Upper Urinary Tract Adenocarcinoma Arising in Tubular Adenoma-A Rare Entity: Report of 2 Cases and Review of Literature.

Tubular adenoma of the urinary tract is a recently recognized entity, and there is a paucity of clinicopathological information. We describe 2 rare cases of adenocarcinoma of upper urinary tract arising in the background of tubular adenoma with high-grade dysplasia. The first case was an extremely uncommon combination of multifocal tubular adenomas involving ureter and renal pelvis and adenocarcinoma of kidney in a patient with renal calculi. The second case was of adenocarcinoma of kidney arising from tubular adenoma of renal pelvis in the right moiety of horseshoe kidney. There was no micros

Semi-closed-circuit vacuum-assisted mini percutaneous nephrolithotomy in the pediatric population: the initial experience of two tertiary referral centers.

BACKGROUND: Percutaneous nephrolithotomy (PCNL) is the gold-standard for complex renal stones treatment in the pediatric population. While the miniaturization of PCNL reduces the risk of bleeding, it can hinder surgical and functional outcomes. The aim of the study was to assess the safety and feasibility of semi-closed-circuit vacuum-assisted Mini-PCNL (vmPCNL) in pediatric patients. METHODS: From January 2017 to December 2018, we prospectively collected data on consecutive vmPCNLs from two European tertiary referral centers. The procedure was performed with the ClearPetra® (Well Lead Medica

Evaluation of nephrolithiasis in autosomal dominant polycystic kidney disease patients.

BACKGROUND AND OBJECTIVES: Nephrolithiasis (LIT) is more prevalent in patients with autosomal dominant polycystic kidney disease (ADPKD) than in the general population. Renal ultrasonography may underdetect renal stones because of difficulties imposed by parenchymal and/or cyst wall calcifications. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: A total of 125 patients with ADPKD underwent ultrasonography and unenhanced computed tomography (CT) scan, routine blood chemistry, and spot and 24-h urine collections. RESULTS: CT scan detected calculi in 32 patients, including 20 whose previous ultr

Radiolucent Pure Matrix Stones on Computed Tomography Scan, Arising in Patient with Type I Diabetes and Chronic Kidney Disease: A Case Report.

Kidney matrix stones are a rare form of calculi, which are challenging to diagnose. Matrix stones consist of a proteinaceous material which has a radiolucent appearance that might be overlooked on imaging. Recently, endourological intervention has been the standard treatment method for matrix stones. We report a case of urinary matrix stones in a patient with type 1 diabetes mellitus and chronic kidney disease, in whom the stones formed into a pure matrix and were not visualized in the computed tomography scan. The stones were found after additional work-up, and they were managed using a trans

Endopyelotomy in horseshoe kidneys.

Three patients aged 23 to 43 years underwent endopyelotomy to treat stenoses of the ureteropelvic junction (UPJ) in horseshoe kidneys. One patient had failed open primary pyeloplasty and division of the isthmus previously. The other two patients had primary UPJ obstruction and small caliceal calculi associated with horseshoe kidneys. A hook-shaped cold knife was used to incise the ventromedial wall of the stenosis because of malrotation of UPJ in the horseshoe kidney. In two patients, a 14F endopyelotomy stent was left indwelling for 6 weeks. All three patients improved symptomatically, and tw

Unfavorable factors in accessing the pelvicalyceal system during retrograde flexible ureteroscopy (fURS).

Flexible ureteroscopy (fURS) is a well-established procedure for treating multiple upper-urinary tract pathologies, particularly renoureteral lithiasis. Endoscopes have undergone significant advancements, including miniaturization, improved optics, and increased maneuverability. In addition, advancements in accessory instruments, such as the performance of laser fibers, guidewires, and extraction probes, have played a significant role in improving the overall performance of flexible ureteroscopy procedures. However, despite these advancements, unique circumstances can make achieving optimum re

Characteristics of renal papillae in kidney stone formers.

The mechanism of kidney stone formation is not well understood. In order to better understand the pathophysiology for specific kidney stone compositions and systemic diseases associated with kidney stones, endoscopic papillary mapping studies with concurrent biopsies have been conducted. This review will summarize the findings of these studies and proposed mechanisms for thirteen disease processes associated with kidney stones. A review of the literature was performed identifying thirteen studies that endoscopically mapped and biopsied renal papillae of different stone formers. These studies c

[Management of renal calix calculosis].

