Two pandemics intertwined around one patient: Interstitial pneumonia as the first presentation of HIV/AIDS, be it Pneumocystis jirovecii, cytomegalovirus, SARS-CoV2 or all?-A case report.

Concerns have been mounting regarding the underdiagnosis of HIV among respiratory co-infections associated with the COVID-19 pandemic. The delay in recognizing HIV/AIDS may be attributed to the similarities in clinical, laboratory (lymphopenia) and imaging presentations, which are typical for advanced AIDS but could also be indicative of a COVID-19 infection. Herein, we present a case of a 38-year-old ultraorthodox Jew with a late diagnosis of AIDS in the context of COVID-19 infection. This occurred after several months of recurrent respiratory infections compounded by SARS-COV 2 infection, du

Johannes C. Pompe, MD, hero of neuroscience: the man behind the syndrome.

Johannes Pompe is famous for describing type II glycogenosis, Pompe disease. However, Pompe's participation in the Dutch resistance during World War II has not been well described in the neurology literature. Pompe saved many Jews by hiding them as patients, saved a Jewish boy who was a neighbor, hid many young resistance fighters in his laboratory, resisted the Nazi call for all Dutch doctors to submit to their puppet physician's chamber, and hid a radio transmitter in the animal room of his laboratory. He was executed by firing squad in a German reprisal shortly before the end of the war. Po

The risk of developing Creutzfeldt-Jakob disease in subjects with the PRNP gene codon 200 point mutation.

We determined the penetrance of the PRNP 200Lys mutation in the large cluster of Creutzfeldt-Jakob disease (CJD) cases among Jews of Libyan-Tunisian origin living in Israel, utilizing data from 52 carriers with definite or probable CJD and 34 unaffected mutation carriers. A life table analysis was carried out with development of CJD as the end point. The probability of developing CJD rose with age, fitting a second-order regression curve (R = 0.97, p < 0.001). The cumulative penetrance reached 50% at the age of 60 and 80% at 80 years. Including seven elderly possible CJD patients in the analys

Familial Mediterranean fever (recurrent hereditary polyserositis) in Arabs--a study of 175 patients and review of the literature.

Recurrent hereditary polyserositis (RHP) or familial Mediterranean fever (FMF) is a chronic inherited illness of obscure aetiology. The disease is characterised by paroxysmal attacks of fever, peritonitis, pleuritis or arthritis, and predominantly affects Sephardic Jews, Arabs, Turks and Armenians. In this study, we report our 11-year experience of 175 Arab patients with this disease. As with other ethnic groups, the most common manifestation (93.7 per cent) was peritonitis. Arthritis (33.7 per cent) and pleurisy (32 per cent) were next in frequency. Adult patients in this series unlike those

Annual discourse--don't eat the quails.

Physicians were the first individuals recorded by name in history. Their attempts to define disease influenced fundamentally the cultures and religions of the world. Surgical skills, although highly developed in ancient times, appear to have been less well documented historically than medical disorders. Because of the greater threat to survival that diseases posed, they became incorporated into religious customs. Contagious diseases also influenced greatly the laws, traditions and historical events of the Bible. Leprosy provided a physical example that presumably represented an image of sin, b

Turning the lens in the study of precarity: On experimental social psychology's acquiescence to the settler-colonial status quo in historic Palestine.

This review examines the coloniality infused within the conduct and third reporting of experimental research in what is commonly referred to as the 'Israeli-Palestinian conflict'. Informed by a settler colonial framework and decolonial theory, our review measured the appearance of sociopolitical terms and critically analysed the reconciliation measures. We found that papers were three times more likely to describe the context through the framework of intractable conflict compared to occupation. Power asymmetry was often acknowledged and then flattened via, for instance, adjacent mentions of Is

Supply of medicines in the Terezín ghetto.

The article describes the supply and distribution of medicines in the Terezín (Theresienstadt) ghetto (1941-1945). The delivery of medicines for the ghetto took place in several ways. At the beginning of the ghettos existence, doctors brought the most medicines in their suitcases. Some of the drugs came from incoming prisoners (confiscated). Later, the German doctor Krönert from Litoměřice (Leitmeritz), supplied medicaments. The Prague Jewish community, with the help of a member of the SS Bartels, procured medicines, too. The drugs were also supplied by nearby pharmacies in Roudnice

A multicenter study reveals a novel pathogenic splice-site founder variant in OTOF.

BACKGROUND: Over 200 pathogenic variants in the OTOF gene encoding otoferlin are associated with sensorineural hearing loss (SNHL) and auditory neuropathy spectrum disorders (ANSD). RESULTS: A novel splice variant, OTOF c.898-18G > A, was detected in seven Jewish Moroccan families with both SNHL and ANSD in an Israeli multicenter study. The effect on splicing was confirmed by a minigene assay, which demonstrated skipping of exon 10. This variant is absent in gnomAD and in all Jewish ethnic groups except for Moroccan Jews, with a carrier rate of 1.5% in this ancestry, suggesting a founder effec

Fine-scale genetic structure and rare variant frequencies.

