The contribution of oncological lateral skull base surgery to the management of advanced head-neck tumors.

BACKGROUND: Lateral skull base is a complex area between the brain and the neck that is characterized by a large anatomic variability in nar … ase. It is also conceived for selected aggressive or benign lesions of the parapharyngeal space and infratemporal fossa abutting the skull base, or crossing it from above downwards to the neck. This paper is focused on the role that oncological skull base surgery plays to resect tumors in this area. METHODS AND RESULTS: Three main types of head and neck lesions can …

Intracranial actinomycosis of odontogenic origin masquerading as auto-immune orbital myositis: a fatal case and review of the literature.

BACKGROUND: Actinomycetes can rarely cause intracranial infection and may cause a variety of complications. We describe a fatal case of intr … itis with septic thromboembolism. Retrospectively, imaging studies showed abnormalities in the left infratemporal fossa and skull base and bilateral cavernous sinus. CONCLUSIONS: In conclusion, intracranial actinomycosis is difficult to diagnose, with potentially fatal outcome. An accurate diagnosis can often only be established by means of histology and biopsy shou …

Variations in the anatomy of the inferior alveolar nerve.

Variations in the anatomy of the inferior alveolar nerve were seen in 2 of the 20 dissections of the infratemporal fossa in 10 cadavers. A connecting nerve branch that originated from the auriculotemporal nerve joined the inferior alveolar nerve on both sides. The second part of the maxillary artery passed between the mandibular nerve, the root of the inferior alveolar nerve, and the connecting nerve branch which formed a loop. The maxillary artery seemed to be entrapped. Neurovascular entrapment can cause pain and numbness. Anatomical variations in this region should be kept in mind, particul…

Solitary fibrous tumor of the parotid gland extending to the parapharyngeal space.

Solitary fibrous tumors (SFT) arise in the pleura and less commonly in extrapleural sites. Head and neck regions have included the nose and paranasal sinuses, soft palate, epiglottis, thyroid, parotid and submandibular glands, as well as the infratemporal fossa and parapharyngeal space. We report a case of SFT arising from the parotid gland and extending to the parapharyngeal space. To our knowledge, this is the fourth case of SFT originating from the parotid gland and is the largest of its kind among the extrapleural lesions described. The characteristics revealed by computed tomography and m…

Rare presentations of ordinary lipomas of the head and neck: a review.

The ordinary lipoma is the most common neoplasm of mesenchymal origin. Only 13% of them arise in the head and neck region and most of these occur subcutaneously in the posterior neck. Rarely, they can develop in the anterior neck, infratemporal fossa, and in or around the oral cavity, pharynx, larynx, and parotid gland. Clinically, they can be confused with other benign lesions; however, CT allows a specific diagnosis to be made in virtually all cases. This article reviews the CT appearance and differential diagnoses of lipomas in these rare locations and discusses the rare infiltrating lipoma…

A different technique used for the closure of sigmoid sinus during infratemporal approach.

OBJECTIVE: Glomus jugulare tumors and neuromas frequently affect the jugular foramen area and necessitate special surgical approaches. It is often essential to stop blood flow by occluding internal jugular vein from below and the sigmoid sinus from above. Obviously, injury to the venous structures without proximal and distal control results serious haemorrhage. We describe here, a different technique for the closure of sigmoid sinus during infratemporal approach and compare it with the other techniques. METHODS: During the infratemporal approach, for closing the sigmoid sinus, we removed the

Five challenging presentations of juvenile nasopharyngeal angiofibroma: our experience in the diagnosis and endoscopic surgical management.

Juvenile nasopharyngeal angiofibroma (JNA) is a benign tumor originating from the pterygopalatine fossa and is the most common lesion of the nasopharynx. Its hypervascularity and complex anatomical extensions make selecting the most effective surgical approach intricate. All patients in this series were males, and their age ranged between 8 and 23 years. Extensions to the carotids, cavernous sinus, and infratemporal fossa were noted. In addition, intraoperative details with particular emphasis on the endoscopic two-surgeon transseptal approach were discussed. Nasal obstruction was the most rep…

Extended endoscopic approaches to the maxillary sinus.

