Percutaneous Transhepatic Cholangiodrainage (PTCD)-Related Hepatitis B Virus Reactivation in Obstructive Jaundice.

Background: Obstructive jaundice is a common clinical condition, often caused by malignant tumors such as hepatocellular carcinoma (HCC) or cholangiocarcinoma (CCA). Percutaneous transhepatic cholangiodrainage (PTCD) is a widely used intervention method to relieve biliary obstruction in patients with obstructive jaundice; however, the impact of PTCD on hepatitis B virus (HBV) reactivation has not been thoroughly studied. Methods: A retrospective analysis was conducted from January 2016 to December 2024 on 235 patients with obstructive jaundice who underwent PTCD. Demographic, clinical, and pro

FGFR2 Regulates Liver Injury and Repair in a Model of Obstructive Jaundice.

BACKGROUND: The molecular mechanisms underlying obstructive jaundice (OJ)-induced liver injury and subsequent repair after biliary recanalization (R-OJ) remain incompletely elucidated. This study aimed to identify and validate core gene involved in regulating OJ-related liver injury and repair via integrated in vitro and in vivo models. METHODS: Animal models (OJ and R-OJ groups) and cell models (OJ serum (OJS) and OJ serum depletion (OJSD) groups) were established. Liver function and histopathologic changes were evaluated. Differentially expressed genes (DEGs) were screened by RNA sequencing

Endoscopic management of recurrent obstructive jaundice after endoscopic ultrasound-guided biliary drainage.

EUS-GUIDED biliary drainage (EUS-BD) has recently gained widespread acceptance as a minimally invasive alternative method for biliary drainage. However, the risks of encountering recurrent biliary obstruction (RBO) after EUS-BD have increased due to the growing clinical experience of EUS-BD and prolonged prognosis of the underlying disease. Previous studies have shown that the incidence of RBO following EUS-BD ranges from 11% to 25%. Nevertheless, literature on the efficacy of reintervention of RBO after EUS-GUIDED hepaticogastrostomy (EUS-HGS) and case reports describing the procedural detail

Lemmel syndrome: An unexpected clinical presentation of obstructive jaundice - A case report.

INTRODUCTION AND IMPORTANCE: Lemmel syndrome is a rare condition causing obstructive jaundice, resulting from the compressive effect of a periampullary diverticulum (PD) on the common bile duct (CBD). This syndrome is unique as it occurs in the absence of gallstones or tumors. Accurate diagnosis is critical to prevent unnecessary interventions. CASE PRESENTATION: We present the case of a 66-year-old man who developed generalized jaundice, significant weight loss, and epigastric pain. Diagnostic imaging, including ultrasound, computed tomography (CT), and magnetic resonance imaging (MRI), reve

Clinical evaluation of endoscopic biliary stenting in treatment of malignant obstructive jaundice.

BACKGROUND: Malignant obstructive jaundice (MOJ) is characterized by the presence of malignant tumors infiltrating or compressing the bile duct, causing poor bile drainage, generalized yellowing, pain, itching, and malaise. MOJ is burdensome for both the society and the families of affected patients and should be taken seriously. AIM: To evaluate the clinical effect of stent placement during endoscopic retrograde cholangiopancreatography for relieving MOJ and the efficacy of percutaneous transhepatic biliary drainage in terms of liver function improvement, complication rates, and long-term pa

Methotrexate-Associated Lymphoproliferative Disorder Presenting as Obstructive Jaundice Due to Pancreatic Involvement.

A 73-year-old man with a 13-year history of methotrexate (MTX) therapy for rheumatoid arthritis presented with jaundice and hepatic dysfunction. Contrast-enhanced computed tomography revealed hypovascular masses in the pancreas, small intestine, and thyroid lobes. A biopsy confirmed diffuse large B-cell lymphoma (DLBCL), leading to the diagnosis of MTX-associated lymphoproliferative disorder (MTX-LPD), a rare condition with pancreatic involvement. MTX was discontinued and R-CHOP therapy was initiated. Although a spontaneous regression of MTX-LPD can occur, early intervention may be warranted i

Gallstone ileus associated with obstructive jaundice and intestinal ischemia: a case report.

Typical gallstone ileus includes small intestine obstruction caused by the gallstone passing from the gallbladder through the common bile duct (CBD). The case of gallstone ileus with simultaneous obstructive jaundice and intestinal ischemia is an extremely rare scenario. Computed tomography of the abdomen and pelvis is the method of choice for the diagnosis. While some cases may be treated conservatively, surgery is often required, with the approach tailored to the patient's general condition and medical history. Hereby, we present a case of an 88-year-old lady with gallstone ileus, obstructiv

Percutaneous thrombolysis via cholecystostomy catheter to dissolve biliary clots causing obstructive jaundice.

Haemobilia, or bleeding within the biliary tree, is rare. It can cause biliary obstruction secondary to blood clots. A comorbid 87-year-old was admitted to hospital with acute cholecystitis, choledocholithiasis, and an Escherichia coli bacteremia. He had a partial pancreatectomy and gastrojejunostomy 35 years prior for severe pancreatitis. He was treated with antibiotics and a percutaneous cholecystostomy. He developed atrial fibrillation and was subsequently commenced on warfarin. He re-presented 5 days after discharge with abdominal pain and fevers. Liver function tests revealed cholestasis

Computed Tomographic Findings in Lemmel Syndrome: A Rare Cause of Obstructive Jaundice.

