[Ischaemic stump necrosis after subtotal gastric resection - a rare complication].

The development of gastric stump necrosis after subtotal gastrectomy with Roux-en-Y loop reconstruction and splenectomy in a patient with gastric cancer is described. The patient's life could be saved by re-operations on the 12th and 50th postoperative day. Due to the patient's poor general condition and local peritonitis the gastric stump was removed and a catheter-jejunostomy was prepared by using the Roux-en-Y loop. The aboral end of the oesophagus was closed. To relieve the oesophageal stump a cervical mucoso-oesophagostoma was carried out. As the patient's general condition improved the r…

Surgical management of severe acute pancreatitis.

The substantial morbidity and mortality associated with severe acute pancreatitis has led to a variety of therapeutic approaches. We reviewed the records of 40 patients who had undergone the "triple-tube" procedure (gastrostomy, cholecystostomy, and jejunostomy) with drainage of the lesser sac and retroperitoneum. Cardiovascular, respiratory, renal, and hepatic dysfunction were common, and reoperation was required in 35% (14) of the cases. Gastrostomies and jejunostomies were associated with numerous complications, and cholecystostomies were rarely of benefit except when biliary tract disease …

Creation of a neopylorus after pyloric exclusion using a "double-endoscope" technique.

OBJECTIVE: Pyloric exclusion may be implemented in the setting of a high-grade duodenal or pancreatic injury. After exclusion, the pylorus should spontaneously open in 3-6 weeks. However, we present the case of a critically ill 17-year-old male with a gunshot wound to the abdomen that underwent stapled pyloric exclusion with gastrostomy and jejunostomy tube placement who did not achieve pyloric patency after 5 months, and describe an innovative "double-endoscope" technique to correct it. METHODS: A gastroscope was inserted through the mouth into the stomach, and an endoscope was inserted retr…

Nurse supervised combined refeeding and home parenteral nutrition in traumatic intestinal failure: A case series.

BACKGROUND: Intestinal failure is a decrease in intestinal function under the minimum absorption requirements of macronutrients, water, and electrolytes. Hartman's procedure with jejunostomy is used as a surgical procedure to prevent further damage in cases of abdominal trauma. Providing parenteral nutrition at home is needed to prevent nutritional deficiencies and prolonged length of stay. PRESENTATION OF CASE: We reported two cases, involving two men aged 25 and 14 years old who had peritonitis due to abdominal trauma and received laparotomy. Both patients had Hartman's procedures and jejun…

[Mastitis with abscesses caused by esophageal fistula].

Mastitis originating from a fistula from intramediastinal esophago-jejunostomy following gastrectomy is an extremely rare event. We report on a 79-year old woman who had undergone repeated surgery due to recurrent breast abscesses for more than a year. The patient's history showed gastrectomy and esophago-jejunostomy two years earlier, with subsequent undetected insufficiency of the anastomosis and inflammation of the breast. The reason for the recurrent breast abscesses was found intraoperatively to be due to a fistula which could be followed to the anastomosis. Histologically, vegetable tiss…

Small bowel necrosis associated with postoperative enteral feeding.

Enteral feeding by jejunostomy is one of the main surgical procedures used to supply the proteins and calories necessary in the early postoperative period after major surgery of the upper digestive tract. The complications associated with early postoperative enteral feeding may vary from signs of gastrointestinal intolerance such as nausea, emesis, diarrhoea and cramp-like abdominal pain to hypotension and hypovolaemic shock, and also to the development of small bowel ischaemia and necrosis. Ischaemic intestinal involvement with progression towards necrosis is fortunately a rare event. The cau…
