 1, 2010. Four patients seemed to be reliable historians, but their cases were greatly clarified by a call to their pharmacist. One subject, who forgot a new medication, underwent an unfruitful evaluation including surgery. Another patient had acetaminophen toxicity that was missed because she grossly underreported

Exploiting semantic relations for literature-based discovery.

We propose using semantic predications to enhance literature-based discovery (LBD) systems, which currently depend exclusively on co-occurrence of words or concepts in target documents. In this paper, the predications, which are produced by the combined application of two natural language processing systems, BioMedLEE and SemRep, are coupled with an LBD system BITOLA. Initial experiments suggest this approach can uncover new associations that were not possible using previous methods.

Structure from planar motion.

Planar motion is arguably the most dominant type of motion in surveillance videos. The constraints on motion lead to a simplified factorization method for structure from planar motion when using a stationary perspective camera. Compared with methods for general motion, our approach has two major advantages: a measurement matrix that fully exploits the motion constraints is formed such that the new measurement matrix has a rank of at most 3, instead of 4; the measurement matrix needs similar scalings, but the estimation of fundamental matrices or epipoles is not needed. Experimental results sho

Cerebellar-parietal connections underpin phonological storage.

Previous research has accumulated convincing evidence to show that the human cerebellum contributes to the short-term storage of verbal information, but its specific role in brain networks involved in phonological storage remains uncertain. In a randomized, crossover and sham-controlled design, we here combined transcranial direct current stimulation (tDCS), applied to the right cerebellum, with fMRI to investigate systematically the contribution of the human cerebellum to encoding, maintenance, and retrieval of verbal information. After anodal, but not cathodal, tDCS, we found a reduced item

Revising the ECRIN standard requirements for information technology and data management in clinical trials.

The pilot phase of the ECRIN (European Clinical Research Infrastructure Network) certification programme for European data centres, in late 2011, led to a substantial revision of the original ECRIN standards, completed by June 2012. The pilot phase, the conclusions drawn from it and the revised set of standards are described. Issues concerning the further development of standards and related material are discussed, as are the methods available to best support that development. A strategy is outlined based on short-lived specific task groups, established as necessary by a steering group drawn f

A Severe Acute Respiratory Syndrome Extranet: supporting local communication and information dissemination.

BACKGROUND: The objective of this study was to explore the use and perceptions of a local Severe Acute Respiratory Syndrome (SARS) Extranet and its potential to support future information and communication applications. The SARS Extranet was a single, managed electronic and limited access system to manage local, provincial and other SARS control information. METHODS: During July, 2003, a web-based and paper-based survey was conducted with 53 SARS Steering Committee members in Hamilton. It assessed the use and perceptions of the Extranet that had been built to support the committee during the

Auditory and visual memory span: cognitive processing by TMR individuals with Down syndrome or other etiologies.

Auditory and visual memory span for letters and component memory processes of TMR young adults with Down syndrome or other etiologies was examined. Component processes of the span task included long-term memory access for labels for the stimuli and memory for the order in which the stimuli were presented. Results indicated that although all subjects had relatively poor auditory memory spans, the Down syndrome group was especially poor at long-term memory access for stimulus identification and at short-term storage and processing of auditory information. Lexical storage and retrieval deficienci

Birthweight, gestational age and familial confounding in sex differences in infant mortality: a matched co-twin control study of Brazilian male-female twin pairs identified by population data linkage.

BACKGROUND: In infancy, males are at higher risk of dying than females. Birthweight and gestational age are potential confounders or mediators but are also familial and correlated, posing epidemiological challenges that can be addressed by studying male-female twin pairs. METHODS: We studied 28 558 male-female twin pairs born in Brazil between 2012 and 2016, by linking their birth and death records. Using a co-twin control study matched for gestational age and familial factors, we applied logistic regression with random effects (to account for paired data) to study the association between mal

Toward a systematic conflict resolution framework for ontologies.

BACKGROUND: The ontology authoring step in ontology development involves having to make choices about what subject domain knowledge to include. This may concern sorting out ontological differences and making choices between conflicting axioms due to limitations in the logic or the subject domain semantics. Examples are dealing with different foundational ontologies in ontology alignment and OWL 2 DL's transitive object property versus a qualified cardinality constraint. Such conflicts have to be resolved somehow. However, only isolated and fragmented guidance for doing so is available, which t

DLAD4U: deriving and prioritizing disease lists from PubMed literature.

BACKGROUND: Due to recent technology advancements, disease related knowledge is growing rapidly. It becomes nontrivial to go through all published literature to identify associations between human diseases and genetic, environmental, and life style factors, disease symptoms, and treatment strategies. Here we report DLAD4U (Disease List Automatically Derived For You), an efficient, accurate and easy-to-use disease search engine based on PubMed literature. RESULTS: DLAD4U uses the eSearch and eFetch APIs from the National Center for Biotechnology Information (NCBI) to find publications related

Disease comorbidity-guided drug repositioning: a case study in schizophrenia.

UNLABELLED: The key to any computational drug repositioning is the availability of relevant data in machine-understandable format. While large amount of genetic, genomic and chemical data are publicly available, large-scale higher-level disease and drug phenotypic data are limited. We recently constructed a large-scale disease-comorbidity relationship knowledge base (dCombKB) and a comprehensive drug-treatment relationship knowledge base (TreatKB) from 21 million biomedical research articles and other resources. In this study, we demonstrated the potential of dCombKB and TreatKB in drug reposi

Identification of Patients with Family History of Pancreatic Cancer--Investigation of an NLP System Portability.

In this study we have developed a rule-based natural language processing (NLP) system to identify patients with family history of pancreatic cancer. The algorithm was developed in a Unstructured Information Management Architecture (UIMA) framework and consisted of section segmentation, relation discovery, and negation detection. The system was evaluated on data from two institutions. The family history identification precision was consistent across the institutions shifting from 88.9% on Indiana University (IU) dataset to 87.8% on Mayo Clinic dataset. Customizing the algorithm on the the Mayo
