Constrained total knee prostheses are used in knees with severe deformities and insufficiency of collaterals to provide stable and mobile knees. Dislocation after constrained knee prosthesis is an extremely rare and dreaded complication. When dislocation is associated with patellar tendon rupture, the management includes restoration of the extensor apparatus along with a stable knee. Repair of the patellar tendon is challenging due to poor soft tissue coverage in the area and a bulky repair can put tension on the wound closure. Ideal method of restoration of the extensor apparatus is a matter

The maximal isokinetic knee extension strength was measured in 20 patients before and after surgical correction of recurrent dislocations of the patella and the pain evoked during the recordings was rated. The preoperative muscle strength was significantly higher and pain was less severe than in a previously studied group of patients with chondromalacia or patellofemoral osteoarthrosis. Twenty months after patella realignment by a new surgical procedure including an extensive lateral release and anteromedial displacement of the tibial tuberosity by an oblique osteotomy through the anterior cre

INTRODUCTION: Patellar dislocation following total knee arthroplasty (TKA) is an uncommon but functionally significant complication. Its management requires careful evaluation of prosthetic alignment, patellofemoral mechanics, and soft tissue integrity. The aim of this study was to describe a surgical stabilization technique using a medial retinacular flap fixed to the patella with suture anchors, combined with controlled lateral retinacular release, and to analyze its clinical and functional outcomes.
MATERIALS AND METHODS: We conducted a retrospective case series between 2011 and 2023 in tw

Knee dislocation is an infrequent complication after rotating hinge total knee arthroplasty (RHTKA). The aim of the present study was to describe our experience with dislocating RHTKA in a consecutive prospective large series, its occurrence rate, causative mechanisms and to perform a review of available literature. In total, six dislocations were observed in 303 RHTKA procedures (NexGen RHK, ZimmerBiomet, Warsaw, Indiana, US) at a mean of 10 (range, 2 - 24) months after surgery. This results in a 2% dislocation rate, which is lower than the 3.1% cumulative rate reported earlier in literature.…

Knee dislocation following total knee replacement arthroplasty is a rare but serious complication. The incidence of dislocation following primary total knee arthroplasty with posterior stabilized implants ranges from 0.15 to 0.5%, and posterior dislocation after revision total knee arthroplasty is even rarer. Here, we report the case of a 76-year-old male who presented with posterior dislocation after posterior stabilized revision total knee arthroplasty.

Meniscal bearing dislocation while rolling over in sleep has never been reported in Oxford unicompartmental knee arthroplasty (UKA). This study reports two cases of meniscal bearing dislocation into the intercondylar ridge while rolling over in sleep. In the case of one patient, closed reduction of the bearing was performed, and the use of a knee brace was effective in preventing re-dislocation. In the second patient, closed reduction was possible; however, bearing dislocation was repeated. Therefore, revision surgery was performed by replacing the tibial component and using a thicker bearing.

Chronic lateral dislocation of the patella is a rare condition and acquired causes are usually secondary to knee trauma. The neglected chronic dislocation leads to progressive genu valgum and external tibial torsion deformities with subsequent gonarthrosis, which becomes painful and debilitating. There is no consensus regarding treatment of these patients, but total knee arthroplasty (TKA) is a useful therapy in cases of painful symptomatic gonarthrosis. Few reports have shown that subvastus approach and lateral release may be a valid option for TKA, since it allows the correction of valgus de

Peripheral nerve blocks have found increased popularity in providing prolonged post-operative analgesia following total knee replacement surgery. They generally provide effective analgesia with fewer complications than epidurals. This report describes an acute low-energy knee dislocation after a well balanced, fixed bearing, cruciate-retaining primary total knee replacement performed under a spinal anaesthetic with combined complimentary femoral and sciatic nerve blocks. The dislocation was not accompanied by neurovascular compromise. Due to the subsequent instability and injury to both collat…

