The Burden of Snakebite in Rural Communities in Kenya: A Household Survey.

Annually, about 2.7 million snakebite envenomings occur worldwide, primarily affecting those living in rural regions. Effective treatment exists but is scarce, and traditional treatments are commonly used. To inform context-specific policies in Kenya, this study aimed to determine the health-seeking behavior and the health, social, and economic burden of snakebites in rural communities. Nonprobability sampling was used to survey 382 respondents from four snakebite-endemic counties, from February to August 2020, using a structured questionnaire. Descriptive statistics, Fisher's exact tests, bin…

Tobacco Use and Risk Factors for Hypertensive Individuals in Kenya.

This study aimed to examine the association between hypertension and tobacco use as well as other known hypertensive risk factors (BMI, waist-hip ratio, alcohol consumption, physical activity, and socio-economic factors among adults) in Kenya. The study utilized the 2015 Kenya STEPs survey (adults aged 18-69) and investigated the association between tobacco use and hypertension. Descriptive statistics, correlation, frequencies, and regression (linear and logistic) analyses were used to execute the statistical analysis. The study results indicate a high prevalence of hypertension in association…

Qualitative Analysis of Community Support to Methadone Access in Kenya.

BACKGROUND: Methadone, as part of Medically Assisted Therapy (MAT) for treatment of opioid dependence and supporting HIV prevention and treatment, has been recently introduced in Kenya. Few low income settings have implemented methadone, so there is little evidence to guide ongoing scale-up across the region. We specifically consider the role of community level access barriers and support. OBJECTIVES: To inform ongoing MAT implementation we implemented a qualitative study to understand access barriers and enablers at a community level. METHODS: We conducted 30 semi-structured interviews with…

Zoonotic Pathogens of Dromedary Camels in Kenya: A Systematised Review.

Kenya is home to Africa's third largest population of dromedary camels, and production at commercial and local levels are increasingly important. In pastoral and nomadic communities in the arid and semi-arid lands (ASALs), camels play a vital role in food security, while commercial milk production and formalized export markets are rapidly emerging as camel populations expand into non-traditional areas. Until recently, little focus was placed on camels as hosts of zoonotic disease, but the emergence of Middle Eastern respiratory coronavirus (MERS-CoV) in 2012, and the discovery of exposure to t…

Analysis of Candida auris fungemia at a single facility in Kenya.

OBJECTIVES: Candida auris emerged as a human pathogen in 2009 and has subsequently been identified around the world as a cause of invasive candidiasis. We did an analysis from a single institution in order to analyze risk factors and outcomes for C. auris candidemia. METHODS: Patients with candidemia were identified by the electronic medical record and reviewed for risk factors and outcome. Candida isolates were identified by Vitek2 as Candida haemulonii, but species determinations for 21 of the isolates using published molecular and proteomic methods identified all as C. auris. FINDINGS: Fr

Nasopharyngeal Carriage of Streptococcus pneumoniae in Children in Coastal Kenya.

Streptococcus pneumoniae (SP) is a leading cause of child mortality globally, killing around half a million children aged 5 years and less per year. Nasopharyngeal carriage of SP is a prerequisite to disease, and the prevalence of colonization reaches 100% within the first few years of life. Serotype prevalence varies geographically, impacting the serotype coverage of pneumococcal vaccines, and serotype prevalence data are limited from large regions of the world, including sub-Saharan Africa. We enrolled 323 unvaccinated children, aged 4-7 years from coastal Kenya and obtained nasopharyngeal s…

The etiology and public health significance of mycobacteriosis of cattle in Kenya.

BACKGROUND: Zoonotic tuberculosis, commonly referred to as bovine, tuberculosis is mainly caused by Mycobacterium bovis subsp. bovis, a member of the mycobacterium tuberculosis complex (MTBC). Infection in humans resembles that caused by Mycobacterium tuberculosis. Mycobacteria Other Than Tuberculous (MOTTs) are also increasingly being associated with tuberculosis humans but reports of infections in animals are scarce, their zoonotic potential being largely ignored. The study aimed to determine the aetiology and public health importance of mycobacteriosis in slaughter cattle in Kenya. METHODS…

Lay knowledge and management of malaria in Baringo county, Kenya.

BACKGROUND: Malaria, a disease caused by protozoan parasites of the genus Plasmodium and transmitted by female anopheline mosquitoes, is a major cause of morbidity, mortality and loss in productivity in humans. Baringo County is prone to seasonal transmissions of malaria mostly in the rainy seasons. METHODS: This cross-sectional study used a mixed methods approach to collect data on knowledge and lay management of malaria. A questionnaire survey was administered to 560 respondents while qualitative data was collected through 20 focus group discussions in four ecological zones covering Baringo

Exploring the prevalence of disrespect and abuse during childbirth in Kenya.

BACKGROUND: Poor quality of care including fear of disrespect and abuse (D&A) perpetuated by health workers influences women's decisions to seek maternity care. Key manifestations of D&A include: physical abuse, non-consented care, non-confidential care, non-dignified care, discrimination, abandonment, and detention in facilities. This paper describes manifestations of D&A experienced in Kenya and measures their prevalence. METHODS: This paper is based on baseline data collected during a before-and-after study designed to measure the effect of a package of interventions to reduce the prevalen…

Epidemiology and Genomics of Invasive Nontyphoidal Salmonella Infections in Kenya.