In response to the current challenge in genetic studies to make new associations, we advocate for a shift toward leveraging population fine-scale structure. Our exploration brings to light distinct fine-structure within populations having undergone a founder effect such as the Ashkenazi Jews and the population of the Quebec' province. We leverage the fine-scale population structure to explore its impact on the frequency of rare variants. Notably, we observed an 8-fold increase in frequency for a variant associated with the Usher syndrome in one Quebec subpopulation. Our study underscores that

Excavation of mass graves with Serbian context: Complexity of the political milieu.

This paper will discuss the complexity of the investigation of mass graves in today's Serbia (part of ex Yugoslavia), belonging primarily to WWII with an exception of one mass grave from WWI, through the prism of historical events behind it. By interpreting historical and political circumstances, we will show why and on what occasions mass graves, as cultural-historical monuments and symbols of the sufferings of Serbs, Jews, Romani, and other nations or ethnic groups, remained outside the focus of expert teams. Also, through examples of explored sites related to WWII (Višnjica, Topola, Kraguje

Illnesses of Herod the Great.

Herod the Great, Idumean by birth, was king of the Jews from 40 to 4 BC. An able statesman, builder and warrior, he ruthlessly stamped out all perceived opposition to his rule. His last decade was characterised by vicious strife within his family and progressive ill health. We review the nature of his illnesses and suggest that he had meningoencephalitis in 59 BC, and that he died primarily of uraemia and hypertensive heart failure, but accept diabetes mellitus as a possible underlying aetiological factor. The possibility that Josephus' classical descriptions of Herod's disease could be biased

Lipoamide dehydrogenase deficiency: a newly discovered cause of acute hepatitis in adults.

Lipoamide dehydrogenase deficiency is a rare disease, manifested in early childhood by lactic acidemia, progressive neurological damage and death in most cases. We report a case of lipoamide dehydrogenase deficiency in a 34-year-old Ashkenazi-Jewish woman. The deficiency manifested as acute hepatitis without cognitive impairment or acidosis. The patient's brother also had lipoamide dehydrogenase deficiency, diagnosed at the age of 20, and manifested as hepatocellular damage, lactic acidemia and myoglobinuria. We assume that the trigger for this hepatocellular damage was prolonged fasting, and

Mutations of the tyrosinase gene in patients with oculocutaneous albinism from various ethnic groups in Israel.

We have analyzed the tyrosinase (TYR) gene in 38 unrelated patients with oculocutaneous albinism (OCA), derived from several different ethnic groups of the diverse population of Israel. We detected TYR gene mutations in 23 of the 34 patients with apparent type I (i.e., tyrosinase-deficient) OCA and in none of the patients with other clinical forms of albinism. Among Moroccan Jews with type IA (i.e., tyrosinase-negative) OCA, we detected a highly predominant mutant allele containing a missense substitution, Gly47Asp (G47D). This mutation occurs on the same haplotype as in patients from the Cana

Antibodies to Epstein-Barr virus in subacute sclerosing panencephalitis patients.

Blood samples from 45 cases of subacute sclerosing panencephalitis in Israel were tested for the presence of antibodies to Epstein-Barr virus capsid antigen and early antigen by the indirect immunofluorescence method. Significantly higher titers were found in patients as compared to healthy controls with higher titers especially in Jews of Asian-African origin. Mean geometric titer was 1:42.8 in this group as compared to 1:10.8 in the control group. Antibodies to Epstein-Barr Virus early antigen were found in 50% of the patients and in none of the controls tested. Elevated titers of antibodies

The epidemiology of wrist and hand injury in two hospitals in Jerusalem: substantial differences between population subgroups.

BACKGROUND: Wrist and hand injuries are common and constitute a major economic burden. General injury prevention programs have failed to demonstrate a decrease in injury rates. We hypothesized that there are differences in injury patterns in culturally diverse subpopulations of a metropolitan area treated within the same medical system, which may partly explain the difficulties associated with injury prevention. METHODS: We conducted a survey of patients admitted to emergency departments of two hospitals in Jerusalem for wrist and hand injuries during a 3 month period. Patients were asked to

Breast feeding in Israel: maternal factors associated with choice and duration.

STUDY OBJECTIVES: To determine the influence of maternal characteristics on the incidence and duration of breast feeding. DESIGN: All the women who delivered in three obstetric wards within a two year period were surveyed. These three wards cover 93% of all births in the Jerusalem district. Women were interviewed on breast feeding of the previous child on the first or second day post partum by a research nurse. PARTICIPANTS: Altogether 8486 women whose previous pregnancy had resulted in a live born singleton who survived for at least one year. MEASUREMENTS AND MAIN RESULTS: Breast feeding i