UNLABELLED: A FREQUENT PROBLEM: Lithiases in the urinary tract form first in the calices, raising complex problems of lithogenesis. As described by Randall, in males caliceal lithiases are often composed of calcium oxalate, generally as a monohydrate. In females, calcium phosphate stones are common, stones related to urease-producing germs are less frequent, and the bi-hydrated form of calcium oxalate predominates. UNPREDICTABLE NATURAL HISTORY: Caliceal lithiases can remain asymptomatic and are often discovered fortuitously. Symptoms include chronic lower back pain or urinary tract infection

Potential Risk of Calcified Nanoparticles for Recurrent Urinary Tract Infection after Minimally Invasive Percutaneous Nephrolithotomy for Renal Calculi.

Among urolithiasis, urolithiasis is a more extensive and difficult to treat disease. In recent years, with the development of endoscopic equipment and related surgical methods, a new treatment, namely minimally invasive percutaneous nephrolithotomy, appears and is widely used in the treatment of renal calculi. It has been developed for more than 50 years since its appearance. Its advantages are a high stone clearance rate, small surgical trauma, fewer intraoperative and postoperative complications, and a short recovery time for postoperative patients. It is widely used in clinical treatment. T

FLEXIBLE URETERORENOSCOPY AND LASER LITHOTRIPSY IN A PATIENT WITH A STONE IN THE TRANSPLANTED KIDNEY: A CASE REPORT.

We report a case of a 31-year-old patient with obstructive ureterolithiasis in a transplanted kidney, treated endoscopically with flexible ureterorenoscopy and laser lithotripsy. The patient presented with biochemical signs of acute renal failure and ultrasonographically detected hydronephrosis. Emergency nonenhanced computed tomography scan revealed an obstructive 5-mm stone in the ureter of the transplanted kidney with resulting hydronephrosis. The patient received a double J stent to relieve allograft obstruction. Since the stone size was deemed favorable for conservative treatment, the pat

Obstructing ureteral calculus causing massive hydronephrosis in a renal allograft.

Nephrolithiasis is a rare complication of renal transplantation. Patients with an obstructing calculus in a renal allograft often lack the usual renal colic symptoms, and therefore present with atypical symptoms. Treatment of obstructing calculi is imperative to prevent renal allograft failure and other complications. We report the case of a 46-year-old man who presented 28 years after renal transplant with renal failure and massive hydronephrosis secondary to an obstructing calculus.

[Experiences with a modified nephrostomy instrument kit in ultrasound-guided kidney fistulization].

During a period of 24 months, ultrasound-guided percutaneous nephrostomies were performed in 51 patients in the Hospital at Köln-Merheim. The procedure was carried out with a modified nephrostomy instrument manufactured by Angiomed, Ettlingen, according to our own design. The procedure was successful in 49 patients. In one case it had to be abandoned because of lack of patient co-operation. In one patient a pyonephrosis was found and the patient was treated surgically.

Hydronephrotic Kidney Previously Treated for Tuberculosis: Rare Primary Squamous Cell Carcinoma of Renal Pelvis Diagnosed by Fine-needle Aspiration Cytology.

Primary squamous cell carcinoma (SCC) of the renal pelvis is a very rare tumor often associated with renal calculi and chronic infections. There are only a few articles in literature which report renal pelvis SCC in kidneys treated for renal tuberculosis, diagnosed after nephrectomy. We report the case of SCC in a hydronephrotic kidney previously treated for tuberculosis, diagnosed by ultrasound (US)-guided fine-needle aspiration cytology (FNAC), prior to core biopsy and nephrectomy. Our report highlights the utility of FNAC and the need for a careful search for renal collecting system tumors,

A case of nephrocalcinosis associated with primary aldosteronism.

Herein, we report a 37-year-old man presenting with nephrocalcinosis associated with primary aldosteronism. Primary hyperaldosteronism is reported to facilitate urinary calcium excretion; however, renal calculi or calcinosis in this disorder has been rarely reported. The patient had renal dysfunction and calcification in the renal medulla on both kidneys. A kidney biopsy was performed. His renal dysfunction seemed to be mainly caused by hypertension and tubulointerstitial damage. Furthermore, von Kossa-positive stones were seen in some tubules. X-ray element analysis revealed that the stones w
