[Inguinal hernia containing uterus and ovary in a young girl].

Inguinal hernias are a common finding in young children. In female patients, these hernias may contain the ovary in some cases, particularly in early childhood. In very rare instances, the uterus may also be found. We present the case of a 3-year-old girl with a left-sided inguinal hernia. During herniotomy, both the left ovary and the uterus were found to be adherent to the hernia sac. For this reason, a laparoscopic approach was chosen for safe evaluation and reduction. Awareness of the potential herniation of internal reproductive organs in young girls is essential, particularly in case of

Computed tomography diagnosis of omental infarction presenting as an acute abdomen.

Omental infarction is a rare cause of acute abdominal pain. Without the support of radiological evidence, diagnosis is difficult to attain owing to its infrequent incidence, low awareness among clinicians, and its nonspecific presentation that mimics other causes of acute abdomen, namely, acute appendicitis and cholecystitis. Incorrect diagnosis may lead to unnecessary invasive surgery in patients with omental infarction, a disorder that is typically managed conservatively without exposing the patient to intraoperative risks and postoperative morbidity. We report a case of a 61-year-old man wh

Massive perineal wound slough after treatment of complex pelvic and acetabular fractures using a traction table.

An operating table with the capacity for skeletal traction against a pudendal post is a helpful and commonly used piece of equipment for certain pelvic and acetabular fracture patterns. Perineal soft tissue trauma and urogenital injuries are associated with some pelvic and acetabular fractures. The perineal region is vulnerable to compromise after such injuries. There is little documentation available in the literature that addresses intraoperative complications of the perineum after the use of a traction table for pelvic fractures. We present here a case in which prolonged traction against a

Cataract surgery in anticoagulated patients.

A prospective study was performed on 31 patients having planned extracapsular cataract extraction with posterior chamber intraocular lens implantation. The patients were considered to be anticoagulated because of the medications they were taking. The patients were instructed to continue their usual medications throughout the perioperative period including the day of surgery. All patients had routine narcoleptic sedation and retrobulbar anesthesia. The surgical technique was altered to use an inferior corneal traction suture and a single planed clear corneal incision. No intraoperative or posto

Evidence of the Warburg Effect During Glioblastoma Resection Resulting in Severe Lactic Acidosis: A Case Report.

The Warburg effect describes the phenomenon in which cancer cells preferentially use glycolysis even under aerobic conditions, leading to excessive lactate production. Though more frequently described in hematologic malignancies, this mechanism can also be seen in solid tumors. Here we report a case of an 84-year-old man who developed a profound intraoperative lactic acidosis during glioblastoma resection, likely attributable to the Warburg effect. This case highlights the importance of maintaining broad differential diagnoses of lactic acidosis. Anesthesiologists, surgeons, and intensivists s

Esophageal Perforation Accompanying Mediastinitis in Blunt Trauma in a Patient with Thoracic Osteophytes.

We herein report a 61-year-old man who sustained injury after a 2-m fall and developed mediastinitis. He presented to another hospital two days after the fall and was transferred to our hospital four days after the fall with a fever and dysphagia. Computed tomography revealed osteophytes on the second and third thoracic vertebrae and free air in the mediastinum, indicating esophageal perforation. Emergent surgery was performed. Intraoperatively, a longitudinal esophageal tear was identified. We stress the importance of being aware of the possibility of osteophyte-related esophageal perforation

Eighteenth Century Complications With 21st Century General Anesthesia: A Case Report of Scurvy.

Autism spectrum disorder (ASD) may be associated with self-imposed dietary restrictions causing nutritional deficiencies, and the anesthesiologist must be aware of their potential clinical implications. Because humans are unable to produce ascorbic acid through endogenous metabolic pathways and children with ASD may avoid vitamin C-containing foods, vitamin C deficiency, or scurvy, may develop in patients scheduled for general anesthesia. Vitamin C deficiency may cause dysfunction in autonomic nervous system processes or pulmonary hypertension resulting in unexpected hemodynamic lability. We d

Bradycardic Arrest During Somatosensory-Evoked Potential Monitoring: A Case Report.

Intraoperative neurophysiological monitoring involves electrical stimulation of nerve pathways and is used to assess the integrity of both motor and sensory pathways. Whereas motor pathway monitoring involves transcranial stimulation and peripheral nerve monitoring, somatosensory monitoring involves peripheral nerve stimulation and cranial monitoring. Arrhythmias are a known complication of motor-evoked potential monitoring but have not been reported with somatosensory monitoring. We describe a case of bradycardic arrest with somatosensory-evoked potential monitoring using median nerve electro

Multicystic Mesothelioma of the Omentum Presenting as an Incidental Finding.

Multicystic mesotheliomas of peritoneal origin (also termed benign multicystic mesotheliomas) are uncommon and are exceptionally rare in the omentum. Lack of familiarity of this entity could lead to overinterpretation as a malignancy or present a challenge at frozen section. We describe a case of multicystic mesothelioma arising in the omentum found incidentally at laparotomy for endometriosis. Frozen section evaluation presented a challenge. Grossly, the mass in the omentum was multicystic and mucoid, raising the concern for a mucinous neoplasm. The cysts, however, were lined by flattened cel

Complete unilateral ureteral duplication encountered during intersphincteric resection for low rectal cancer.

Complete duplication of ureters is a very rare clinical entity that may either be asymptomatic or present with a variety of clinical findings. In the presented case a 51-year-old Caucasian female underwent an intersphincteric resection for low rectal cancer. Intraoperatively, during the standard bilateral recognition and mobilization of the ureters, complete unilateral duplication of the left ureter was incidentally detected, deriving from a single renal parenchyma. Such a congenital abnormality though constitutes a major risk-factor of accidental ureteral injury during operations including pe

Femoral artery thrombosis after internal fixation of a transverse acetabular fracture in a patient with osteogenesis imperfecta type I.