Knee dislocations are rare injuries in any age group, but even more unusual in skeletally immature individuals. Such injuries often occur from high-energy mechanisms and are commonly associated with disruption of both anterior and posterior cruciate ligaments. Although there are several previous reports of knee dislocation without disruption of the posterior cruciate ligament, there is only one report citing 3 cases of knee dislocation with the anterior cruciate ligament remaining intact, each occurring in skeletally mature individuals. We present a high-energy knee dislocation in a skeletally…

BACKGROUND: We report a rare complication of posterior dislocation of a cruciate-retaining total knee arthroplasty following an acute bacterial infection. The mechanism of dislocation proved to be septic loosening of the femoral component and a tear of the posterior cruciate ligament near to the femoral insertion site. The tear arose during the treatment of acute septic arthritis following total knee arthroplasty when the patient attempted full weight-bearing with the affected limb in a semiflexion position and twisted the knee.
METHODS AND RESULTS: Successful treatment was provided with subs

Instability of the knee joint, particularly in combination with the loss of one meniscus, regularly leads to the early development of arthritis. This paper describes the case of a 19-year-old male with ruptures of the anterior (ACL) and posterior cruciate ligament (PCL) along with the loss of the medial meniscus due to knee dislocation. Combined, time-delayed reconstruction of both the ACL and PCL and the allogenic fresh meniscal transplantation of the medial meniscus without bone plugs were performed. The control arthroscopy performed 6 months post-transplantation revealed good vitality and i…

An increased incidence of dislocation is the most important potential disadvantage introduced by the use of meniscal-bearing prostheses. The aim of this in vitro study was to measure the resistance to dislocation of a meniscal-bearing total knee arthroplasty in various circumstances and to establish which anatomic structures contribute to bearing stability. The prosthesis was implanted into cadaver knee specimens mounted in a 6 df rig. Dislocation was provoked by applying anteriorly or posteriorly directed forces (20-100N) to the tibia in the plane of the tibial plateau. Dislocation was define

INTRODUCTION: Knee joint dislocation is a rare and limb-threatening injury, which is often missed due to its association with other life-threatening injuries and is particularly challenging to manage in a low-resource setting. Our study is a longitudinal observational study evaluating the functional recovery trends following knee joint dislocation in a tertiary care hospital in the Himalayan Region of India.
MATERIALS AND METHODS: This was a longitudinal study conducted from September 2022 to June 2024. It included a total of 28 patients who presented to the hospital with a dislocated knee jo

CASE: We describe the clinical course and treatment of a patient who sustained simultaneous bilateral knee dislocation under low-velocity atraumatic conditions, and provide a review of the literature. Dislocations of the native knee joint are uncommon orthopaedic injuries but they are true emergencies because of the concern for concomitant neurovascular injury; therefore, they may be limb-threatening injuries.
CONCLUSION: To our knowledge, there are few reports of simultaneous bilateral knee dislocation and no reports of this occurring during weight training. The risk of knee dislocation can …

Complete knee dislocation is a rare injury and an associated incidence of popliteal artery damage ranges from 16-60% of cases. It occurs commonly in road traffic accidents and in high velocity trauma where significant contact remains as the usual mode of injury. We describe a rare case of non-contact knee dislocation with popliteal artery injury sustained while practising Aikido, a type of martial art. This patient successfully underwent closed reduction of the knee with an emergency vein bypass graft. Similar injury in association with Aikido has not been described in the English literature p…

OBJECTIVE: Complete knee dislocation is a rare injury. The purpose of this study was to evaluate the spectrum of injuries that are depicted by MR imaging in patients with a dislocation of the knee and to determine if there is any predictive factor that might indicate those patients who may be at risk for popliteal nerve injury.
MATERIALS AND METHODS: A retrospective search for patients sustaining traumatic knee dislocations who had radiographs and an MR imaging examination of the knee as part of their initial evaluation was done at three level I trauma centers for the period between 1989 and …
