[Complications of knee arthroscopy].

BACKGROUND: Today, knee arthroscopy is one of the standard interventions performed by an orthopaedist and usually has a low potential for complications. PURPOSE: The surgeon should still be aware of possible problems, and be able to control and explain them to the patient in detail before surgery. MATERIALS AND METHODS: The possible relevant peri-, intra- and postoperative complications of knee arthroscopy are discussed. RESULTS: Evaluation of the patient's medical history and comorbidities is crucial to successful treatment, in addition to a correct diagnosis and indications with the assis…

Ultrasound-Guided Knee Procedures.

Most knee structures can be accurately targeted using ultrasound guidance. These structures are usually superficial, and the overlying soft tissues are mobile and compressible, facilitating excellent visualization with a high-frequency linear array transducer. The circumferential accessibility to the knee affords flexibility and often multiple procedural approach options. In most cases, an in-plane approach is easily achieved. Studies of ultrasonography-guided knee procedures have consistently shown high accuracy, and its use is particularly beneficial for obese patients, diagnostic injection …

Imaging of knee osteoarthritis.

New imaging modalities are broadening the possibilities in osteoarthritis (OA) research, and are offering new insights to help better understand the pathogenesis of this disease. Although knee radiographs are widely employed in epidemiological and clinical studies to assess structural pathology, joint radiographs provide limited outcome measures in knee OA, and other more valid, reliable and sensitive imaging modalities are now available. In particular, magnetic resonance imaging can directly visualize articular cartilage and other joint structures, such as bone and soft tissue, that are now r…

Complications in knee arthroscopy.

All simple arthroscopic procedures during 1999 through 2001 performed at Baerum community hospital were retrospectively examined. Procedures were excluded when being part of more complex procedures. A total of 876 procedures performed on 785 patients were left for examination. Complications were registered from the patient record and all received a written questionnaire or phone call. The answer was obtained from 97.6%. The overall complications rate was low, giving total of 5.00%. A total of 0.68% of the complications had therapeutic consequences. There were two superficial infections, one th

[Revision knee joint alloplasties].

In the presented paper the authors evaluate the results of treatment in patients following revision knee joint alloplasties due to aseptic loosening of the tibia element in the endoprosthesis. The clinical material from the years 2000-2003 consists of 12 patients aged from 68 to 73; namely, 10 women (83.5%) and 2 men (16.5%). The longest time between the original implantation of endoprosthesis into the knee joint and the diagnosis of the aseptic loosening of its tibia element was 6 years and the shortest 2 years. The longest time of follow-up in patients who had had the revision operation was …

Knee osteoarthritis and obesity.

OBJECTIVES: To assess the risk of knee osteoarthritis (OA) attributable to obesity, and the interactions between obesity and other established causes of the disorder. METHODS: We performed a population-based case-control study in three health districts of England (Southampton, Portsmouth and North Staffordshire). A total of 525 men and women aged 45 y and over, consecutively listed for surgical treatment of primary knee OA, were compared with 525 controls matched by age, sex and family practitioner. RESULTS: Relative to a body mass index (BMI) of 24.0-24.9 kg/m(2), the risk of knee OA increa…

Jumper's knee and ultrasonography.

Twenty-five patients, aged between 15 and 45 years old, who were athletically active, presented with jumper's knee (patellar tendinitis). By use of ultrasonographic examination, new and precise information was obtained that benefited the diagnosis, choice of treatment, and monitoring of the evolution of jumper's knee in our patients. In 18 of the 25 patients, only 1 knee was affected; in 7 of the patients, both knees were affected, thus making a total of 32 painful tendons. These 25 patients were compared with a control group of 15 healthy, athletically active subjects between the ages of 25 a…

Knee problems in children.

The paper describes physical examination of the knee and some of the knowledge functional anatomy necessary for diagnosis and management of knee disorders that occur in children. It includes brief descriptions of the diagnostic features of these conditions, as well as principles of conservative and surgical management.

Knee injuries in gymnastics.

A review of injury patterns to the knee joint in gymnastics seen at our sport medicine clinic over a six and one-half year period reveals the patellofemoral pain syndrome to be more than twice as frequent as any other type of injury. Sprains are second in frequency but first in order of problems requiring emergency surgical treatment. The intensity of training correlates with the various overuse syndrome encountered, and the perfection of intricate routines and tricks can lead to significant injury of ligaments and menisci.

Infected total knee arthroplasties.

Thirty infected total knee arthroplasties were investigated in 29 patients over an average interval of 42 months. Eleven infections began in the immediate perioperative period. Six developed from postoperative wound-healing problems. The remainder were late infections. Staphylococcus was found in 16 infections, gram-negative agents in five, mixed organisms in five, and other gram-positives in four. Sixteen knees were arthrodesed, six knees were treated by retention of the components, and two above-knee amputations and one resection arthroplasty were performed. Five patients had two-stage revis…

Traumatic chylous knee effusion.

A 47-year-old patient presented with a chylous knee effusion and traumatic infected skin lacerations. The diagnosis of septic arthritis was considered because of purulent looking joint fluid, spuriously high synovial fluid white cell count, and signs of acute knee synovitis. Synovial fluid lipid analysis showed increased total lipids due to high concentration of neutral lipids, mainly triglycerides, and repeated knee radiographs showed a small fracture of the tibial spine. Joint trauma with subchondral fracture can be associated with frank chylous effusion, which may mimic acute septic arthrit…

Below knee temporary prosthesis.

There are definite advantages in using temporary prostheses. Their use reduces the length of time spent in hospital by amputee patients because stump conditioning and gait retraining is virtually completed by the time the patient receives his permanent prosthesis.

Patellar tendinitis (jumper's knee).

A jumper's knee is an overload lesion of patellar or quadriceps tendon near its insertion at the lower or upper pole of the patella. If conservative treatment fails, an operation can be performed in Phase 3 where disabling symptoms, are present. The necrotic tissue in the patellar tendon is excised. The early results of this surgery are encouraging.