OBJECTIVES: Treatment of inflammatory and neoplastic disease in the maxillary sinus, pterygopalatine and infratemporal fossae requires appropriate surgical exposure. As modern rhinology evolves, so do the techniques available. This paper reviews extended endoscopic approaches to the maxillary sinus and the evidence supporting each technique. METHODS: A literature search of the Ovid Medline and PubMed databases was performed using appropriate key words relating to endoscopic approaches to the maxillary sinus. RESULTS: Mega-antrostomy and medial maxillectomy have a role in the surgical treatme…

Ipsilateral keratoconus associated with long-standing primary hydatid cyst of the orbit.

Hydatid cyst is a cyclozoonotic infection of the larvae form of a platyhelminthes Echinococcus granulosus. The majority of hydatid cysts appear in the liver (65%) and lungs (25%). Kidneys and brain are other less common sites for this disease. Only 1% to 2% cases are seen in the maxillofacial region. These commonly appear as cystic lesions located in the mandible, maxillary sinus, orbit, infratemporal fossa, pterygopalatine fossa, parapharyngeal space, tongue, and parotid and submandibular salivary gland. Hydatid cysts of the orbit are rare and account for 1% of all hydatid cysts. The article …

The use of intensity modulated radiotherapy for the treatment of extensive and recurrent juvenile angiofibroma.

These case series are presented to describe the application and advantages of intensity modulated radiotherapy (IMRT) for the treatment of extensive and/or recurrent juvenile angiofibroma. Two patients were diagnosed with recurrence at 11 and 13 months postoperatively, and one was surgically unresectable. The affected areas included the base of skull, cavernous sinus, pterygopalatine fossa, infratemporal fossa, posterior orbit and nasopharynx. Highly conformal IMRT was delivered with limited radiation doses to the optic nerves, optic chiasm, brainstem, brain, spinal cord, lens, retina, mandibl…

Dumbbell-formed lipomas under the zygomatic arch.

Two cases of dumbbell-shaped lipoma under the zygomatic arch and the therapeutic courses are described. Both tumours were present in the buccal and temporal processes and the isthmus, which showed dumbbell forms occupying temporal and infratemporal fossae on coronal MR images. According to the location, the tumours seemed to be derived from a masticatory fat pad. The tumours were completely excised via a temporal and small cheek skin incision, with no postoperative complications. Over the 5-year follow-up period, both cases have shown good function and aesthetics, and no recurrences have been …

Anatomical relationship between the lateral pterygoid muscle and maxillary artery: Morphological classification and clinical implications.

BACKGROUND: The lateral pterygoid muscle is important for temporomandibular joint function and is closely related to the maxillary artery, which is clinically significant during procedures like botulinum toxin injections and infratemporal fossa surgery. METHODS: Thirty hemifaces from human cadavers were dissected. The study documented the number of muscle heads, insertion sites, and the course of the maxillary artery in relation to the muscle (superficial, deep, or transitional). Morphometric measurements included the distance between the maxillary artery and surface landmarks. RESULTS: A tw…

Imaging in Gingivobuccal Cancers.

The gingivobuccal space is a subsite of the oral cavity with strict anatomic boundaries. Although squamous cell carcinoma related to tobacco use is the most common malignant tumor occurring in this area, minor salivary gland tumors and sarcomas can also occur. Malignant lesions of this space have a less favorable prognosis than other subsites of the oral cavity. Tumors arising within the gingivobuccal space have unique patterns of invasion of adjacent anatomy, submucosal spread, invasion of the infratemporal fossa and skull base, perineural spread, and nodal metastasis. Appropriate treatment r…

How I do it: internal maxillary artery to middle cerebral artery bypass to manage giant thrombosed internal carotid artery aneurysm.

BACKGROUND: Internal maxillary artery (IMA) bypass has become popularized due to its medium-to-high blood flow, short graft length, and well-matched arterial caliber between donor and recipient vessels. METHOD: We described an open surgery of a NEW "workhorse," the IMA bypass, to treat a giant, thrombosed cerebral aneurysm. The extracranial middle infratemporal fossa (EMITF) approach was used to unveil the pterygoid segment of the IMA for cerebral revascularization. CONCLUSION: Although this technique is technically challenging, the variations in IMA can be effectively identified and suffici…
