An Unusual Cause of Intraoperative Hemodynamic Instability Complicating Elective Mastectomy With Immediate Free Flap Reconstruction: A Case Report.

Extrinsic compression of the heart consequent to intrapleural fluid is a rare cause of cardiac tamponade. Cases of massive hemothorax resulting in external cardiac tamponade due to injury of the internal thoracic artery (ITA) following blunt or penetrating trauma have been described in the literature. Here, we present a case of iatrogenic injury to the right ITA complicating mastectomy and deep inferior epigastric perforator flap reconstruction. It manifested as hemodynamic instability that persisted despite aggressive fluid resuscitation. Investigation with an intraoperative transesophageal e

Intraoperative breakage of Sachse's knife blade: A rare complication of optical internal urethrotomy (one case managing experience).

Optical internal urethrotomy (OIU) is the most common procedure performed for short segment bulbar urethral stricture worldwide. This procedure most commonly performed using Sachse's cold knife. Various perioperative complications of internal urethrotomy have been described in literature including bleeding, urinary tract infection, extravasation of fluid, incontinence, impotence, and recurrence of stricture. Here we report a unique complication of breakage of Sachse knife blade intraoperatively and its endoscopic management.

Massive right hemothorax as the source of hemorrhagic shock after laparoscopic cholecystectomy - case report of a rare intraoperative complication.

A 62-year old man was referred to our institution in hemorrhagic shock after a laparoscopic cholecystectomy for acute cholecystitis, performed at an outside hospital. A chest X-ray revealed a right-sided massive pleural effusion. Urgent surgical exploration was performed through a video-assisted mini-thoracotomy which revealed active bleeding from a pleural adherence. Successful hemostasis was achieved intraoperatively and the patient had an uneventful recovery. In absence of intra-abdominal hemorrhage, a hemothorax should be considered as a potential source of major bleeding in patients who d

Late complication in patients undergoing pancreatic resection with intraoperative radiation therapy: gastrointestinal bleeding with occlusion of the portal system.

BACKGROUND: There are few reports of late complications in patients who have undergone pancreatic resection with intraoperative radiation therapy (IORT), because carcinoma of the pancreas (PCa) and the bile duct (BCa) have a poor prognosis. The purpose of the present paper was to review gastrointestinal (GI) bleeding occurring with occlusion of the portal system (PVs) as a complication of IORT in patients surviving long term without recurrence. PATIENTS: From 1990 to 1999, 45 patients underwent surgical resection of the pancreas with IORT. Eleven of these patients survived >3 years without re

Intraoperative Breakage and Endoscopic Retrieval of a Dismantled Triprong Forceps During Ureterorenoscopic Lithotripsy: A Rare Complication and Review of the Literature.

Ureterorenoscopic lithotripsy (URSL) has become the gold-standard procedure for management of ureteric calculi. Ancillary stone-retrieval instruments such as triprong and biprong forceps facilitate fragment extraction; however, mechanical failure of these devices during active use is a recognized, albeit rarely documented, intraoperative hazard. We describe a 68-year-old woman who underwent left-sided URSL for a 1 cm upper ureteric calculus at the L4 vertebral level. During stone-fragment retrieval, the distal prongs of a triprong forceps sheared off within the ureter. The retained fragment wa

Intraoperative Abdominal Penetration of the Lag Screw: A Rare Complication During Proximal Femoral Nail Anti-Rotation Surgery.

Although the proximal femoral nail anti-rotation (PFNA) is accompanied by several benefits, using the nail is technically challenging and may pose some errors, leading to osteosynthesis failure. Here, we report a critical presentation of an uncommon side effect of PFNA surgery. Intraoperative abdomen penetration via lag screw is a unique complication of PFNA surgery of which surgeons should be aware.

Temporary sternoclavicular plating for an unusual double clavicle fracture (medial nonunion, lateral acute) complicated by an intraoperative pneumothorax.

Double (segmental) clavicle fractures, involving both the medial and lateral aspects of the clavicle, are very uncommon. Even less common is an asynchronous double fracture with one of the fractures being a nonunion. We report the case of a 30-year-old healthy male patient who had an unusual double clavicle fracture (medial nonunion, lateral acute) that occurred in separate traumatic events during motocross (motorcycle) racing. His fractures were treated surgically in two stages. In the first stage a long reconstruction plate was used that spanned onto the sternum and two transcortical screws

"Hepar divisum"--as a rare donor complication after intraoperative mortality of the recipient of an intended living donor liver transplantation.

In this study, we present our experience with 4 healthy donors having the rare condition of "hepar divisum" after the intraoperative death of the recipient of an intended right adult living donor liver transplantation (LDLT). The study included 4 donors and 4 intended right LDLT recipients affected by neuroendocrine tumor (n = 2), hepatocellular carcinoma (n = 1), and cryptogenic cirrhosis (n = 1). All 4 recipients died intraoperatively. At the time of recipient death, the dissection of the donor liver parenchyma was complete, the vessels intact, and the hepatic duct(s) already divided. In eac

Dual-view cross-semantic graph neural network for predicting intraoperative complications in patients with acute myocardial infarction undergoing percutaneous coronary intervention.

