Voices from the ice: exploring wellbeing and job performance in a Latin American Antarctic expedition.

This study examines wellbeing and job performance in Isolated, Confined, and Extreme (ICE) environments, using Antarctica as a case study. The research focuses on the logistical personnel of the XXVI Ecuadorian Antarctic Expedition, recognizing the distinct geographical and human dynamics involved. Its relevance stems from the absence of similar studies in Latin America and the critical role of human capital in the success of missions carried out under adverse conditions. A qualitative, cross-sectional, exploratory-descriptive design was employed. The purposive sample consisted of 17 members o

Spectrum of Cytogenetic and Molecular Abnormalities in Disorders of Sex Development: A Retrospective Observational Study From Western India.

BACKGROUND: Disorders of sex development (DSD) are a social and medical emergency. A thorough history, detailed clinical examination, and appropriate investigations will help to identify the aetiology of DSD. But still, in most cases, the exact aetiology is not reached. Reaching a definite and firm diagnosis with the help of genetic testing is vital for the long-term management of DSD patients. METHODS: We conducted a hospital record-based, retrospective cross-sectional study of 20 children with DSD aged 0-14 years who attended the paediatric endocrine clinic at our institute from February 20

Nutrition rehabilitation programs and cachexia clinics for anorexia-cachexia syndrome in patients with cancer.

The anorexia-cachexia syndrome (ACS) is characterized by loss of appetite and unintentional weight loss. Important clinical outcomes are associated with ACS including increased risk of chemotherapy side effects, decreased survival, and quality of life. Because ACS is driven by complex metabolic mechanisms and a chronic pro-inflammatory response, the weight loss and muscle wasting cannot be reversed by conventional nutritional supplementation alone. However, insufficient intake of calories and protein exacerbate weight loss experienced by patients with ACS, while physical inactivity accelerate

Traumatic Bladder Ruptures: A Ten-Year Review at a Level 1 Trauma Center.

Bladder rupture occurs in only 1.6% of blunt abdominopelvic trauma cases. Although rare, bladder rupture can result in significant morbidity if undiagnosed or inappropriately managed. AUA Urotrauma Guidelines suggest that urethral catheter drainage is a standard of care for both extraperitoneal and intraperitoneal bladder rupture regardless of the need for surgical repair. However, no specific guidance is given regarding the length of catheterization. The present study seeks to summarize contemporary management of bladder trauma at our tertiary care center, assess the impact of length of cathe

A multidisciplinary comparison of transfusion and perioperative support for high-risk cardiac surgery at three large academic centres in North America.

Cardiac surgery is associated with numerous peri- and post-operative haemostatic complications and blood transfusion requirements. Complex procedures such as redo-sternotomy heart transplantation or type A aortic dissection repairs are at high-risk for severe coagulopathy and significant transfusion requirements. However, current practice guidelines do not specifically address high-risk surgeries, resulting in variable practice. To optimise outcomes, a multidisciplinary approach to blood transfusion and haemostasis is critical. How individual institutions construct these multidisciplinary team

Creating a bowel management plan for pediatric orthopaedic spine surgery patients.

PURPOSE: Opioid-induced constipation is a common problem in patients who have undergone surgery. No standard gastrointestinal protocol exists to manage perioperative care in pediatric orthopaedic spinal fusion patients despite data which support the need for a bowel regimen while a patient is taking narcotics. At our institution, this group of patients often present to the emergency department with constipation and other gastrointestinal complaints. We developed a quality improvement initiative to create a standardized bowel management plan for pediatric patients undergoing orthopaedic spine s

Incidence of acute compartment syndrome with routine use of regional anesthesia for patients with long bone fractures: a large single-center retrospective review from a level I trauma tertiary academic institution.

INTRODUCTION: Traditionally, using peripheral nerve blocks (PNBs) in patients with long bone fractures has been limited due to concerns that it may interfere with the timely diagnosis of acute compartment syndrome (ACS). However, our large academic institution and level I trauma center have been using regional anesthesia routinely for pain management of patients with long bone fractures for more than a decade, with strict adherence to a comprehensive management protocol. The aim of this retrospective review is to present our experience with this practice. METHODS: Following Institutional Revi

Meningeal cryptococcosis in a pancreas transplant recipient requiring grafectomy: A case report.

BACKGROUND: Through continuous improvement in transplantation medicine, a wider range of solid organ transplant (SOT) recipients is considered suitable for complex procedures. Despite advances in modern transplantation practice, transpiring invasive fungal infections pose a substantial threat for SOT recipients. To our knowledge, cryptococcal infection confined amidst sole pancreas SOT recipients has not been described to date. Enforcement of a multidisciplinary transplant team approach in the management of pancreas SOT recipients presenting with complex cryptococcal complications is fundament