INTRODUCTION: Obstructive jaundice resulting from a duodenal diverticulum is known as Lemmel syndrome. Lemmel syndrome should be included in the differential diagnosis in patients presenting with obstructive jaundice in the absence of choledocholithiasis, mass, or a stricture. AIMS AND OBJECTIVES: To describe the computed tomography (CT) findings in patients with Lemmel syndrome. MATERIALS AND METHODS: Eighteen cases with incidentally detected peri-ampullary duodenal diverticula were retrospectively reviewed over a period of one year. Out of these, eight patients who presented to the hospita

Pancreatic Myeloid Sarcoma Causing Obstructive Jaundice: A Case Report and Literature Review.

Myeloid sarcoma (MS) is an extramedullary manifestation of acute myeloid leukemia (AML) and commonly occurs in sites such as the lymph nodes, skin, soft tissues, and bone. It more rarely manifests in the pancreas, with less than 20 cases reported in the literature since 1987. Despite its rarity, MS should be considered in the differential diagnosis of a soft tissue mass causing obstructive jaundice, especially if the patient has a known hematologic disease. Isolated cases of pancreatic MS have been known to progress to AML; therefore, it is crucial to differentiate MS from more common diagnose

Time-Related Evidence of Intestinal Oxidative Stress in Obstructive Jaundice-Induced Rats.

INTRODUCTION: Obstructive jaundice is known to affect intestinal permeability and facilitate bacterial translocation through related mechanisms. This study was conducted to evaluate the alterations concerning blood biochemistry and levels of several markers of oxidative stress (OS) in blood and intestinal mucosa caused by obstructive jaundice and how these fluctuate over time, in order to further explore the possibility of intervening in the OS path in future experiments. METHODS: A total of 54 albino Wistar rats were randomly divided into three groups (control, sham operated, and bile duct l

An Unusual Cause of Obstructive Jaundice and Acute Pancreatitis: Visceral Artery Pseudoaneurysm.

Visceral artery pseudoaneurysm (VAPA) is an uncommon vascular disorder with a tendency to present with nonspecific signs and abdominal symptoms. This case describes a patient with severe atherosclerosis who developed multiple VAPAs including a hepatic artery pseudoaneurysm bleeding into a large hematoma, which resulted in obstructive jaundice and acute pancreatitis. Prompt diagnosis of VAPA is important due to the high risk of vessel wall perforation with associated increased mortality rate. Biliary obstruction with acute pancreatitis is not a well-described presentation for VAPAs.

Diverticulum of common hepatic duct leading to obstructive jaundice, a case report.

Choledochal cyst is a dilation of the intrahepatic and/or extrahepatic biliary tree. The pathogenesis is unknown and potentially is multifactorial. In 1977, Todani classified the cysts under five different types according to their morphology, number and distribution along the biliary tree. Presenting symptoms of Choledocal cysts which include upper abdominal pain, acute cholangitis and jaundice, although often they are clinically silent and discovered as an incidental finding. Biliary complications include cholangitis, biliary stones, pancreatitis, portal hypertension and cholangiocarcinoma. W

A Simple and Easily Reproducible Model of Reversible Obstructive Jaundice in Rats.

BACKGROUND/AIM: Cholangiocarcinoma and pancreatic carcinoma are major malignancies that cause obstructive jaundice (OJ). This study aimed to develop a simple and easily reproducible rat model of reversible OJ (ROJ). MATERIALS AND METHODS: OJ was induced by clamping the common bile duct (CBD) using a U-shaped titanium hemoclip and its base was attached by ligation using 2-cm long 4-0 polypropylene suture. An anti-adhesive sheet was placed around the CBD. OJ was mitigated by pulling the suture to remove the clip under laparotomy 3 days later. Serum chemistry and liver histopathology were compar

Lemmel's Syndrome: A Rare Cause of Obstructive Jaundice Secondary to Periampullary Diverticulum.

UNLABELLED: In 1934 Lemmel was the first to report the presence of juxtapapillary diverticula and hepatocholangiopancreatic disease, excluding cholelithiasis. Obstructive jaundice caused by periampullary duodenal diverticulum (PAD) in absence of choledocholithiasis or tumor is known as Lemmel syndrome. A patient with an extraluminal duodenal diverticulum presenting with obstructive jaundice and pancreatitis is presented in this case. Although initially managed conservatively, the patient had recurrence of symptoms after 2 months but then had successful surgical resection of the duodenal divert

Hepatic peribiliary cysts with rapidly progressive refractory obstructive jaundice and esophageal varices.

A 54-year-old man with decompensated alcoholic liver cirrhosis presented with acute cholangitis. Although no localized lesions were detected in the liver on contrast-enhanced computed tomography and no risky varices were noted on endoscopy, hepatic peribiliary cysts (HPBCs) developed along the intrahepatic portal vein in the course of only 40 days. Moreover, esophageal varices with the red color sign grew rapidly during the same period, and the patient ultimately died due to rupture. HPBC formation is a rare complication of liver disease, including cirrhosis. Although HPBCs are generally harml

Obstructive jaundice secondary to Morgagni hernia in an infant with Fontan circulation.

The vast majority of congenital diaphragmatic cases present with severe respiratory distress in the immediate neonatal period. Obstructive jaundice secondary to CDH is a rare presenting symptom, previously unreported with Morgagni type hernias which comprise 2% of all CDHs. Although the safety of minimally invasive repair for CDH has been shown previously, there has been little data on its safety for patients with complex congenital cardiac disease. In this case report, we present an 18-month old infant with Fontan circulation presenting with jaundice secondary to Morgagni hernia who ultimatel