For acute myocardial infarction (AMI) patients undergoing percutaneous coronary intervention (PCI), accurate prediction of intraoperative complications (IOCs) in AMI patients undergoing PCI is critical for timely risk stratification and clinical intervention. However, existing approaches lack a patient-indicator dual-view synergy modeling framework, leading to semantic isolation between patient and indicator representations. Yet current risk prediction approaches often treat patient characteristics and clinical indicators as separate, disconnected pieces of information. What's missing is a uni…

A Systematic Review and Meta-Analysis of Postoperative Opioid Consumption, Pain and Complications Following Intraoperative Autonomic Nervous System-Based Nociception Monitoring.

BACKGROUND: Autonomic nervous system-based nociception monitors have been proposed to optimise intraoperative opioid administration. While they reduce intraoperative opioid use, evidence on postoperative outcomes remains limited. We conducted a systematic review and meta-analysis to evaluate their effects on postoperative pain, opioid consumption and complications. METHODS: MEDLINE, EMBASE, LILACS, Google Scholar, Cochrane and ClinicalTrials.gov were searched for randomised controlled trials comparing intraoperative nociception monitoring with standard care and reporting postoperative outcome

Effect of intraoperative hypothermia on postoperative pulmonary complications in elderly off-pump coronary artery bypass grafting patients: a retrospective cohort study.

OBJECTIVE: Intraoperative hypothermia has significant effects on physiological functions such as anti-infection and coagulation, but its association with specific postoperative pulmonary complications (PPCs) still remains unclear. The objective of this study is to explore the association between intraoperative hypothermia and PPCs. METHODS: This study retrospectively analyzed 311 consecutive elderly patients who received off-pump coronary artery bypass grafting (OPCABG) at Xuanwu Hospital between January 2019 and April 2024. Baseline characteristics, intraoperative data, anesthesia records, pe

Association between intraoperative inspired oxygen exposure and risk of postoperative pulmonary complications in abdominal surgery: a multi-center prospective cohort study.

PURPOSE: The impact of higher intraoperative oxygen administration increases the risk of postoperative pulmonary complications (PPCs) remains uncertain. This study aimed to investigate whether high supraphysiological oxygen exposure during abdominal surgery is associated with the development of PPCs. METHODS: We conducted a multi-center prospective cohort trial of adults scheduled for abdominal surgery under general anesthesia lasting at least two hours. Supraphysiological oxygen exposure was quantified as the area under the curve of the FiO2 above the 21% threshold while the corresponding Sp

Intraoperative acetabular periprosthetic fracture (IAPF) in total hip arthroplasty: a systematic review of outcomes and complications in conservative and surgical treatment.

BACKGROUND: Total hip arthroplasty (THA) is a widely performed and highly successful orthopedic procedure; however, intraoperative acetabular periprosthetic fractures (IAPF) remain a significant yet underreported complication. The increasing prevalence of THA, coupled with rising cases of osteoporosis, necessitates a better understanding of the incidence, risk factors, and long-term impact of these fractures. Despite advances in surgical techniques and implant designs, IAPFs present considerable challenges, affecting implant stability and patient outcomes. The aim of this study is to analyze t

Aggressive intraoperative warming and postoperative pulmonary complications in elderly patients recovering from esophageal cancer surgery: sub-analysis of a randomized trial.

BACKGROUND: Elderly patients having esophagectomies often become hypothermic which may promote complications. We tested the hypothesis that aggressive warming to a core temperature of 37°C reduces postoperative pulmonary complications (PPCs) in elderly patients having esophageal cancer resections. METHODS: This study was a pre-defined sub-study of a multi-center, parallel group, superiority trial (PROTECT). Patients aged >65 years and having elective radical resection of esophageal cancer in a single center were randomly allocated into either aggressive warming group (target intraoperative co

Severe Intraoperative Complications During VATS Anatomical Ressections and their Surgical Resolution in a Portuguese Thoracic Surgery Centre - A 9-Year Review.

INTRODUCTION: Many studies have demonstrated that video-assisted thoracoscopic surgery (VATS) is not only feasible and safe but is actually the approach chosen for an increasing number of pulmonary anatomic resections. There are however few studies reporting on severe intraoperative complications during VATS anatomical ressections and their resolution. OBJECTIVE: Our aim is to analyse the incidence of severe intraoperative complications during VATS anatomical ressections, at our department, in the past nine years, and describe their technical resolution during the surgery. METHODS: We perfor…

Association between intraoperative low renal pelvic pressure time and postoperative infectious complications after percutaneous nephrolithotomy: a single-centre retrospective cohort study.

BACKGROUND: Percutaneous nephrolithotomy (PCNL) is frequently complicated by infectious events, some of which progress to sepsis. Conventional pressure metrics are usually based on instantaneous values, such as peak or mean pressure, and therefore fail to capture the temporal dimension of pressure exposure. This study proposes “time at low pressure” as a novel exposure metric to evaluate its association with infectious complications after PCNL. METHODS: We conducted a single-centre retrospective cohort study including adult patients who underwent PCNL with continuous intraoperative renal pelvi