Osteogenesis imperfecta is a genetic disorder characterized by increased susceptibility to fractures and vascular injuries due to connective tissue fragility. In this case report, we present a patient with osteogenesis imperfecta type I who sustained a transverse fracture of the right acetabulum while transferring from bed to chair. The fracture was repaired through an ilioinguinal approach. During the surgery, an iatrogenic injury to the femoral artery and vein occurred. This intraoperative complication was salvaged by immediate vascular repair. We discuss the possible causes of iatrogenic va

Venous oxygen embolism produced by injection of hydrogen peroxide into an enterocutaneous fistula.

We report a venous oxygen embolism that occurred in a 66-yr-old man after 60 mL of 3% hydrogen peroxide was injected into a perianal fistula intraoperatively to locate its internal opening. The diagnosis was made after detecting hypoxemia, decreased end-tidal carbon dioxide tension, systemic hypotension, increased central venous pressure, and a new heart murmur. The patient recovered quickly and had no long-term sequelae. Oxygen embolism is a potentially fatal complication that can develop when hydrogen peroxide is used near venous spaces, and clinicians should be aware of the potential danger

Delayed Fatal Lumbar Artery Bleeding Following Less Invasive Posterolateral Decompression and Fusion.

STUDY DESIGN: A case report. OBJECTIVE: The aim of this study was to outline the potential risks of late bleeding from the segmental vessels following less invasively dorsolateral approaches to the thoracolumbar spine. SUMMARY OF BACKGROUND DATA: A low risk of major intraoperative bleeding, associated traditional dorsolateral approach to the lumbar spine is reported, but segmental arteries and great vessels may be rarely damaged. Spine surgeons who are involved with these approaches should be aware of this rare but potential dangerous vascular complication because it could be life-threatenin

Occult capsular injury in suspected traumatic and iatrogenic cataracts: a case report with imaging and pearls for surgical management.

Traumatic and iatrogenic cataracts may conceal occult capsular injury that routine slit-lamp examination and biometry fail to detect, increasing the risk of intraoperative complications. We present a mechanism-aware, imaging-guided approach to cataract surgery in an eye with suspected capsular compromise. An elderly man with a dense cataract and limited fundus view underwent preoperative imaging, which revealed a focal anterior capsular defect on ultrasound biomicroscopy, while posterior capsule integrity remained uncertain. His history included multiple intravitreal injections and prior anter

Left-sided perforated appendicitis in a 22-year-female with intestinal malrotation: A case report from a resource-limited setting.

INTRODUCTION AND IMPORTANCE: A left-sided appendicitis is rare and usually caused by congenital anomalies such as intestinal malrotation or situs inversus. Diagnosis can be particularly challenging in low-resource settings where imaging such as CT is unavailable. PRESENTATION OF CASE: A 22-year-old female presented with periumbilical pain that migrated to left iliac fossa area over five days. Due to the lack of CT imaging, clinical evaluation led to a diagnosis of acute abdomen. Emergency laparotomy revealed a perforated appendix located in the left lower quadrant due to intestinal malrotatio

Concomitant Thoracic Spinal Hemangioma and Dural Arteriovenous Fistula: Case Report and Technical Note.

Background and Clinical Significance: The coexistence of spinal hemangiomas and dural arteriovenous fistula (SDAVF) is uncommon. Unclear imaging and progressive neurological impairment require early surgical management. Case Presentation: A 76-year-old woman presented with progressive thoracolumbar pain and worsening bladder dysfunction. Magnetic resonance imaging (MRI) of the thoracic spine revealed a round-shape expansive lesion at T11 with spinal cord edema and homogeneous contrast enhancement. Despite a chronic presentation, the subacute progression of bladder dysfunction and spinal cord e

Hemostatic therapies and ambiguous abnormal bleeding in cardiac operating rooms: A real-world, case-based evaluation of clinical practices.

OBJECTIVE: Hemostatic therapy is commonly used in cardiac surgery. Despite tools to reduce inappropriate use, variability remains, mainly due to a lack of standards, especially intraoperatively. This study examined practices in cardiac operating rooms using a standardized clinical case. METHODS: French cardiac anesthesiologists and surgeons responded to a nationwide questionnaire, including a multiple-choice test based on a clinical case of ambiguous bleeding despite normal tests. The use of hemostatic therapies during surgery-reversal, chest closure, and transfer-was assessed with a fictitio

Intraoperative hypotension is a risk factor for postoperative acute kidney injury after femoral neck fracture surgery: a retrospective study.

BACKGROUND: Hip fracture in elderly patients is a serious health concern due to the associated morbidity and mortality. Although acute kidney injury after hip fracture is known to be a significantly poor prognostic factor for morbidity and mortality, the literature regarding the risk factors for acute kidney injury after hip fracture is insufficient. This study aimed to investigate the incidence and associated risk factors for acute kidney injury in patients with femoral neck fracture. METHODS: A total of 248 patients who underwent an operation for femoral neck fracture between January 2011 a

Laparoscopic Findings Associated with Bladder Endometriosis Are Correlated with Disease Severity.

STUDY OBJECTIVE: Bladder involvement is rare in endometriosis. The prevalence is estimated to be ∼1% in patients with endometriosis. However, this figure seems to be an underestimation. The diagnosis of bladder endometriosis might be missed during laparoscopy. The aim of this study was to determine an intraoperative visual score of pelvic lesions that help the surgeon suspect bladder endometriosis. Design, Design Classification: A retrospective analysis was performed on the intraoperative photographs and videos of 69 patients with histological confirmation of bladder endometriosis. SETTING: A