BACKGROUND: In Kenya, invasive nontyphoidal Salmonella (iNTS) disease causes severe bacteremic illness among adults with human immunodeficiency virus (HIV) and especially among children <5 years of age coinfected with HIV or malaria, or who are compromised by sickle cell disease or severe malnutrition. The incidence of iNTS disease in children ranges from 166 to 568 cases per 100,000 persons per year. METHODS: We review the epidemiology of iNTS disease and genomics of strains causing invasive illness in Kenya. We analyzed a total of 192 NTS isolates (114 Typhimurium, 78 Enteritidis) from bloo…

Effects of wildlife and cattle on tick abundance in central Kenya.

In African savannas, large mammals, both wild and domestic, support an abundant and diverse population of tick ectoparasites. Because of the … ide-treated cattle on host-seeking tick populations in a long-term, exclusion experiment in central Kenya. Over seven years, we sampled larval, nymphal, and adult ticks monthly on replicated treatment plots that controlled for the presence of cattle and for the presence of two guilds of large wild mammals: megaherbivores (giraffes and elephants) and all …

Diet of Theropithecus from 4 to 1 Ma in Kenya.

Theropithecus was a common large-bodied primate that co-occurred with hominins in many Plio-Pleistocene deposits in East and South Africa. Stable isotope analyses of tooth enamel from T. brumpti (4.0-2.5 Ma) and T. oswaldi (2.0-1.0 Ma) in Kenya show that the earliest Theropithecus at 4 Ma had a diet dominated by C4 resources. Progressively, this genus increased the proportion of C4-derived resources in its diet and by 1.0 Ma, had a diet that was nearly 100% C4-derived. It is likely that this diet was comprised of grasses or sedges; stable isotopes cannot, by themselves, give an indication of t…

A study on the geophylogeny of clinical and environmental Vibrio cholerae in Kenya.

Cholera remains a significant public health challenge in many sub-Saharan countries including Kenya. We have performed a combination of phylogenetic and phenotypic analysis based on whole genome DNA sequences derived from 40 environmental and 57 clinical V. cholerae from different regions of Kenya isolated between 2005 and 2010. Some environmental and all clinical isolates mapped back onto wave three of the monophyletic seventh pandemic V. cholerae El Tor phylogeny but other environmental isolates were phylogenetically very distinct. Thus, the genomes of the Kenyan V. cholerae O1 El Tor isolat…

Prevalence and diversity of cystic echinococcosis in livestock in Maasailand, Kenya.

Cystic echinococcosis (CE) is a zoonotic disease caused by several members of the Echinococcus granulosus species complex. In East Africa, several species/strains are known to occur in livestock and humans, but host preferences, relative frequencies and spatial distribution of these taxa are poorly known. Here, we contribute livestock data for Maasailand of southern Kenya. Total CE prevalence was 25.8 % in cattle (151/587), 16.5 % in sheep (71/430) and 10.8 % in goats (21/194), which is a significant increase compared to surveys done about three decades ago. The majority of cysts occurred in t…

Feeding patterns and growth of term infants in Eldoret, Kenya.

BACKGROUND: There are limited longitudinal data from developing countries on early infant feeding and growth patterns. In Kenya only 34.8% of infants are exclusively breastfed at 2 months. This finding is of concern, and further understanding of infant feeding and growth patterns is important. OBJECTIVE: To determine the feeding and growth patterns of Kenyan term infants during early infancy. METHODS: A longitudinal study was conducted. One hundred and fifty-one resource-constrained mother-infant pairs were recruited from the West Municipal Health Centre (WMHC) within 24 hours after birth, a…

Patterns of drug abuse in public secondary schools in Kenya.

The objective of this study was to establish the association between substance abuse and the sociodemographic characteristics of secondary school students. All the students of 17 randomly stratified public secondary schools in Nairobi were required to complete self-administered sociodemographic and the School Toolkit questionnaires in a cross-sectional descriptive survey. Nearly all (96.6%; 1252/1296) the students, comprising more males (62.5%) than females, completed all the items on the questionnaires. Their mean age was 17 years. Alcohol and cigarette use were common and began as early as b

Potential contribution of mangoes to reduction of vitamin A deficiency in Kenya.

The β-carotene content of fresh and dried mangoes commonly consumed in Kenya was evaluated and converted to retinol equivalent (RE). Mango fruits of varieties Ngowe, Apple, and Tommy Atkins were harvested at mature green, partially ripe, and ripe stages and their β-carotene content analyzed. The stability of β-carotene in sun dried mangoes was also studied over 6 months under usual marketing conditions used in Kenya. The effect of using simple pretreatment methods prior to drying of mango slices on retention of β-carotene was as well evaluated. In amounts acceptable to children and women, fres…

Effect of rice cultivation on malaria transmission in central Kenya.

A 12-month field study was conducted between April 2004 and March 2005 to determine the association between irrigated rice cultivation and malaria transmission in Mwea, Kenya. Adult mosquitoes were collected indoors twice per month in three villages representing non-irrigated, planned, and unplanned rice agro-ecosystems and screened for blood meal sources and Plasmodium falciparum circumsporozoite proteins. Anopheles arabiensis Patton and An. funestus Giles comprised 98.0% and 1.9%, respectively, of the 39,609 female anophelines collected. Other species including An. pharoensis Theobald, An. m…

Determinants of teenage pregnancies: the case of Busia District in Kenya.

Sub-Saharan Africa has one of the highest levels of teenage pregnancies in the world. In spite of that, there is paucity of empirical research on causes of teenage pregnancies in African countries. This paper investigates the determinants of teenage pregnancies based on a case study of Busia District in Kenya. The data are from a household survey conducted in 1998/1999. Empirical results indicate that girls' education level has significant influence on the probability of teenage birth, with non-schooling adolescents and those with primary school level education being more vulnerable. Among the…
