Laparoscopic Appendectomy in Children With Perforated Appendicitis: A Meta-Analysis.

BACKGROUND: The purpose of this meta-analysis was to compare the influence of different surgical procedures on perforated appendicitis in pediatric population. MATERIALS AND METHODS: Studies were identified through a computerized literature search of Pubmed, Embase, Ovid, the Cochrane Colorectal Cancer Group Controlled Trials Register, the Cochrane Central Register of Controlled Trials, the Cochrane Library database, and Web of Science. Two reviewers independently assessed the quality of each study and abstracted outcome data. Data were extracted comparing laparoscopic appendectomy (LA) and o

Standardized irrigation technique reduces intraabdominal abscess after appendectomy.

PURPOSE: The utility of irrigation at the time of appendectomy for acute appendicitis has been debated, with recent studies showing no benefit to irrigation. In our practice, two techniques have been used; one in which irrigation was at the discretion of the surgeon, and one in which irrigation was standardized. The standardized irrigation technique involved large volume (3-12 l) irrigation in small, focused, directed aliquots to achieve optimal dilution. We sought to retrospectively assess whether the standardized large volume irrigation technique was associated with measurably reduced intraa

Refractory Cryptoglandular Perianal Abscess and Fistulas Due to Mycobacterium avium Infection in an Immunocompetent Adult.

Mycobacterium avium is a slow-growing nontuberculous mycobacterium (NTM) requiring prolonged treatment with multiple antimicrobials. It primarily affects immunocompromised patients and causes infection of the respiratory tract, skin, and soft tissue. While enteric carriage of M. avium has been reported, it has not been associated with clinical infection in immunocompetent hosts. To our knowledge, this is the first case report of a perirectal abscess caused by primary M. avium infection in an otherwise healthy patient and indicates the importance of considering NTMs as causative organisms in in

Persistent diffuse peritonitis in children.

In adults, persistent diffuse peritonitis (PDP) having a special microbiologic spectrum is now defined as a distinct intraabdominal infection because of its aggressive clinical course. Considering also other types of peritonitis, this study was performed to determine characteristics of childhood PDP in regard to clinical picture, microbiologic features, treatment and outcome. Classification of 175 patients with peritonitis showed that nine patients had primary peritonitis and 121, 37 and eight patients had secondary peritonitis, PDP, and intraabdominal abscess, respectively. Rates of host defe

Current advances in pharmacotherapeutic strategies for the treatment of complicated intra-abdominal infections.

INTRODUCTION: Complicated intra-abdominal infections (cIAIs) represent a major cause of surgical emergencies, arising from community-acquired conditions or heathcare-associated complications. This review, based on literature through 03/2026, from the PubMed (pubmed.ncbi.nlm.nih.gov/) and SMART (globalsmartsite.com) websites, explores strategies for implementing adequate anti-infective therapy in cIAIs. AREAS COVERED: cIAIs typically involve polymicrobial infections, including aerobic/anaerobic bacteria, and fungi. The rise of multidrug resistance, particularly among Gram-negative microorganis

Reactive arthritis incidence in a community cohort following a large waterborne campylobacteriosis outbreak in Havelock North, New Zealand.

OBJECTIVES: In August 2016, Campylobacter spp contaminated an untreated reticulated water supply resulting in a large-scale gastroenteritis outbreak affecting an estimated 8320 people. We aimed to determine the incidence of probable reactive arthritis (ReA) cases in individuals with culture-confirmed campylobacteriosis (CC), self-reported probable campylobacteriosis (PC) and those reporting no diarrhoea (ND). DESIGN: We conducted a retrospective cohort study to identify incidence of probable ReA cases. We identified cases with new ReA symptoms using an adapted acute ReA (AReA) telephone quest

Burden of Complicated Intra-Abdominal Infections in Children in Nigeria: Recent Experience and Systematic Review.

Background: Intra-abdominal infections encompass a wide range of conditions from uncomplicated appendicitis to fecal peritonitis. Little is known about the burden of complicated intra-abdominal infection (cIAIs) in children in low- and middle-income countries (LMICs). Method: This a report of recent experience and a systematic review of the burden in Nigeria. Results: Of 85 children with cIAIs, the most common primary cause was typhoid intestinal perforation (54; 63.5%) and complicated appendicitis (20; 23.5%). The complication rate after surgery was high, including surgical site infection (SS

Complicated Intra-Abdominal Infections: The Old Antimicrobials and the New Players.

Complicated intra-abdominal infections (cIAIs) are an important cause of morbidity and mortality worldwide. They are diagnosed when the initial abdominal organ infection has spread into the peritoneal space. Successful treatment relies on adequate source control and appropriate empiric antimicrobial therapy. Inappropriate antimicrobial therapy may result in poor patient outcomes and increases in healthcare costs. Current guidelines recommend several single and combination antimicrobial regimens; however, empiric antimicrobial treatment has been complicated by the increasing rates of resistant

Changing bacteriology of abdominal and surgical sepsis.

PURPOSE OF REVIEW: This review focuses on recent changes in epidemiological aspects of bacteria-induced intra-abdominal infections (IAIs), including the dominant pathogens, antimicrobial susceptibility profiles, and emerging resistance phenotypes. RECENT FINDINGS: Enterobacteriaceae species, including Escherichia coli and Klebsiella pneumoniae, remain the major pathogens contributing to abdominal sepsis, although Pseudomonas aeruginosa and Acinetobacter baumannii have recently become common causes of hospital-acquired IAIs. The prevalence of multidrug-resistant Gram-negative bacilli, especial

New onset fever in the intensive care unit.

Fever is defined as a core body temperature of >38.3 degrees C or 101 degrees F. About 50% of fevers in the ICU are due to infectious causes. Absence of fever in patients with infection heralds a poor prognosis. Temperatures between 102 degrees F-106 degrees F are more likely to be due to infection. The common infectious causes of fever are pneumonia, urosepsis, line infections and intraabdominal infections. Temperatures <102 degrees F or >106 degrees F are usually due to non-infectious causes like deep venous thrombosis, infusion reactions, aspiration, drug fever and the neuroleptic malignant…

Calcification within the urachus: A case report of rare presentation of urachal cyst with calculus formation.

Urachal anomalies are rare congenital lesions of the genitourinary tract and are important causes of pediatric and adolescent hospital presentations. It can mimic many other causes of intraabdominal pathology and fever, and pose diagnostic challenges, often aided by imaging, is crucial to prevent complications such as infection and calculus formation. Surgical intervention, preferably laparoscopic, is the primary treatment, with complete excision necessary to avoid potential malignant transformation.

Endoscopic and clinical features of gastric emphysema.

Gastric emphysema is characterized by the presence of intramural gas in the stomach without bacterial infection. Due to its rarity, most reports on gastric emphysema have been limited to single-case studies, and this condition's clinical and endoscopic features have not been thoroughly investigated. In this study, we analyzed 45 patients with gastric emphysema from 10 institutions and examined their characteristics, endoscopic features, and outcomes. The mean age at diagnosis of gastric emphysema in our study population (35 males and 10 females) was 68.6 years (range, 14-95 years). The top fiv
